Integração da Vigilância em Saúde e Atenção Básica na Zona Noroeste de Santos: desafios do controle de endemias

Introdução: O município de Santos enfrentou nos últimos anos o desafio de integrar as ações de Vigilância em Saúde e Atenção Básica, incorporando ações do Agente de Combate às Endemias (ACE) na Atenção Primária à Saúde. Esta proposta apresenta um desafio que exige a construção de estratégias que facilitem o processo de integração entre os Agentes Comunitários de Saúde (ACS) e Agentes de Combate às Endemias (ACE) na rede de Atenção Primária a Saúde, especialmente em regiões de maior vulnerabilidade social. Objetivo: investigar as possibilidades de integração do trabalho dos Agentes de Combate às Endemias (ACE) às equipes de Saúde da Família, principalmente quanto ao processo de trabalho em conjunto dos ACE e Agentes Comunitários de Saúde (ACS) na rede de atenção básica em saúde, especialmente em regiões de maior vulnerabilidade social, como a zona noroeste de Santos – São Paulo - Brasil. Método: foi realizada uma pesquisa qualitativa com entrevistas individuais, por meio de um roteiro temático de questões e de modo virtual, em função da Pandemia de Covid-19. Foi apresentada aos participantes uma proposta de oficina de integração entre o trabalho dos ACE e ACS, visando identificar sua opinião, críticas e sugestões. Os discursos dos participantes foram transcritos e categorizados pela técnica de Análise de Conteúdo temática. Aspectos éticos: o projeto de pesquisa foi aprovado pela Secretaria de Saúde de Santos (COFORM) e pelo Comitê de Ética da UNIFESP sob número CAAE 24301119.2.0000.5505. Resultados: foi identificado que o trabalho de controle de vetores na rotina da Estratégia de Saúde da Família atualmente ocorre de forma dissociada ao trabalho de intervenções de promoção à saúde e prevenção às doenças. As falas dos atores participantes que são mobilizadores sociais, direcionam para a necessidade de capacitações e planejamento de ações integradas entre o ACE e ACS, de modo a construir caminhos para se chegar a essa integração, fortalecendo assim as ações de Vigilância em Saúde junto às equipes de Saúde da Família. Considerações finais: no que tange ao processo de integração em si, há portarias e políticas públicas que são claras quanto à necessidade, não só de integrar o trabalho dos ACE com o dos ACS, mas, sobretudo, integrar ações de vigilância em saúde ao trabalho das equipes da Estratégia de Saúde da Família. Embora os participantes tenham ressaltado que a integração de atividades se mostra benéfica na prática, também referem que há dificuldades no processo de integração, o que evidencia a necessidade de ações de formação consistentes e continuadas.
Introduction: Santos city has faced in recent years the challenge of integrating the actions of Health Surveillance and Primary Care, incorporating actions of the Endemic Fighting Agent (EFA) in Primary Health Care. This proposal presents a challenge that requires construction of strategies that facilitate the process of integration between Community Health Workers (CHWs) and Endemic Fighting Agent (EFA) in the Primary Health Care network, especially in regions of greater social vulnerability. Objective: To investigate the possibilities of integrating the work of EFA in the Family Health teams, especially regarding the joint work process of the ACEs and Community Health Workers (CHWs) in the primary health care network, especially in regions of greater social vulnerability, such as the northwest of Santos - SP. Method: A qualitative research was carried out with individual interviews with a thematic script of questions. The interviews were carried out virtually due to the COVID-19 pandemic. During the interview, a proposal for an integration workshop between the work of the EFAs and CHWs was presented to the participant, in order to identify the respondent's opinion, criticism and suggestions. The speeches were transcribed and analyzed using thematic content analysis technique. Ethical aspects: participants signed a consent form according to CNS Resolution 422/12. The research project was evaluated and approved by the Health Department of Santos (COFORM) and by the Ethics Committee of UNIFESP under number CAAE 24301119.2.0000.5505. Results: it was identified that the vector control work has not been done as routine of the Family Health Strategy and has been done dissociated from the work of health promotion and disease prevention interventions. The participant’s speeches point to the need for training/planning of integrated actions between the ACE and ACS, in order to build paths to reach this integration, strengthening the Health Endemic Surveillance actions with the health family teams. Final considerations: regarding the integration process itself, there are ordinances and public policies that are clear about the need, not only to integrate the work of ACEs with that of ACS, but, above all, to integrate health surveillance actions into work of the ESF teams. Although the participants highlighted that the integration of activities proves to be beneficial in practice, they also mention that there are difficulties in the integration process, which evidences the need for consistent and continuous training actions.
