Investigação sobre a implantação do serviço de terapia ocupacional em cuidados paliativos em hospitais gerais e especializados no Estado de São Paulo

Resumo
Ao longo dos séculos o hospital foi sofrendo transformações, mudando tanto o público
atendido como a sua função. Com o desenvolvimento científico da área médica, este local
acabou se tornando um espaço medicalizado. Assim o hospital passou a ser destinado a
promover e proteger a saúde, na recuperação e reabilitação de doentes. A Terapia
Ocupacional em contexto hospitalar tem atuado tentando dinamizar e facilitar a vida
ocupacional do paciente, utilizando das ocupações ou atividades como recurso ou instrumento
terapêutico. O profissional terapeuta ocupacional atua de modo a oferecer prevenção,
desenvolver programas de tratamento para a melhoria do estado de saúde e da qualidade de
vida do paciente, promove autonomia e independência necessárias para manutenção de sua
vida ativa, eliminando, reduzindo ou evitando os processos de exclusão, estando atento às
necessidades psicoafetivas e sociais do paciente e seus familiares e/ou cuidadores. A terapia
ocupacional em cuidados paliativos contribui para a qualidade de vida, visando o conforto, as
atividades significativas para o paciente e os projetos de vida que este paciente deseja realizar,
levando em consideração as suas capacidades e potencialidades. Este profissional auxilia
tanto o paciente, quanto a família e o cuidador. Assim com o objetivo de conhecer sobre a
implantação de serviços de terapia ocupacional em cuidados paliativos em hospitais gerais e
especializados realizou- se entrevista com quatro terapeutas ocupacionais que já participaram
de implantação deste tipo de serviço e trabalham em hospitais no estado de São Paulo. Usouse para tal uma entrevista semi-estruturada elaborada especificamente para esta finalidade. Os
resultados possibilitaram conhecer as áreas e as equipes em que os profissionais Terapeutas
Ocupacionais estão inseridos, verificar de onde vêm a demanda atendida nestes serviços e
avaliar quais foram às dificuldades encontradas pelos profissionais que implantaram serviços
de terapia ocupacional em alguma área dentro do hospital. Assim conclui-se que é importante
a terapia ocupacional estar envolvida com o processo de gestão, para dar conta de implantar
algum serviço, uma vez que a gestão está intrínseca em todas as falas das participantes. Sabese que a implantação de serviços é algo constante, pois esta profissão é recente e tem aberto
cada vez mais espaços com possibilidade do profissional terapeuta ocupacional estar enserido.

Thus, the hospital is now destined to promote and protect the health, recovery and rehabilitation of patients. Occupational therapy, in the hospital context, has worked trying to streamline and facilitate the occupational life of the patient, using occupations or activities as a resource or therapeutic tool. The professional occupational therapist works to offer prevention; develop treatment programs for the improvement of health status and quality of life of the patient; promotes autonomy and independence necessary to maintain your active lifestyle, eliminating, reducing or avoiding the processes of exclusion, being aware of the psychoaffective and social needs of the patient and their family members and/or caregivers. Occupational therapy in palliative care contributes to the quality of life, aiming comfort, meaningful activities and the plans that the patient want to accomplish, taking into account his abilities and potential. This professional helps both the patient and the family and caregivers. This way, in order to know more about the deployment of occupational therapy services in general and specialized hospitals, interviews were performed with four occupational therapists who have participated in establishing this kind of service and work in hospitals in São Paulo. A semi-structured interview designed specifically for this purpose was used. The results allowed to know the insertion of the professional occupational therapists in areas and teams, to verify what the source of demands for these services is, and evaluate which were the difficulties encountered by professionals who have implemented occupational therapy services in some area within the hospitals. Thus, we conclude that occupational therapy has an important role in the patient’s well-being and recovery. This profession is recent and can be inserted into different niches, bringing progressive benefits. Thus, it is important the involvement of occupational therapy in the management process, facilitating the implementation of the service and the insertion of the professional occupational therapist.
