Construção e validação de um aplicativo móvel para autogerenciamento de pacientes com insuficiência cardíaca

Resumo
Objetivo: Construir e validar um aplicativo de autogerenciamento para pacientes com Insuficiência Cardíaca. Método: Trata-se de um estudo metodológico para evidência de validade de um protótipo de aplicativo baseado no autogerenciamento para pacientes com Insuficiência Cardíaca. Realizado no ambulatório de cardiologia do Hospital São Paulo da Universidade Federal de São Paulo, no período de dezembro de 2018 a dezembro de 2019 (criação do protótipo) e novembro de 2021 a março de 2022 (validação). Constou de duas etapas, sendo a primeira para construção e validação de protótipo com juízes especialistas, e a segunda para avaliação do benefício do uso do aplicativo por uma amostra aleatória simples de pacientes acompanhados na instituição e maiores de 18 anos. Os juízes deveriam ser membros da equipe multiprofissional e especialistas em cardiologia. Para a validação de conteúdo foi utilizado o Instrumento de Validação de Conteúdo Educativo em Saúde, considerando uma Razão de Validade de Conteúdo acima de 0,8 e Porcentagem de Concordância igual ou superior a 90%. Foi realizada uma randomização aleatória de pacientes, para avaliação do protótipo do aplicativo ou o manual de orientação aos portadores de Insuficiência Cardíaca a fim comparar o método convencional com o uso do aplicativo em relação à satisfação, aceitabilidade e engajamento. Resultados: O protótipo VivaCor-IC consta de 30 telas. Foram avaliadas por 21 juízes, obtendo Relação de Validade de Conteúdo superior a 0,80 em três itens: sequência lógica de ideias, tema atual e contribuição para o conhecimento na área. Como avaliação complementar, realizou-se a Porcentagem de Concordância com a média acima de 90% em todos os itens avaliados. Foi realizada uma randomização por sorteio com 10 pacientes, sendo que 5 avaliaram o protótipo do aplicativo e o restante um manual impresso com orientações sobre a doença. Não houve diferença estatisticamente significante entre os grupos em relação às respostas, à idade, gênero e ao tempo de acompanhamento no ambulatório. A concordância entre os pacientes ficou acima de 50% na maioria dos aspectos avaliados, obtendo aprovação inferior apenas em dois itens relacionados à linguagem interativa e tamanho do texto. Houve diferença estatisticamente significante em relação à escolaridade, sendo que o grupo avaliador do manual apresentou maior nível de escolaridade em relação ao grupo avaliador do protótipo do aplicativo. Conclusão: O protótipo obteve aprovação por concordância em todos os itens pela avaliação por especialistas e satisfação na maioria dos aspectos pela opinião de um grupo amostral de pacientes. Posteriormente, será necessário aperfeiçoamento e nova rodada de validação para obtenção da versão final do aplicativo VivaCor-IC.
Objective: Build and validate a self-management application for patients with Heart Failure. Method: This is a methodological study for evidence of validity of a prototype application based on self-management for patients with heart failure. Performed at the cardiology outpatient clinic of the Hospital São Paulo of the Federal University of São Paulo, from December 2018 to December 2019 (prototype creation) and November 2021 to March 2022 (validation). It consisted of two phases, the first for building and validating a prototype with expert judges, and the second for evaluating the benefit of using the application by a simple random sample of patients followed up at the institution and over 18 years of age. The judges should be members of the multidisciplinary team and specialists in cardiology. For content validation, the Health Education Content Validation Instrument was used, considering a Content Validity Ratio above 0.8 and a Percentage of Agreement equal to or greater than 90%. A random randomization of patients was performed to evaluate the prototype of the application or the guidance manual for patients with heart failure in order to compare the conventional method with the use of the application in terms of satisfaction, acceptability and engagement. Results: The VivaCor-IC prototype consists of 30 screens. They were evaluated by 21 judges, obtaining a Content Validity Ratio greater than 0.80 in three items: logical sequence of ideas, current topic and contribution to knowledge in the area. As a complementary assessment, the Percentage of Agreement was performed with an average above 90% in all items evaluated. A randomization was carried out by drawing lots with 10 patients, 5 of which evaluated the prototype of the application and the rest a printed manual with guidelines on the disease. There was no statistically significant difference between the groups in terms of responses, age, gender and length of follow-up at the outpatient clinic. The agreement between patients was above 50% in most aspects evaluated, obtaining lower approval only in two items related to interactive language and text size. of schooling in relation to the app prototype appraiser group. Conclusion: The prototype was approved by agreement on all items by the evaluation by experts and satisfaction in most aspects by the opinion of a sample group of patients. Subsequently, it will be necessary to improve and a new round of validation to obtain the final version of the VivaCor-IC application.
