- ItemSomente MetadadadosManagement of intraventricular meningiomas(Lippincott Williams & Wilkins, 2000-09-01) Braga, Fernando Menezes [UNIFESP]; Braga, Otavio Augusto Piccinillo [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Meningiomas are the most commonly occurring benign intracranial tumors, but an intraventricular location is rare. Inside the ventricles. the lateral one is the most common site for the tumor, almost always close to the atrium. Meningiomas occur less often in children, but a higher percentage are located intraventricularly. The authors review the literature on the management of intraventricular meningiomas and describe their own experience. Five cases of meningiomas within the lateral ventricles and one in the fourth ventricle are discussed, including two children tone of whom had tumors related to von Recklinghausen's disease).
- ItemAcesso aberto (Open Access)Meningeoma da goteira olfatória(Academia Brasileira de Neurologia - ABNEURO, 2006-03-01) Tella Jr, Osvaldo Inácio de [UNIFESP]; Paiva Neto, Manoel Antonio de [UNIFESP]; Herculano, Marco Antonio; Faedo Neto, Atílio; Universidade Federal de São Paulo (UNIFESP); Hospital Professor Edmundo Vasconcelos; Faculdade de Medicina de JundiaíOlfactory groove meningiomas compromise approximately 8-18% of intracranial meningiomas and harbor frequently large sizes when diagnosticated. We report 13 consecutive cases operated on Hospital São Paulo and Hospital Professor Edmundo Vasconcelos between 1995 and 2003. These patients were retrospectively studied regarding clinical preservation, surgical results and complications. The patients underwent uni or bifrontal craniotomies. There were nine complete resections including duramater and infiltrate bone and four patients with tumor removal and duramater coagulation. One patient died on post-operative period due to brain infarction and three patients had cerebrospinal fluid fistula that resolved. There were no symptomatic recurrence on the follow-up period of 3.3 years(range 11 m to 8 years). With current microsurgical techniques, these tumors can be resected safely with low morbidity.
- ItemAcesso aberto (Open Access)Meningeoma de goteira olfatória: relação entre achados clínico-radiológicos e índice proliferativo (Mib-1)(Academia Brasileira de Neurologia - ABNEURO, 2006-03-01) Paiva Neto, Manoel Antonio de [UNIFESP]; Tella Jr, Oswaldo Inácio de [UNIFESP]; Herculano, Marco Antonio [UNIFESP]; Stávale, João Norberto [UNIFESP]; Bonatteli, Antonio Pádua Furquim [UNIFESP]; Hospital Edmundo Vasconcelos; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: The aim of this study is to correlationate demografic, radiologic features and proliferative index defined by imunohistochemical expression of Mib-1 in olfactory groove meningiomas, and to observe possible predictive factors of recurrence of this tumor. METHOD: There were analysed demografic, clinical and radiological features of 15 patients with olfactory groove meningioma. Fifteen paraffin-embebed tumors were studied by imunohistochemical methods for the expression of proliferative index by the Mib-1. RESULTS: There was a statiscally positive correlation between tumoral and edema volumes. There was no correlation betweem Mib-1, tumoral volume, tumoral edema and frontal skull base abnormalities. Two cases with tumor recurrence had proliferative indices higher than the mean index. CONCLUSION: Larger tumors were more susceptible to develop peritumoral edema. The proliferative índex has no positive correlation with tumoral volume, edema volumes and skull base invasion, but probably it is associated with tumoral recurrence.

- ItemAcesso aberto (Open Access)Meningiomas anteriores e antero-laterais do forame magno(Academia Brasileira de Neurologia - ABNEURO, 2006-06-01) Tella Jr, Osvaldo Inácio de [UNIFESP]; Paiva Neto, Manoel Antonio de [UNIFESP]; Aguiar, Paulo Henrique; Herculano, Marco Antonio; Universidade Federal de São Paulo (UNIFESP); Hospital Professor Edmundo Vasconcelos; Universidade de São Paulo (USP); Faculdade de Medicina de JundiaíWe report our experience with 11 cases of foramen magnum meningiomas, eight originating inside the posterior fossa and three in the caudal region. The mean age of the patients was 50.8 years and the main complaint was cervical headache for at least 18.6 months and at the neurological examination, tetraparesis and deficit of the lower cranial nerves were very often observed. All patients were submitted to surgical treatment, always with exposition of the vertebral artery at the entry zone in the duramater of the posterior fossa, with partial removal of the occipital condyle in only three cases. Total resection was obtained in seven patients and partial removal in the other four due to adherences to vessels and nerves. The prognostic was related to the neurological condition before surgery.
- ItemSomente MetadadadosSuccessful Intra-arterial Thrombolysis in a Patient with an Intracranial Meningioma(Elsevier B.V., 2013-11-01) Baiense, Robson Fantinato [UNIFESP]; Abrahao, Agessandro [UNIFESP]; Ricarte, Irapua F. [UNIFESP]; Fukuda, Thiago G. [UNIFESP]; Oliveira, Ricardo A. [UNIFESP]; Silva, Gisele S. [UNIFESP]; Costa, Maristela; Teixeira, Manoel J.; Hosp Coracao; Universidade Federal de São Paulo (UNIFESP)Alteplase (recombinant tissue plasminogen activator [rt-PA]) label approval by the Food and Drug Administration remarks the contraindication of its use with known intracranial neoplasm because of potential bleeding complications. Despite this concern, the real risk of intracerebral bleeding in patients with intracranial neoplasms treated with rt-PA is unknown, and there are few reports of thrombolysis in patients with brain tumors. We report a case of a 78-year-old man who was seen in our emergency department within 2 hours from sudden onset of aphasia, right-sided hemiplegia, hypoesthesia, and homonymous hemianopsia. the National Institutes of Health Stroke Scale (NIHSS) score at admission was 20. Intra-arterial thrombolysis was performed with administration of .3 mg/kg of alteplase combined with mechanical thrombectomy. At discharge, his NIHSS score was 1, and after 90 days, his modified Rankin score was 1. To our knowledge, this is the first report of intra-arterial thrombolysis in a patient with acute ischemic stroke with an intracranial tumor.
- ItemAcesso aberto (Open Access)Supra-orbital keyhole removal of anterior fossa and parasellar meningiomas(Academia Brasileira de Neurologia - ABNEURO, 2010-06-01) Paiva Neto, Manoel Antonio de [UNIFESP]; Tella-jr, Oswaldo Inácio de [UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Hospital Professor Edmundo VasconcelosThe improvement of surgical techniques as well as the introduction of new surgical instruments promoted the use of keyhole craniotomies in neurosurgery. We evaluated the technical aspects of the supra-orbital keyhole approach considering the indications, limitations, and complications of this approach to treat anterior cranial fossa and parasellar meningiomas. Twenty-four patients (21 females; mean age, 53±8.6 years) operated on between 2002 and 2006 through a supra-orbital eyebrow approach were studied. Maximal tumor diameter ranged from 1.6 to 6 cm. Gross total resection was done in 20 (83.3%). All tumors were histologically benign. Two patients (8%) experienced CSF rinorhea and another two patients suffered transitory diabetes insipidus (8%). One patient experienced transitory hemiparesis. There was one case of meningitis and one mortality. Follow-up ranged between 6 to 66 months (mean 31.5±20.1 months), with no recurrence. The supra-orbital keyhole craniotomy is a useful minimally invasive approach to treat selected anterior fossa and parasellar meningiomas.
