Basal-like and HER2+ breast cancers have a high propensity to spread to the brain. The mechanisms that allow cells to colonize the brain are unclear. Methods and objectives: We have analyzed matched and unmatched samples of breast cancer and brain metastases using morphology, immunophenotype and expression profiling and validated the data using in vitro cell coculturing of breast cancer cells and neuronal cells and an in vivo mouse model using intracranial injections. Results: Most of the brain metastases were triple negative and ‘basallike’. Metastatic cells to the brain over-expressed HER3 compared to primaries and showed evidence of higher activation of elements of the MAPK pathway. Inhibition using anti-neuregulin antibody, trastuzumab and lapatinib reduced tumor growth in vitro and in vivo. Conclusions: The majority of the samples has a basal phenotype. The gene expression, in vitro and in vivo data also demonstrated activation of neuregulin-HER3 pathway in brain metastases from breast cancer and suggested that even in absence of HER2 amplification (e.g. triple negative/ basal cancers), anti-epidermal growth factor receptor family inhibitors may have a role in treating these patients.

It is the main cause of mortality in the female population. In the face of the impossibility of primary prevention measures that impede the development of breast cancer, efforts have been directed at scheduling secondary prevention measures through early detection. Early diagnosis and timely initiation of treatment may alter staging and consequently prognosis. It is noteworthy that the physicians of the Family Health Strategy (FHS) play a fundamental role in the information and awareness of women for the early detection of the disease. Objectives: To verify the opinion about the practices of breast cancer care in FHS physicians in the Municipality of Santos. Method: This is a research with a qualitative approach, with physicians from ESF teams in the municipality of Santos with more than one year of operation. Data were collected from July to August 2016 through semi-structured interviews. The analysis of the data was performed by the technique of content analysis proposed by Bardin. Results: Of the 21 invited physicians, 17 interviews were carried out. Units in operation for more than a year are located in the Morros, Northwest Zone and Continental Area. There was a similar proportion between men and women and the age varied between 29 and 72 years. Most are foreigners participating in the Mais Médicos Program. The totality of doctors knows the practices recommended in women's health, and most of them practice them, and there is no great difficulty in carrying out the requested tests. Doctors know the specialty reference (secondary care) and refer referral patients. The issue of lack of health education was pointed out as a difficulty, as well as the need for a specialist (Gynecologist) in family health.: The creation of new NASFs and the increase of gynecologists and pediatricians in the teams should be the proposal to strengthen the FHS in the city of Santos, providing better resolution of cases in Primary Care. Intensifying surveillance in women's health may be a strategy for early detection of breast cancer.

Métodos: Estudo prospectivo randomizado em que foram elegíveis mulheres com câncer de mama em uso de endocrinoterapia adjuvante (tamoxifeno ou anastrozol) com sinais e/ou sintomas SGUM de intensidade moderada a grave, de acordo com a escala visual analógica (EVA ³ 4). Após a assinatura do termo de consentimento, as pacientes preencheram os questionários FSF-I (Female Sex Function Index) e IQo-L (Incontinence Quality of Life Instrument) e se submeteram ao exame ginecológico, com determinação do índice de saúde vaginal (ISV). A seguir, foram randomizadas em três grupos de tratamento: CO2L (n = 32), RF (n = 33) ou CT (n = 30). Nos grupos CO2L e RF, foram realizadas três sessões mensais para aplicação das energias na região vulvovaginal. O grupo CT fez uso domiciliar de 1g promestrieno creme vaginal, por 21 noites consecutivas, seguidas por aplicações duas vezes por semana, por quatro meses. O seguimento foi realizado no quarto e décimo mês após o início das terapias, com avaliação dos mesmos parâmetros aferidos inicialmente, bem como do grau de satisfação pós-tratamento por meio da escala de 5 pontos de LIKERT. Resultados: Foram elegíveis 95 pacientes, cinquenta e oito concluíram o seguimento de dez meses, 21 no grupo CO2L, 19 no RF e 18 no CT. Após as terapias, em todos os grupos avaliados, houve melhora significativa da sintomatologia expressa pela redução da nota do EVA (p<0.05), melhora no ISV (p < 0.001). Além disso, houve melhora da função sexual, demonstrada pelo aumento da pontuação do questionário FSF-I (p < 0.001). O questionário IQO-I revelou melhora persistente dos sintomas urinários no grupo das energias, que foi observada no grupo CT apenas durante o uso do promestrieno (p= 0.004). Conclusão: O tratamento com energias (CO2L e RF) e com o promestrieno mostrou melhora significativa e semelhante dos sintomas da SGUM, da saúde vaginal e da qualidade de vida em mulheres com câncer de mama em uso de endocrinoterapia adjuvante. Não foram observados dano tecidual, estrutural ou complicações clínicas relevantes. O uso das energias mostrou-se seguro neste grupo de pacientes desde que respeitados os padrões técnicos.

Objetivo: Examinar os indicadores de fadiga e nível de aptidão física em mulheres sobreviventes de câncer de mama e praticantes de remada. Métodos: As participantes do estudo foram avaliadas por meio do Questionário Internacional de Atividade Física (IPAQ) e do Questionário EORTC QLQ-FA12, um instrumento utilizado para analisar a percepção de fadiga em pacientes com câncer.

Nos últimos anos, os efeitos de exercícios, incluindo a prática da remada e a sua preparação têm sido estudados e mostrado resultados positivos para essa população. Objetivo: analisar os indicadores cardiorrespiratórios de mulheres que, após o tratamento de câncer de mama, adotaram a prática de remo como atividade física. Sujeitos e Método: Mulheres que passaram pelo tratamento do câncer de mama que praticam atividade física regular em preparação para a remada foram submetidas a avaliações como o Questionário Internacional de Atividade Física (IPAQ) e o Teste de Caminhada de 6 Minutos (TC6) para verificar variáveis com frequência, duração e intensidade de atividades, distância percorrida e consumo máximo de oxigênio (VO2máx). Resultados: A média de tempo de atividades totaliza 586 minutos/semana, com um desvio de ± 270, equivalente a aproximadamente 83,7 minutos de atividade diária sem considerar a intensidade da atividade. Enquanto isso, a média de distância percorrida pelas sete participantes do estudo foi de 501,3m (± 41,2m) com média de consumo máximo de oxigênio de 14,3 ml/kg/min ± 2,14 e de consumo metabólico de 4,1 METs ± 0,61. Conclusão: É evidente que os melhores resultados surgiram daquelas que eram mais assíduas na prática física e em treinos mais vigorosos, porém ainda há muita oportunidade de desenvolvimento do condicionamento cardiorrespiratório para essas mulheres. Atividade física frequente e intensa é essencial para garantir a qualidade de vida dessas mulheres.

Nitric oxide (NO) is a gaseous free radical produced by organisms distributed through all phyla, acting as a signaling mediator in various cell types. NO-mediated cellular signaling occurs through post-translational modifications in proteins, responsible for physiological as well as pathological mechanisms. The manipulation of endogenous NO concentrations through administration of NO donors would be an interesting and viable method of therapy, with unexplored therapeutic potential. In physiological conditions, cells remain in a reducing environment, and its redox balance is maintained by antioxidant systems, such as the Thioredoxin-1 system (Trx-1), which has been shown to be overexpressed in several cancers and associated with aggressive tumor growth, resistance to conventional therapies, and decreased survival rates. Trx-1 is an appropriate molecular target for the study of the regulation of these processes, as well as in the development of new compounds that aim to alter the availability of this protein and of cell oxidants that may alter the cellular environment. Purpose: The objective of this study was to test the susceptibility of different cell lines to concentrations of S-nitrosothiols ortho and meta-chloro-SNOKs, in order to characterize them as compounds with antitumor activity that would exert their cytotoxic action preferentially on tumor cell lines. Methods: Four cell lines were used - two human breast cancer cell lines, one ER+ (MCF-7) and one ER- (MDA-MB-231); and two normal human mammary cell lines (HUVEC and MCF-10A). All cell lines were tested for viability assays, measurement of intra- and extracellular NO concentrations, cell death verification, and expression level of Pro-Caspase 3, PARP, cleaved PARP and Trx-1 proteins. Results: Differential toxicity of SNOKs to tumor lineages was demonstrated in cell viability assays and cell death assay. These results were correlated with higher intra and extracellular NO concentrations found in the tumor cell lines after incubation with the compounds. Differential toxicity was related to an xxiv apoptotic process due to a lower expression of Pro-Caspase 3 and Trx-1 proteins, and presence of cleavage of PARP protein. Conclusion: The studied compounds stimulate proliferation and/or maintain cell viability in the tumor cell lines when incubated at low concentrations, while inducing cell death at higher concentrations. These compounds presented molecular characteristics that make them as potential chemotherapeutic agents in the treatment of several types of cancer. Given the relevance of the results and potential chemotherapy to be explored further investigation of the mechanisms and pathways presented here would be of great clinical relevance in the future.

The occurrence of invasive lobular carcinoma within fibroadenoma is rare as well. DESIGN: Case report. CASE REPORT: A 58-year-old black female patient was referred to the Mastology unit of the Department of Gynecology, Federal University of São Paulo / Escola Paulista de Medicina, in February 1990, presenting an ulcerated tumor in the right breast with fast growth over the preceding six months. She was a virgin, with meno-pause at the age of 45 years and had not undergone hormone replacement treatment. The physical examination showed, in her right breast, an ulcerated tumor of 20 x 30 cm which was not adher-ent to the muscle level, multilobular and with fibroelastic consistency. The axillary lymph nodes were not palpable. The left breast showed a 2 x 3 cm painless, movable nodule, with well-defined edges, and fibroelastic consistency. We performed left-breast mammography, which showed several nodules with well-defined edges, the largest being 2 x 3 cm and exhibiting rough calcification and grouped microcalcifications within it. The patient underwent a frozen biopsy that showed a malignant variant of the phyllodes tumor in the right breast and fibroadenoma in the left one. After that, we performed a total mastectomy in the right breast and an excision biopsy in the left one. Paraffin study confirmed the frozen biopsy result from the right breast, yet we observed that in the interior of the fibroadenoma that was removed on the left, there was a focal area of invasive lobular carcinoma measuring 0.4 cm. The patient then underwent a modi-fied radical mastectomy with total axillary lymphadenectomy. None of the 21 dissected lymph nodes showed evidence of metastasis. In the follow-up, the patient evolved asymptomatically and with normal physical and laboratory examination results up to July 1997.

The main objective of this study is to describe cord frequency, characteristics and risk factors. Methods: Prospective cohort study of 173 women performed at the Breast Diseases division of Universidade Federal de São Paulo between July 2014 and September 2015. The data concerning socio-demographic status, anthropometric values, ROM, comorbidities, pain, cord frequency and characteristics (localization, number, thickness, if palpable and/or visible) in preoperative and 7, 15, 30, 60, 90 and 180 days after surgery, were collected. Results: Most cords appeared on the 7th postoperative day (66.1%) with a total incidence of 90.9% at the final evaluation (180 days). More than 70% of the cords occurred were palpable and 80% occur only in the axilla in any evaluation time. A correlation was observed between cord occurrence and reduced flexion and abduction shoulder ROM (p<0.001 and p<0.001, respectively). Hypertension (p=0.003) and axillary lymphadenectomy (p=0.029) increased the risk of developing cords. Diabetes decreased the risk to less than 30% (p=0.022). Conclusions: The first week after breast cancer surgery showed more frequent presence of AWS. The cords were palpable, thicker and more frequent in the axilla. Limited shoulder ROM was also associated with the cords. Axillary lymphadenectomy and hypertension were the main risk factors for development the AWS; diabetes lowered the risk. A careful and specific AWS evaluation seen in this study should explain its high incidence. The association and significance of hypertension (positive) and diabetes (negative) with AWS should be evaluated in future researches.
