Análise do custo- benefício da reabilitação por teleatendimento comparado com atendimento presencial na perspectiva do paciente após artroplastia total de joelho

INTRODUÇÃO: Ao longo das últimas décadas, houve um aumento na longevidade e expectativa de vida das pessoas no estado de São Paulo. Com o aumento da expectativa de vida no Brasil, houve aumento do número de pessoas idosas com Osteoartrose de joelho indicadas para tratamento por Artroplastia Total de Joelho, principalmente no estado de São Paulo. OBJETIVO: O objetivo do estudo foi realizar uma análise de custo-efetividade da telerreabilitação comparada com a reabilitação presencial de pacientes no pós-operatório de Artroplastia Total do Joelho e verificar se há diferença de custo-efetividade entre as técnicas. MÉTODOS: A população estudada são pacientes com idade entre cinquenta a oitenta e cinco anos, provenientes de internação após cirurgia de ATJ em um hospital privado. O número de pacientes estudados foram vinte em cada grupo de Videoconferência (VC) e Acompanhamento Convencional (AC), totalizando quarenta pacientes em um ensaio clínico randomizado ao longo de um ano e meio. A análise de custo-efetividade foi realizada em ambos os grupos, englobando custos diretos e indiretos. RESULTADOS: A análise teve diferença de p<0,001 para valores iniciais e finais para o questionário GDS (Escala de Depressão Geriátrica), considerando ambos os grupos, o questionário EQ-5D que teve diferença de p<0,001 para valores iniciais e finais em ambos os grupos e entre ambos. A diferença nos custos entre os grupos se explica pelo fato do grupo VC não precisar utilizar meios de locomoção até a Fisioterapia, consequentemente não ter gasto com transporte e locomoção. CONCLUSÃO: Em conclusão, este estudo sugere que a fisioterapia por teleatendimento, ao oferecer os mesmos resultados em relação à qualidade de vida em comparação com o acompanhamento convencional, é mais custo-efetiva.

With the increase in life expectancy in Brazil, there was an increase in the number of elderly people with knee osteoarthritis indicated for treatment by Total Knee Arthroplasty, mainly in the state of São Paulo. OBJECTIVE: The objective of the study was to carry out a cost- effectiveness analysis of the telerehabilitation compared with face-to-face rehabilitation of patients in the postoperative period of Total Knee Arthroplasty and verify whether there is a difference in cost-effectiveness between the techniques. METHODS: The studied population are patients aged between fifty and eighty-five years, admitted after TKA surgery in a private hospital. The number of patients studied were twenty in each Videoconference (VC) and Conventional Monitoring (AC) group, totaling forty patients in a randomized clinical trial over a year and a half. The cost-effectiveness analysis was carried out in both groups, covering direct and indirect costs. RESULTS: The analysis had statistically significant difference of p<0.001 for initial and final values for the questionnaire GDS (Geriatric Depression Scale), considering both groups, the EQ-5D questionnaire that had a statistically difference of p<0.001 for initial and final values in both groups and between both. The difference in costs between groups is explained by the fact that the VC group did not need use means of transportation to Physiotherapy, consequently you will not have expenses with transport and locomotion. CONCLUSION: In conclusion, this study suggests that telehealth physiotherapy, by offering the same results in terms of quality of life compared to conventional monitoring, is more cost-effective.
