Adaptação transcultural e mensuração da validade e confiabilidade do Functional Capacity Index (FCI)

Resumo
Objetivo: adaptar culturalmente para o português e validar o instrumento de medida de capacidade funcional, o Functional Capacity Index (FCI), à realidade brasileira e verificar as propriedades psicométricas, confiabilidade e validade do instrumento adaptado em pacientes com lesões traumáticas um ano após o acidente de trânsito. Método: pesquisa metodológica que envolveu tradução/retrotradução, adaptação transcultural, incluindo-se avaliação semântica, idiomática, conceitual e cultural e validação do FCI. A versão final foi aplicada a um grupo de pacientes vítimas de trauma após um ano da alta hospitalar. Para verificar a confiabilidade foi utilizado o Coeficiente Alfa de Cronbach. Para a avaliação da validade de construto discriminante comparou-se dois grupos, caso e controle. Para a validação concorrente aplicou-se o índice de Katz e para a verificação da validade convergente, o questionário Injury Severity Score (ISS). Resultados: Após a tradução do instrumento, do total de 58 itens avaliados, oito (13,8%) não apresentaram concordância entre os tradutores. Na retrotradução, seis (10,3%) apresentaram palavras diferentes do original, porém com significados semelhantes. Na avaliação pelo comitê de especialistas, cinco itens (8,6%) não alcançaram a taxa de concordância maior que 90,0%. Após a aplicação do instrumento final, a consistência interna de todos os domínios que compõe o FCI, verificada pelo Alfa de Cronbach, mostrou-se aceitável (? ? 0,7); os valores do FCI correlacionaram-se de forma moderada e significante com os valores do índice de Katz (r = 0,387 e p = 0,006); observou-se que não houve correlação significante entre os escores ISS e FCI (r = - 0,118 e p = 0,415); os valores do FCI foram estatisticamente maiores no grupo caso em comparação ao grupo controle (p<0,001), confirmando a validade de critério discriminante. Conclusão: A adaptação transcultural do instrumento de avaliação da capacidade funcional, FCI, para o português do Brasil, aplicado às pessoas que sofreram acidente de trânsito um ano após a alta hospitalar, permitiu concluir que o instrumento final mostrou resultados satisfatórios quanto à confiabilidade. Em relação à validade, os resultados quanto à validade de critério concorrente e de construto discriminante indicaram resultados favoráveis a sua utilização.
Objective: cultural adaptation of the instrument measuring Functional Capacity Index (FCI) to the Portuguese language, and validation to the Brazilian reality, in addition to determining the psychometric properties, reliability and validity of the instrument, as adapted for patients with traumatic lesions, one year after suffering a traffic accident. Method: methodological research involving translation/back translation and transcultural adaptation, and including a semantic, idiomatic, conceptual and cultural evaluation, as well as validation of the FCI. The final version was applied to a group of patients still suffering from trauma a year after discharge from the hospital. The Cronbach Alpha Coefficient was used to verify reliability. Two groups were compared - case and control - to evaluate the discriminant construct validity. The Katz Index was applied to determine concurrent validation, and the Injury Severity Score (ISS), to assess the convergent validity. Results: after the instrument was translated, eight (13.8%) of the total 58 items evaluated showed no agreement between the translators. In the back translation, six items (10.3%) had words different from the original, but with a similar meaning. As appraised by the committee of specialists, 28 items (48.3%) had an agreement rate lower than 90.0%. After the final instrument was applied, the internal consistency of all the domains composing the FCI, as verified by Cronbach?s Alpha, proved acceptable (? ? 0.7). Correlation of the FCI values with the Katz Index values was both moderate and significant (r = 0.387 and p = 0.006). There was not significant correlation between the ISS and the FCI scores (r = -0.118 and p = 0.415). The FCI values were statistically greater in the case group, as compared with the control group (p<0.001), thus confirming the validity of the discriminant criterion. Conclusion: it was concluded that the transcultural adaptation of the instrument assessing functional capacity ? FCI ? to Brazilian Portuguese, as adapted for people suffering traffic accidents one year following discharge from the hospital, showed that the final instrument proved both satisfactory and reliable. In relation to instrument validity, the concurrent criterion validity and the discriminant construct also yielded results favoring its application.
