Fitoestrógenos para síndrome dos ovários policísticos: revisão sistemática e metanálise de ensaios clínicos controlados e aleatórios

A síndrome dos ovários policísticos afeta de 6% a 20% das mulheres em idade reprodutiva, sendo associada a complicações metabólicas, reprodutivas e cardiovasculares que impactam negativamente a qualidade de vida. Dada a inexistência de um tratamento farmacológico abrangente para todas as manifestações da SOP, alternativas como exercício físico e intervenções nutricionais têm ganhado destaque. Fitoestrógenos, compostos dietéticos com estrutura semelhante ao 17-β-estradiol, foram sugeridos como potenciais moduladores hormonais e promotores de melhor sensibilidade à insulina. O objetivo deste estudo foi avaliar os efeitos do consumo de fitoestrógenos em mulheres com SOP. Para isso, a revisão sistemática utilizou os termos "polycystic ovary syndrome" e "phytoestrogen" nas bases PubMed/Medline, CINAHL/EBSCOhost, Cochrane, Embase, Scopus, Web of Science e SciELO, além de literatura cinza e registros clínicos. Os estudos foram selecionados com base em critérios predefinidos, sendo a análise de dados realizada sistematicamente. Quando possível, metanálises foram conduzidas para síntese quantitativa dos desfechos.
Polycystic ovary syndrome (PCOS) affects 6% to 20% of women of reproductive age and is associated with metabolic, reproductive, and cardiovascular complications that negatively impact quality of life. Given the lack of a pharmacological treatment that addresses all PCOS manifestations, alternatives such as physical exercise and nutritional interventions have gained attention. Phytoestrogens, dietary compounds with a structure similar to 17-β-estradiol, have been suggested as potential hormonal modulators and promoters of improved insulin sensitivity. The aim of this study was to evaluate the effects of phytoestrogen consumption in women with PCOS. To achieve this, a systematic review used the terms "polycystic ovary syndrome" and "phytoestrogen" across databases such as PubMed/Medline, CINAHL/EBSCOhost, Cochrane, Embase, Scopus, Web of Science, and SciELO, in addition to grey literature and clinical trial registries. Studies were selected based on predefined criteria, and data analysis was conducted systematically. Where possible, meta-analyses were performed to quantitatively synthesize outcomes.
