AIM: To evaluate the applicability of audiometric evaluation during drug treatment for otospongiosis. MATERIALS AND METHODS: A prospective, randomized, controlled, double-blind study involving 26 patients with clinical, audiometric and CT scan image of otosclerosis. Patients eligible for the study were divided into three groups (A, B and C) and received treatment with alendronate sodium (B), sodium fluoride (C) and placebo (A) for 6 months. After this period they were submitted to new tests. RESULTS: There were not statistically significant differences between air and bone conduction (gap). We also found no differences in the speech recognition threshold (SRT) and speech discrimination (IRF) between before and after treatment. CONCLUSION: After six months of drug treatment the audiometric evaluation kept the same hearing thresholds, suggesting stabilization of the otospongiotic lesions.

Neto, M.e.l.r.b.; Lobato, M.f.; Universidade Federal de São Paulo (UNIFESP); Hospital Paulista de OtorrinolaringologiaIntroduction: Otospongiosis or otosclerosis is a common degenerative and hereditary disease of the labyrinthine capsule and occurs mainly in women with age between 20 and 30 years. In recent decades stapedotomy has increasingly tended to become the most used surgical technique to the treatment of otosclerosis. Thus, this study analyzes the results of 59 stapedotomies realized in Paulista Hospital of Otorhinolaryngology on the last 7 years. Study design: Clinical randomized. Materials and Method: Retrospective analysis of 59 patients with otosclerosis who underwent stapedotomy performed by the same surgeon and follow-up clinical and audiological. Results: Hearing improvement proved by audiogram air-bone gap closure in 53 patients (90%). The complications occurred were displaced prosthesis (7%); taste alterations (7%); facial paralysis (3%); vertigo (3%); prosthesis total extrusion (1,5%); tinnitus (1,5%); tympanic membrane perforation (1,5%). Conclusion: Stapedotomy has been seen as good therapeutic choice to the treatment to the conductive hearing loss by otosclerosis seeing that generally has low morbidity and high success rates, providing better life quality to this patients.

STUDY DESIGN: A prospective and multicentric study using computed tomography (CT) of the temporal bone. MATERIAL AND METHOD: 54 patients with a clinical and surgical diagnosis of otosclerosis. A tomographic analysis has been performed to evaluate the sensitivity of CT scans in diagnosing fenestral otosclerosis, verifying the sites of predilection for otosclerotic lesions, the bilateralism and symmetry of the disease. In this study, 22 patients undergone CT scans of temporal bones have been included in order to investigate other otologic diseases and have constituted the control group. RESULTS: The CT scan was positive in 87% of the patients (77% of the ears). The tomographic findings were mostly bilateral (78,7%) and the fenestral focus more prevalent was the anterior focus to the oval window. CONCLUSIONS: The CT scan has shown a high index of positivity and the anterior focus to the oval window has been the most prevalent in this population, and it could be considered the highly suggestive tomographic finding for otosclerosis.

METHODS: A prospective and multicentric study using computed tomography (CT) of the temporal bone in 54 patients with a clinical and surgical diagnosis of otosclerosis. A tomographic analysis has been performed to evaluate the utility of CT scans in diagnosing retrofenestral otosclerosis, verifying the sites of predilection for otosclerotic lesions. In this study, 22 patients undergone CT scans of temporal bones have been included in order to investigate other otologic diseases and have constituted the control group. RESULTS: The tomographic findings were mostly bilateral and the retrofenestral focus more prevalent was the pericochlear focus. CONCLUSIONS: The CT scan was useful in the identification of the retrofenestral focuses. The evidence of this focuses and the presence of the endosteal involvement of the cochlea can suggest a higher activity of the disease.
