Methods: cross-sectional study with 187 children and adolescents between 8 and 12 years of age. We evaluated weight, height, waist circumference, Tanner pubertal stage, blood pressure and serum uric acid concentration. To test the association of serum uric acid concentration (UA) with blood pressure (BP), univariate linear regression was performed, in which BP was the outcome variable. In addition to AU, the following covariates were considered: a) sex, b) age, c) BMI Z-score, and d) waist circumference and interaction between UA and each other covariate. Finally, we performed the multivariable linear regression analysis. Results: We found no association between serum uric acid concentration and systolic blood pressure (p=0.410) and diastolic blood pressure (p=0.806). When we evaluated the interaction of uric acid with other independent variables, we found a positive effect of the interaction between serum uric acid concentration and age on both systolic blood pressure (p=0.001) and diastolic blood pressure (p=0.024). However, this association did not remain positive after analysis of multivariate linear regression. Conclusion: we conclude that serum uric acid concentration has no effect on blood pressure in the sample of children and adolescents studied. However, our data suggest that this association might happen in older children.

Para tanto, o desenvolvimento de novos métodos analíticos se faz necessário para torná-la viável e confiável. O objetivo do estudo foi aprimorar um método analítico para avaliar as concentrações de ácido úrico salivar e sua relação com marcadores metabólicos séricos em pacientes com obesidade mórbida submetidos à dieta ou gastroplastia e acompanhados por um período de 6 meses. Dois grupos foram seguidos: grupo Gastroplastia (GG; n=39) com participantes submetidos à gastroplastia (bypass) e grupo Controle (GC; n=34), cujos participantes receberam aconselhamento dietético por 6 meses. O exame clínico envolveu avaliação antropométrica e bucal (índice de cárie/CPOD e índice periodontal comunitário). Amostras de saliva estimulada e não estimulada foram coletadas para dosagem do fluxo salivar, capacidade tampão, proteína total e ácido úrico por cromatografia líquida de alta eficiência (HPLC). Dados quanto aos marcadores séricos metabólicos foram extraídos dos prontuários médicos. Utilizou-se estatística exploratória, teste de correlação de Spearman e ANOVA modelo misto. Foi observada redução significativa do IMC no grupo Gastroplastia (p<0,001) e aumento da atividade de cárie em ambos os grupos. Correlação significativa foi observada entre os níveis de ácido úrico salivar e ácido úrico sérico (rho=0,46), proteína total salivar (rho=0,32), LDL (rho=-0,39) e circunferência do pescoço (rho=0,30) e cintura (rho=0,24) (p<0,05). Não houve alteração significativa nos níveis de ácido úrico salivar após 6 meses de acompanhamento, enquanto os níveis séricos diminuíram em ambos os grupos (efeito tempo p<0,001). As variações nos níveis de ácido úrico e proteína total salivares foram muito semelhantes ao longo dos 6 meses no grupo gastroplastia. A metodologia empregada para a dosagem de ácido úrico salivar mostrou-se viável e com parâmetros laboratoriais adequados para a aplicação proposta, e os resultados enfatizam a importância de se considerar o status de saúde bucal como possível fator de influência na avaliação do ácido úrico como biomarcador salivar.

Hyperuricemia has already been identified as a risk factor for chronic kidney disease (CKD). It is often found in these patients and is considered an important cardiovascular risk factor. Currently there is a discussion if hyperuricemia acts as a marker or as an independent risk factor for CKD. In a previous study, we evaluated post-pubertal, obese and eutrophic adolescents without chronic kidney disease and the results showed that a high serum uric acid concentration was associated with higher values of systolic blood pressure in obese children. These data led us to formulate the hypothesis that uric acid concentration may modulate the effect of increased body mass on blood pressure in pediatric patients. However, our previous study was cross-sectional and therefore did not allow us to correctly assess this hypothesis. Kidney transplantation seemed an interesting condition to test our hypothesis due to the frequency of rapid gain in body mass that characterizes the first months of follow-up of children undergoing this treatment. Objectives: To test whether the serum concentration of uric acid increases the hypertensive effect of body mass gain in pediatric kidney transplant recipients, as well as to verify the impact of post-transplant changes in uric acid levels on the estimated glomerular filtration rate. Patients and methods: An observational and longitudinal study of children and adolescents who received kidney transplants was performed, with analysis of clinical, anthropometric and laboratory data at the following times: pre-transplant and 1, 3, and 6 months after kidney transplantation. The sample consisted of children and adolescents treated and transplanted at Hospital do Rim, from March 2017 to May 2019. During the study period data was collected on the following parameters: weight, height, blood pressure, drug prescriptions, serum uric acid and creatinine levels. We developed repeated measures models to test the study hypotheses, using the “generalized estimating equations” (GEE) method for the outcomes of hypertension and the evolution of eGFR. Results: 103 transplant patients were included and regarding the occurrence of hypertension, the significant associations at the end of the analysis were kidney and urinary tract malformation as the etiology of chronic kidney disese (Odds Ratio: 0.45, 95% CI: 0.24-0.84, p = 0.01) and age (Odds Ratio: 0.84, 95% CI: 0.77 - 0.92 p = 0.00). In the analysis of the glomerular filtration rate, we observed an inverse relationship with uric acid (β= -11.14, IC95%: -1.79- -5.50), p<0.01). Conclusion: Although we observed a high prevalence of hypertension, elevated weigh gain and high uric acid, we found no association between the three variables. Nevertheless, the metabolic situation of children after kidney transplantation deserves attention, as the period is characterized by significant gain in body mass, high frequency of hyperuricemia and hypertension.
