Histological changes that may predispose to this irreversible malignant transformation remain as undefined lesions in the larynx. With the analysis of immunohistochemical parameters and the discovery of a biological marker able to identify these early lesions, a more effective early treatment can be provided, which can result in the preservation of phonatory organ. Objective: To analyze the immunohistochemical protein expression of cathepsin B-and ECadherin in leukoplakia of the vocal fold and correlate them with clinical data, epidemiology and prognosis of the disease. Method: We retrospectively evaluated 32 patients with leukoplakia of the vocal folds in the Institute of Larynx- city of Sao Paulo- diagnosed through laryngoscopy and treated with surgery during 2000 to 2004. Patients were divided into two groups according to histological results, Group A (Low Grade, n = 16) and Group B (High Grade, n = 16). Slides added by anti-cathepsin-B and anti-E-cadherin were obtained from paraffin blocks for subsequent reading of markers through Imagna- ImageLab processing. The quantitative expression of cathepsin B-and Ecadherin was evaluated according to the coloration intensity and tissue distribution. The expression index (EI) of each marker was correlated to patient´s lifestyle (use of tobacco and alcohol), signs of gastroesophageal reflux disease and local recurrence of the lesion in two separate groups and among all patients. Results: In the descriptive analysis, the vocal fold leukoplakia showed more intensity in men who had frequent habit of smoking and alcohol consumption. These patients had high levels suggestive of signs of gastroesophageal reflux disease and local recurrence of the lesion after initial treatment. Statistical analysis comparing the levels of expression (IE) of the markers between the groups (A and B), patients with local recurrence in Group B (High Grade) had higher IE for cathepsin-B. In the statistical analysis of IE markers among all patients without histological differentiation, the IE of cathepsin-B was higher among smokers and those with no signs suggestive of reflux. In the same analysis, E-cadherin was higher in all patients with local recurrence. Conclusion: Smoking patients who have moderate or severe dysplasia, carcinoma in situ and high levels of cathepsin-B were more susceptible to local recurrence of disease. Those with signs suggestive of gastroesophageal reflux disease had lower IE of cathepsin-B, independent of histologic differentiation. The increase in cathepsin-B expression, for high grade lesions group, indicates this protein as a possible marker of aggressiveness to leukoplastic vocal cord lesions. The rate of expression of Ecadherin is shown as a potential marker for recurrence of leukoplastic vocal fold lesions.

Twenty-five to forty-five percent of all schwannomas occur in the head and neck region, but location of such tumors in the larynx is rarely observed. The present report is aimed at describing a clinical case of laryngeal schwannoma, with emphasis on sonographic findings.
- ItemSomente MetadadadosMecanismos de ação da proteína anti-inflamatória anexina A1 e do receptor para peptídeos formilados FPR2/ALX no carcinoma epidermóide de laringe humano: estudos in vivo e in vitro(Universidade Federal de São Paulo (UNIFESP), 2012) Gastardelo, Thais Santana [UNIFESP]; Oliani, Sonia Maria [UNIFESP]Introdução: Varias evidencias demonstram que o desenvolvimento e a progressao do cancer dependem das interacoes entre as celulas tumorais e inflamatorias. A proteina anti-inflamatoria anexina 1 (ANXA1) tem sido associada com a regulacao da migracao/invasao das celulas tumorais e, recentes estudos tem sugerido que a sua acao regulatoria pode ser mediada por meio dos receptores para peptideos formilados (nFPRs). OBJETIVOS: Em funcao da importancia no entendimento da acao anti-inflamatoria/antiproliferativa da ANXA1 na interacao cancer/inflamacao, o objetivo das nossas investigacoes foi avaliar, in vivo e in vitro, as celulas tumorais e inflamatorias e, o mecanismo de acao da ANXA1 e do receptor FPR2/ALX no carcinoma epidermoide de laringe humano. MATERIAIS E METODOS: Nas amostras de tecidos laringeo controle, peritumoral e tumoral, obtidas de 20 pacientes portadores de carcinoma epidermoide de laringe, analisamos histopatologica e quantitativamente os mastocitos (MCs) e neutrofilos (Nøs). A heterogeneidade dos MCs foi analisada pela expressao das peptidases triptase e quimase por analises imuno-histoquimicas, enquanto a co-localizacao da ANXA1/FPR2 foi investigada nas celulas inflamatorias e tumorais, por meio de reacoes imunocitoquimicas ultraestruturais. As celulas Hep-2 (linhagem humana originalmente descrita como procedente de carcinoma epidermoide de laringe) foram preservadas no meio de cultura MEM (controle) ou tratadas com peptideo ANXA12-26, ANXA12-26 + antagonista Boc2, Dexametasona (Dexa), Dexa + Boc2, e somente Boc2, nos tempos de 6, 24, 48, 72, 96 e 120 horas. Nessas condicoes, analisamos a morfologia e o indice de proliferacao pela microscopia de luz, as citocinas IL-6, IL-8 e MCP-1 pelo analisador Multiplex Magpix, a expressao da ANXA1 e FPR2/ALX pelas reacoes de Western blotting e imunocitoquimica ultraestrutural. Ainda, a expressao dos genes EP3, EP4, MMP2 e MMP9 foi analisada por RT-PCR. RESULTADOS: No estroma laringeo tumoral observamos influxo de Nøs e, ainda, MCs em processo de desgranulacao, triptase positivos (MCT) nas amostras controles e tumorais e, triptase/quimase positivos (MCTC) nas peritumorais, condicoes que caracterizam um microambiente inflamatorio

Three patients, three healthcare providers and three family members participated in the study. Data were collected in semi-structured interviews. The data analysis identified the following categories: smoking background; looking for help for the disease; health, larynx cancer and tobacco; family care and professional care. The study showed the need of broadening the focus of healthcare providers' performance when qualifying these workers by adding paradigms of human sciences in order to enable behavioral changes as to health guidance on habits and/or life styles of users of healthcare services and their family members.
