Today the surveys show a significant decrease in consumption in North America, but in Brazil and other countries in South and Central America, the consumption of these substances has increased significantly. There are many advances in research in the areas of neurobiology, neuropsychiatry and neuroimaging that show neuroadaptations from the chronic use of CNS stimulants, such as cocaine, from morphological changes to genetic changes within the so-called brain reward system, modulated primarily by dopamine. Despite this, there are still many controversies regarding the cognitive impairments resulting from the chronic use of substances and the reversibility of these possible deficits. Objective: To evaluate the cognitive functioning of chronic users of crack during early withdrawal and after four weeks of abstinence, comparing with the performance of healthy controls matched by sex, age, economic class and years of schooling. Methods: Fifty-four patients and 35 controls participated in the study. The subjects were evaluated by means of a battery of neuropsychological tests in an interval corresponding to approximately thirty days. The controls were submitted to urine tests for substance detection. RESULTS: The main results showed a loss compared to controls, especially in relation to attention, memory and learning ability, which remained even after the detoxification period corresponding to four weeks. In addition, patients with severe alcohol use performed worse in aspects related to decision making. Conclusions: Despite significant improvement among users, the losses found remained even after a period of four weeks of abstinence when compared with the control group.
