Aptidão física de hemodialíticos crônicos: um estudo transversal

Introdução: A baixa aptidão física (AF) tem sido associada a piores desfechos de saúde que podem levar a um aumento de hospitalizações, morbidade e mortalidade em hemodialíticos crônicos. Objetivo: Comparar a AF autorreferida entre hemodiálíticos crônicos e indivíduos sem doença renal crônica (DRC). Métodos: Neste estudo transversal, hemodialíticos crônicos (n=674) e indivíduos sem DRC (controle, n=518) autorreferiram a AF respondendo à "The International Fitness Scale" (IFIS). As análises foram ajustadas para sexo, idade, cor da pele, escolaridade, diabetes, tabagismo, atividade física, comportamento sedentário e índice de massa corporal. Resultados: Hemodialíticos crônicos são mais propensos a autorrelatar baixa AF geral (OR=2,03; IC 95%: 1,35 a 3,05; p=0,001), aptidão cardiorrespiratória (OR=3,18; IC 95%: 2,27 a 4,46; p<0,001), força (OR=2,51; IC 95 %: 1,71 a 3,68; p<0,001) e agilidade (OR=1,83; IC 95%: 1,26 a 2,64; p=0,001) do que indivíduos sem DRC. Conclusão: Hemodialíticos crônicos apresentaram uma menor percepção de AF do que indivíduos sem DRC. Nesse cenário, considerando a alta prevalência de baixa AF nesses indivíduos e as consequências relacionadas a isso, estratégias para melhorá-la, como iniciativas relacionadas à prática de exercícios físicos com essa população, inclusive estrategicamente durante a hemodiálise, podem ser interessantes.

Objective: To compare self-reported PF between patients undergoing chronic hemodialysis and those without chronic kidney disease (CKD). Methods: In this cross-sectional study, patients undergoing chronic hemodialysis (n=674) and individuals without CKD (control, n=518) self-reported PF by responding to "The International Fitness Scale"(IFIS). Analyses were adjusted for sex, age, skin color, educational years, diabetes, smoking, physical activity, sedentary behavior, and body mass index. Results: Patients undergoing chronic hemodialysis are more likely to self-report lower general PF (OR=2.03; 95% CI: 1.35 to 3.05; p = 0.001), cardiorespiratory fitness (OR=3.18; 95% CI: 2.27 to 4.46; p<0.001), strength (OR=2.51; 95 % CI: 1.71 to 3.68; p<0.001) and agility (OR=1.83; 95% CI: 1.26 to 2.64; p=0.001) than the control group. Conclusion: Patients undergoing chronic hemodialysis presented a lower perception of PF than individuals without CKD. In this scenario, considering the high prevalence of low PF in these individuals, and the consequences related to it, strategies to improve it, like initiatives related to the practice of physical exercises with this population, including strategically during hemodialysis, may be interesting.
