Resumo
Objetivo: Construir um protocolo assistencial para o primeiro atendimento do enfermeiro ao paciente com dor torácica. Método: Estudo metodológico de desenvolvimento de tecnologia em saúde, realizada em duas etapas, sendo a primeira voltada à revisão de escopo e a segunda para a construção de um protocolo assistencial por meio da revisão e da experiência empírica dos autores. Na primeira etapa, com a finalidade de embasar os tópicos relevantes do protocolo, foi realizada a revisão de escopo com a pergunta estruturada por meio da estratégia PICo. Os descritores utilizados foram de acordo com as Ciências da Saúde (DeCS) e o Medical Subject Headings (MeSH), como Dor torácica (Chest Pain); Síndrome Coronariana Aguda (Acute Coronary Syndrome); Guias de Prática Clínica como Assunto (Practice Guidelines as Topic); Padrões de Prática em Enfermagem (Practice Patterns, Nurses'); Eficácia (Treatment Outcome). As bases de dados consultadas foram a Biblioteca Virtual em Saúde - BVS (BIREME); Web of Science; e National Library of Medicine (PubMed). Para a seleção dos estudos pertinentes à pergunta norteadora utilizaram-se os critérios de inclusão e exclusão. Na segunda etapa de desenvolvimento do estudo, construiu-se tópicos e itens destinados ao primeiro atendimento do enfermeiro ao paciente com dor torácica. Resultados: Dezessete estudos compuseram o corpus de análise para a síntese qualitativa, os quais versam sobre estudos primários, experimentais e observacionais, publicados em periódicos nacionais e internacionais. Após a revisão de escopo, estabeleceram-se 04 tópicos: Tópico 1: Preparo da unidade para resposta rápida ao paciente com dor torácica; tópico 2: Resposta rápida à dor torácica; tópico 3: Acompanhamento intra-hospitalar; tópico 4: Prevenção secundária pós Síndrome Coronariana Aguda. Conclusão: O protocolo elaborado a partir da revisão de escopo, o qual incluiu dezessete artigos, destina-se para o primeiro atendimento do enfermeiro ao paciente com dor torácica, que contempla quatro tópicos e vinte e dois itens assistenciais. Este instrumento tem o potencial de guiar e auxiliar o enfermeiro na tomada de decisão frente ao paciente com dor torácica até a chegada do médico para continuidade do atendimento.
Objective: To build a care protocol for the first care provided by nurses to patients with chest pain. Method: Methodological study of health technology development, carried out in two stages, the first focused on the scope review and the second on the construction of a care protocol through the review and empirical experience of the authors. In the first stage, in order to support the relevant topics of the protocol, a scope review was carried out with a structured question through the PICo strategy. The descriptors used were according to the Health Sciences (DeCS) and the Medical Subject Headings (MeSH), such as Chest Pain (Chest Pain); Acute Coronary Syndrome; Clinical Practice Guides as Topic (Practice Guidelines as Topic); Nursing Practice Patterns (Nurses'); Effectiveness (Treatment Outcome). The databases consulted were the Virtual Health Library - VHL (BIREME); Web of Science; and National Library of Medicine (PubMed). For the selection of studies relevant to the guiding question, the inclusion and exclusion criteria were used. In the second stage of the study development, topics and items were constructed for the first care provided by nurses to patients with chest pain. Results: Seventeen studies composed the corpus of analysis for the qualitative synthesis, which deal with primary, experimental and observational studies, published in national and international journals. After reviewing the scope, 04 topics were established: Topic 1: Preparation of the unit for rapid response to patients with chest pain; topic 2: Rapid response to chest pain; topic 3: In-hospital follow-up; topic 4: Secondary prevention after Acute Coronary Syndrome. Conclusion: The protocol developed from the scope review, which included seventeen articles, is intended for the first care provided by nurses to patients with chest pain, which includes four topics and twenty-two care items. This instrument has the potential to guide and assist nurses in decision-making regarding patients with chest pain until the doctor arrives for continuity of care.
