- ItemSomente MetadadadosEfeito da infusão de células-tronco mesenquimais na regeneração do fígado após hepatectomia a 70%: modelo experimental em ratos(Universidade Federal de São Paulo (UNIFESP), 2015-12-18) Alves, Ana Karina Soares [UNIFESP]; Linhares, Marcelo Moura Linhares [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: Major hepatic resections still have high levels of morbimortality, mainly due the post-operatory hepatic failure. In order to enable fast regeneration of the remnant liver techniques as previous embolization or portal vein ligature, two-stage hepatectomy and the ALPPS have been used, yet the insufficient hepatic mass continues to be a challenge. The cell therapy has been speculated as a promising therapeutic arsenal to accelerate or improve the hepatic regeneration after extended resections. The aim of this study is to evaluate the effect of the mesenchymal stem cells on the hepatic regeneration in rats submitted under 70% hepatectomy. Methods: in fourty Winstar rats submitted under 70% hepatectomy it was infused 106 of mesenchymal stem cells (trial group) or saline solution (control group) through portal vein. These groups were divided in early and late subgroups (euthanasia on the third and fifth post operative day, respectively). Outcomes of the hepatic regeneration tax, albumin variation, TGO, TGP and FA, cell proliferation index (ki-67) and the number of mitosis were compared. Results: Significant outcomes in the hepatic regeneration tax, number of mitosis and cell proliferation index were not observed. Likewise there was no difference of albumin levels variation, TGO and FA. It was found significant variation in the levels of TGP, higher in the trial group. Conclusions: It was not observed improvement of the hepatic regeneration tax on the third and fifth post hepatectomy day among rats that received cellular therapy. There was also no improvement of hepatic function, of cell proliferation index and of the number of mitosis in the groups that were submitted under the mesenchymal stem cells infusion.
- ItemAcesso aberto (Open Access)Laparoscopic left lateral segmentectomy for metachronic metastases of small intestine adenocarcinoma: a case report(Sociedade Brasileira de Coloproctologia, 2011-12-01) Pais-costa, Sergio Renato [UNIFESP]; Araujo, Sergio Luiz Melo; Lima, Olímpia Alves Teixeira; Paes, Marcio Almeida; Martins, Sandro José; Universidade Federal de São Paulo (UNIFESP); Hospital Santa Lucia; Universidade de Brasília (UNB); Hospital de Base do Distrito Federal; Universidade de Brasilia (UNB)Hepatectomy has been the standard treatment for metachronic metastases of non-colorectal (NCR) origin, mainly when the disease-free interval is more than two years. Laparoscopic hepatectomy has become the golden standard mainly for left side resections, due to lower morbidity, shorter hospital stay, early recovery and good cosmetic outcome. The authors report the case of a female patient with two metachronic metastases (ten years of disease-free survival), of non-colorectal origin (adenocarcinoma of small intestine), treated by laparoscopic left lateral segmentectomy (left hepatic lobectomy) with success. The postoperative progress was satisfactory. To date, the patient has presented no tumoral recurrence (six months of follow-up period). Laparoscopic left lateral segmentectomy can be satisfactorily performed in selected cases of hepatic metastasis. This approach presents low morbidity and good cosmetic result. The lack of alternative treatments and the poor prognosis of untreated cases have justified surgical resection in order to increase overall survival. Nevertheless, this approach should be performed by hepatic surgery expertise teams trained on advanced laparoscopic procedures.
