Interações entre benzodiazepínicos e etanol: uma revisão bibliográfica

Este estudo teve como objetivo avaliar diversas interações entre o etanol e benzodiazepínicos relatadas entre os anos 2003-2023. Foram utilizados os termos de busca "Ethanol"[Mesh],"Benzodiazepines"[Mesh] e "Drug Interactions"[Mesh]", o operador booleano “AND” foi utilizado para combinar os termos de pesquisa, na base de dados PubMed e, após a leitura do título e resumo, foram incluídos 24 artigos que contemplavam os seguintes critérios: (i) resumo citando interação entre algum benzodiazepínico e etanol e (ii) estudos in vivo e in vitro relatando interações. Após a leitura completa destes, apenas 12 foram utilizados na discussão deste trabalho. A exclusão do restante se deu por 3 critérios: (i) não relacionar os benzodiazepínicos com o etanol e vice versa, relacionando apenas um deles com outra condição, como exemplo a ansiedade, (ii) avaliar os efeitos individuais dos benzodiazepínicos ou do etanol nos receptores individualmente e (iii) artigo não disponível. As alterações farmacocinéticas descritas envolvem tanto a alteração da biotransformação de fármacos quanto do álcool quando há a coadministração das substâncias. Os efeitos farmacodinâmicos e comportamentais foram avaliados de várias formas, como alteração no efeito anticonvulsivante, miorrelaxante, efeitos na memória, intensificação de efeito reforçador, tanto de animais quanto de seres humanos, e avaliação de interações entre o benzodiazepínico e o etanol em casos post-mortem nos quais a causa da morte foi intoxicação por um deles ou por ambos. Todos os estudos apontam interações entre o benzodiazepínico e o etanol, mesmo com significância baixa. Avalia-se que o estudo dessas interações são extremamente importantes, visto que os benzodiazepínicos são altamente consumidos pela população, com ou sem prescrição médica e orientação adequada, principalmente quando há a utilização concomitante de etanol na vigência do tratamento medicamentoso, podendo desencadear reações inesperadas, devido a intensificação do efeito dos fármacos quando se há a ingestão de bebidas alcoólicas.

All studies point to interactions between benzodiazepines and ethanol, even with low significance. It is considered that the study of these interactions is extremely important, since benzodiazepines are highly consumed by the population, with or without a medical prescription and adequate guidance, especially when there is concomitant use of ethanol during drug treatment, which can trigger unexpected reactions, due to the intensification of the effect of drugs when alcoholic beverages are consumed.
