Method: A retrospective study was conducted including 837/990 RBC transfusion requisition forms for children and adolescents (0 to 13 years old) filed in between January 2007 and April 2015 by the pediatricians of the emergency room (ER), infirmary ward and intensive care unit (pICU). Transfusion requisition forms belonging to patients with chronic anemia or acute hemorrhage, as well as incompletes requisition forms, were excluded. Results: Trigger, prescribed volume and subtype of RBC concentrates were adequate in 532 (65.3%), 460 (58.8%) and 805 (96.2%) of the transfusions, respectively. When the clinical picture was considered, prescription adequacy was higher compared to the use of the hemoglobin level alone (70.9% vs. 41%). Thus, a classification of package red blood cells according to storage time has been investigated and used in the transfusion practices. The complexity of factors involving the transfusion process and concerns of possible deleterious effects in the transfused patient grounded this investigation regarding the quality of the blood component to be transfused. Objectives: To describe hematological and biochemical parameters of red blood cells and to verify the influence of storage and pre infusion temperature, bag concentrates brand and storage time on hematological and biochemical parameters. Materials and Methods: Descriptive research developed with secondary data of experimental studies accomplished in the Laboratory of Nursing Experiments at the Federal University of São Paulo, between 2015 and 2016. The following parameters were studied in a total of 82 red blood cells: total hemoglobin (g/dL), free hemoglobin (g/dL), hematocrit (%), degree of hemolysis (%), potassium (mmol/L), lactic dehydrogenase (U/L), haptoglobin (g/L), pH and osmolality (mOsm/Kg). For the statistical analysis arithmetic mean, standard deviation, median and interquartile range were used, and the tests of variance, Skewness and Kurtosis, D'Agostino & Pearson, Kolmogorov-Smirnov and Shapiro-Wilk were applied, t Student, Mann-Whitney, One-Way ANOVA (p <0,05). Results: Among the 82 units, 79 (96.34 %) had blood typing A+, 2 (2.44 %) O+ and 1(1.22 %) typing AB+. Regarding the bags brands, 68.29 % were Grifols®, 28.05 % JP Farma® and 3.66 % Fresenius®. There was correlation between higher initial temperature and alterations in levels of lactic dehydrogenase (p=0.040) and osmolality (p=0.020); higher preinfusion temperature and higher levels of free hemoglobin (p=0.003), hematocrit (p=0.002) and osmolality (p=0.020). The red blood cells in the Grifols® bags’ brand showed a statistically significant difference in the potassium level (p = 0.020). In relation to the storage time, longer periods lead to higher levels of free hb levels (p <0.0001), degree of hemolysis (p<0.0001), potassium (p=0.010), lactic dehydrogenase (p=0.0001) and decreased pH (p <0.0001) and haptoglobin levels (p=0.004). Conclusion: The majority of the studied red blood cells presented central tendence measureaments of hematological and biochemical parameters according to national and international standards; red blood cells submitted to higher temperatures and storage for longer periods presented more variations.
