- ItemSomente MetadadadosAnálise clínica e epidemiológica dos pacientes avaliados pela interconsulta da neurologia no Hospital São Paulo(Universidade Federal de São Paulo (UNIFESP), 2020-03-05) Correa, Karin Mitiyo [UNIFESP]; Pedroso, Jose Luiz [UNIFESP]; Universidade Federal de São PauloBackground: Medical consultation by a specialist physician consists of an evaluation to review diagnosis and management of inpatients receiving care in other specialty wards. This mode of care delivery has gained relevance in the field of neurology and adequate training is of great importance so that neurologists can provide quality care to patients with neurological manifestations. The present study aimed to characterize neurology consultations and to understand the roles of the neurologist within a hospital setting. Methods: The study was based on information from 223 in patients admitted to a university hospital in São Paulo, Brazil, who were evaluated in neurological consultations. Results: There were evaluated 117 female and 106 male inpatients with a mean age of 53.8 ± 2.4 years. The medical specialties that most frequently requested neurological consultations were Internal Medicine (17%), Vascular Surgery (13.9%), Cardiology (11.2%) and Infectious Diseases (9.4%). The main reasons for consultation were seizures (15.7%); neurological examination (9.9%); depressed level of consciousness (9%) and muscle weakness (7.6%). The main neurological conditions diagnosed were preoperative neurological evaluation (11.2%), stroke (8.5%); hypoxic ischemic encephalopathy (3.6%); and sepsis (3.1%). Conclusion: Our findings show the growing importance of the role of neurologists within hospital settings as many medical conditions present with neurological manifestations and the significance of the neurohospitalist model of care, which is now a key topic for discussion.
- ItemAcesso aberto (Open Access)Assessment of a consultation-liaison psychiatry and psychology health care program(Faculdade de Saúde Pública da Universidade de São Paulo, 2002-04-01) Andreoli, Paola Ba [UNIFESP]; Mari, Jair de Jesus [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To investigate the relevance of subjective criteria adopted by a psychiatry and psychology consultation-liaison service, and their suitability in the evaluation of case registries and objective results. METHODS: Semi-structured interviews were conducted and all supervisors of the university hospital service were interviewed. Routinely collected case registries were also reviewed. Standardized assessment with content analysis for each category was carried out. RESULTS: The results showed distortions in the adopted service focus (doctor-patient relationship) and consultant requests. This focus is more on consulting physician-oriented interventions than on patients. DISCUSSION: Evaluation of the relevance of service criteria could help promoting quality assessment of the services provided, mainly when objective criteria have not yet been established to assure their suitability.
- ItemAcesso aberto (Open Access)The frequency of smoking and problem drinking among general hospital inpatients in Brazil - using the AUDIT and Fagerström questionnaires(Associação Paulista de Medicina - APM, 2000-09-07) Figlie, Neliana Buzi [UNIFESP]; Pillon, Sandra Cristina [UNIFESP]; Dunn, John [UNIFESP]; Laranjeira, Ronaldo [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)CONTEXT: Although the CAGE questionnaire is one of the most widely used alcohol screening instruments, it has been criticized for not identifying people who are drinking heavily or who have alcohol related problems but do not as yet show symptoms of alcohol dependence. The AUDIT (Alcohol Use Disorder Identification Test) questionnaire was developed by WHO as a screening instrument specifically designed to identify problem drinkers, as well as those who were already dependent on alcohol. OBJECTIVE: The aim of this study was to use the AUDIT and Fagerström questionnaires in a general hospital inpatient population to measure the frequency of problem drinking and nicotine dependence, and to see if levels varied between medical speciality. DESIGN: Retrospective cross-sectional study. SETTING: Federally funded public teaching hospital. SAMPLE: 275 inpatients from both genders. MAIN MEASUREMENTS: Socio-demographic data, AUDIT (Alcohol Use Disorders Identification Test) and Fagerström Test for Nicotine Dependence. RESULTS: We interviewed 275 inpatients, 49% of whom were men and 51% women. Thirty-four patients were identified as cases by the Audit questionnaire; 22% of the male patients and 3% of the females. Just over 21% of inpatients were current smokers. The gastroenterology (26%) and general medicine (16%) inpatient units had the largest number of individual cases. CONCLUSIONS: Only by knowing the prevalence of alcohol abuse/dependence and nicotine dependence in a general hospital can we evaluate the need for a specialized liaison service to identify and treat these patients.
