Objective: To develop the Brazilian Portuguese version of the Creighton Model FertilityCare System (CrMS) Follow-up Form (FUF). Methods: Translation and cultural adaptation of the FUF for use in Brazil, developed in 6 stages: Translation, Expert Panel, Back-translation, Pre-testing, Review, and Final Version. According to the World Health Organization methodology. Results: The 25 sections that make up the FUF were translated, and 14 sections underwent an adaptation process at one of the stages, aimed at maximizing semantic, idiomatic, experiential, and conceptual equivalence of the items from the original English version to Portuguese. The need for adaptation arose primarily for four reasons: first, due to the format of paired and seemingly repetitive questions that some sections of the FUF have. Second, because of the differences in cultural realities, hygiene habits, and consumption that exist in the United States and not in Brazil. Third, the use of technical terms and medical vocabulary. And fourth, sentences that present many concepts related to CrMS usage. A total of 127 users were included, who voluntarily contacted an instructor to learn the method. They were then invited, before their introductory session or Follow-up, to participate in the research. If they declined, they continued normally in the teaching system without any detriment. If they accepted, the informed consent form was sent individually to each user, and upon signing, they began participating in the study. The average age was 33.7 years, 88.2% were married, and 48.8% had never conceived. The majority, 68 (53.5%), were not using any family planning method when they began CrMS. Almost half of them, 48%, were trying to get pregnant in the last year and 58.3% resided in the State of São Paulo. Conclusions: The translation of CrMS FUF into Brazilian Portuguese resulted in a final version conceptually equivalent to the original English version. This instrument can be used by all instructors in Brazil with the confidence that standardization in the application of CrMS will be maintained throughout the country.
