Vivenciando a comunicação entre a equipe de enfermagem e os pais de recém-nascidos de alto risco: um estudo de caso

A unidade de terapia intensiva neonatal (UTIN) e um local de tecnologia avancada onde encontramos equipamentos que auxiliam na assistencia ao recem-nascido grave, que embora sejam necessarios para salvar vidas, podem criar um ambiente ameacador para os pais de RN de alto risco. Para a equipe de enfermagem atuar na UTIN e necessaric superar varios desafios e dentre esses, destacamos o dominar toda a parafernalia tecnologica e realizar cuidados ao trinomio recem-nascido-mae-pai, alicercadas em uma visao holistica. Nos ultimos tempos, a presenca dos pais na UTIN, tem sido cada vez mais frequente o que nao contribuiu para a reducao da angustia e ansiedade vivenciadas por eles neste momento critico. A comunicacao e uma estrategia essencial para dar inicio, manter e fortalecer o relacionamento entre equipe de enfermagem e os pais, criando assim um elo forte entre os pais que necessitam de cuidados psicoi emocionais e a equipe qualificada para proporcionar este apoio. Face ao exposto, c objetivo deste estudo foi conhecer como se da a comunicacao entre a equipe de enfermagem e os pais de RN de alto risco. A opcao metodologica escolhida foi o estudc de caso, realizado em uma UTIN de um hospital escola da cidade de São Paulo. A analise das observacoes e entrevistas nos permitiu a construcao das seguintes categorias: Comunicacao Terapeutica e Comunicacao Nao Terapeutica. Os resultados demonstraram que houve quase o mesmo numero de cenas descritas de comunicacao terapeutica) e nao terapeutica Na comunicacao terapeutica' os aspectos mais observados foram a utilizacao de tecnicas de comunicacao verbal dos grupos de expressao e clarificacao. Na comunicacao nao-verbal cinesica, o aspecto mais citado foi o olhar e da proxemica, foram os tipos de distancias Na comunicacao nao terapeutica) os aspectos relevantes foram os modos de comunicacao verbal) desconfirmacao e negacao. Outro aspecto ressaltado, o toque foi pouco observado entre a equipe de enfermagem e os pais de RN de alio risco. 0 desafio da equipe de enfermagem e aprender mais sobre comunicacao manifestada pelos pais, buscando decodifica-las nas suas formas...(au)
The newborn intensive care unit is a site for advanced technology where we find equipment that helps to assist the newborn children. In spite of the fact that they are necessary to save lives, they may create a threatening environment for the parents of newborn children at high risk. In order for the nursing team to work at the intensive care unit it is necessary to overcome many challenges and among them we highlight the expertise for the whole technological equipment and provide care for the trinomy newborn-mother-father, based upon an holistical perspective. Lately, the presence of the parents at the intensive care unit, has been gradually more frequent, what does not contribute for the reduction of the distress and anxiety that they face on this critical moment. Communication is an essential strategy to start, keep and strengthen the relationship between nursing team and parents who need the psychological and emotional care and the instrumentally skilled team to provide such support. In view of the exposed, the aim of this study was to gain awareness about how the communication between the nursing team and parents of newborn children at risk takes place. The chosen methodological option was the case study, performed at an intensive care unit of a school-hospital in São Paulo city. The analysis of the observations and the interviews allowed us the construction of the following categories: Therapeutic Communication an Nontherapeutic Communication. The results showed that there was nearly the same amount of described scenes of both therapeutic and non-therapeutic communication. In the therapeutic communication the most outstanding aspects were the use of verbal communication techniques of the expression and clarification groups. In the Kinesics non-verbal communication, the most present aspect was the looking and the Proxemics, and the types of distance. In the nontherapeutic communication, the ways of verbal communication, denial and nonconfirmation, were relevant aspects. Another outstanding aspect, the touch was hardly observed between the nursing team and the parents of the newborn at risk. The challenge of the nursing team is to learn more about the communication that the parents manifest, seeking to decode them into their verbal and non-verbal forms. Intending to achieve their expectations, contributing to reduce the anguish and anxiety and narrowing the relationship among the participants in this scenario.
