O cuidado nas unidades básicas de saúde ao paciente com comportamento suicida

Introdução: No mundo o comportamento suicida representa uma demanda de saúde pública e um desafio para os profissionais de saúde na atenção primária. Objetivo: Compreender como se configura o cuidado à pessoa com comportamento suicida por profissionais no contexto da atenção primária à saúde, identificar suas atitudes e suas associações com variáveis sociodemográficas, acadêmicas e ocupacionais. Método: Dois estudos em Unidades Básicas de Saúde de município da grande São Paulo, um quantitativo transversal e correlacional e outro qualitativo. A amostra foi constituído de 192 respondentes de vinte unidades e aplicado o Instrumento de Avaliação da Assistência Profissional às Pessoas com Comportamento Suicida e avaliação sociodemográfica, sendo os dados compilados no programa Excel e SPSS. Variáveis categóricas descritas por frequências relativas e absolutas; variáveis contínuas por média, desvio padrão, mínimo, mediana e máximo. O coeficiente de correlação de Pearson e Spearman analisou os quatro domínios e as variáveis contínuas. Para análise dos domínios Percepção Profissional e Experiência Profissional entre as variáveis categóricas foram aplicados o teste de Mann-Whitney e teste de Kruskal-Wallis. Os domínios Conhecimento, Habilidade Profissional e Organização da Rede de Atenção, e as variáveis categóricas foram analisados pelo teste T e a ANOVA e o nível de significância de 5% (p-valor<0,05). Participaram de entrevista semiestruturada 23 participantes de três Unidades Básicas. Para a análise de dados foi utilizado o software MaxQDA 2022.2.0. Foram respeitados os preceitos éticos e o critério de inclusão foi o de ser profissionais da ativa. Resultados: A maioria tem menos de 50 anos, é do gênero feminino, nível superior e o tempo de experiência é de 10,71 anos. Menos da metade tem experiência nessa demanda. Apresentaram melhor desempenho nos domínios percepção profissional e conhecimento e habilidades. O estudo qualitativo destacou que os profissionais acolhem com uma escuta qualificada, veem a importância do trabalho interdisciplinar e da rede, fazem a avaliação de risco e incluem a família no cuidado. Mostram tensão, ansiedade, angustia e medo que o paciente cometa o suicídio e sentem-se pouco preparados para lidar com o assunto. Conclusão: Os estudos mostraram a importância de garantir uma aproximação dos profissionais sobre esse tema desde a graduação e em educação continuada, bem como uma maior organização dos serviços para promover uma escuta tanto dos profissionais como dessa população, pautada no acolhimento, vinculo e empatia.

Objective: To understand how care for people with suicidal behavior is conducted by Primary Health Care professionals, in order to identify their attitudes and associations with sociodemographic, academic and occupational variables. Method: We used two studies carried out at Basic Health Units in the Metropolitan Region of São Paulo, one quantitative cross-sectional and correlational and the other, qualitative. The sample consisted of 192 respondents from twenty units.The Evaluation of Professional Assistance to People with Suicidal Behavior instrument and sociodemographic assessment were applied, with data being compiled in Excel and SPSS. Categorical variables were described by relative and absolute frequencies. Continuous variables were described by mean, standard deviation, minimum, median and maximum. Pearson and Spearman’s correlation coefficient analyzed the four domains and continuous variables. To analyze the “professional perception” and “professional experience” domains among categorical variables, the Mann-Whitney test and the Kruskal-Wallis test were applied. The “knowledge”, “professional skill” and “organization of the care network” domains and categorical variables were analyzed by Student’st-test and ANOVA, with significance level of 5% (p-value<0.05). Twenty-three participants from three Basic Units participated in a semi-structured interview. For data analysis, the MaxQDA 2022.2.0 software was used. Ethical precepts were respected, and we included active professionals in the study. Results: Most are under 50 years old, female, with higher education and10.71-year work experience. Less than half have experience in this demand. They performed better in “professional perception” and “knowledge and skills”. The qualitative study highlighted that professionals welcome patients with qualified listening.They see the importance of interdisciplinary work and the network and carry out risk assessment and include the family in care. They show tension, anxiety, anguish and fear that the patient will commit suicide and feel unprepared to deal with the matter. Conclusion: The studies showed the importance of ensuring an approachto professionals on this topic since graduation and in continuing education, as well as a greater organization of services, to promote listening to both professionals and this population based on welcoming, bonding and empathy.
