Resumo
Introdução: A avaliação objetiva da correção cirúrgica das assimetrias
mamárias deve considerar não somente o contorno final das mamas, mas também, a simetria de volume. A ressonância magnética e as medidas lineares são instrumentos de medida amplamente utilizados para se avaliar o volume e
o posicionamento mamário respectivamente. Objetivo: Avaliar a simetria
mamária pós-operatória através de ressonância magnética e medidas lineares
em pacientes com assimetria mamária. Métodos: Foram operadas 22
pacientes com assimetria mamária primária, as quais foram submetidas à
ressonância magnética e obtenção de medidas lineares no pré e no pósoperatório
com seis meses. As medidas lineares foram obtidas baseadas em
estruturas ósseas e mamárias bem definidas. O resultado de assimetria das
medidas lineares no pós-operatório foi avaliado através de escores que
poderiam variar de excelente a ruim. Foram obtidas imagens axiais de ambas
as mamas, sendo o volume mamário estimado a partir da reformatação
tridimensional destas imagens, através da fórmula Cavalieri. Resultados: As
pacientes com menor assimetria de volume no pós-operatório tenderam a
apresentar melhores resultados na avaliação das medidas lineares. Quando
realizada mastoplastia redutora e/ou de aumento bilaterais e mastoplastia
redutora e/ou de aumento unilaterais, a simetria de volume tendeu a ser maior.
Conclusão: A ressonância magnética e as medidas lineares são instrumentos
de medida capazes de avaliar a simetria pós-operatória em pacientes com
assimetria mamária
Introduction: The surgical treatment objective evaluation of breast asymmetry should not consider only final breast shape, but also breast volume. Magnetic resonance image and anthropomorphic breast measurements are used to analyze breast volume and breast position. Objective: Analyze breast symmetry after surgery using resonance magnetic image and linear measurements. Methods: 22 patients with primary breast asymmetry were evaluated preoperative using MRI and linear measurements and 6 months postoperative. Linear measurements were obtained based on well defined bone structures and mammary structures. The symmetry result for postoperative linear measurements was scored from excellent to poor. Axial images were taken from both breasts. Breast volume was estimated using MRI, based on 3D reconstruction, using Cavalieri formula and specific software. Results: Patients with less postoperative volume asymmetry tend to achieve best scores using linear measurements. When the same surgical procedure was performed bilaterally or only one breast was treated by surgery, greater volume symmetry was observed. Conclusions: MRI and linear measurements are methods capable of evaluate postoperative symmetry in patients with breast asymmetry
