- ItemAcesso aberto (Open Access)Alterações encontradas no potencial visual evocado por padrão reverso em pacientes com esclerose múltipla definida(Conselho Brasileiro de Oftalmologia, 2007-12-01) Andrade, Eric Pinheiro De; Sacai, Paula Yuri [UNIFESP]; Berezovsky, Adriana [UNIFESP]; Salomão, Solange Rios [UNIFESP]; Universidade de Santo Amaro; Universidade Federal de São Paulo (UNIFESP)INTRODUCTION: Multiple sclerosis is an idiopathic demyelinating disease that may affect the optic nerve leading to acute unilateral visual loss, which could be observed by means of evoked visual potential (VEP). This exam is much valued for studying prechiasmatic visual paths in multiple sclerosis. PURPOSE: To analyze the findings of pattern reversal VEP in patients with prior diagnosis of multiple sclerosis and to compare them to visual acuity. METHODS: Twenty-four patients with a definite diagnosis of multiple sclerosis were submitted to pattern reversal VEP from October 2001 to March 2007. RESULTS: In P100 component response, at 15' and 1º stimuli, 19 eyes presented latency response up to 115 ms in both stimuli, which coincided in 6 eyes with latency response between 116 and 135 ms; 11 eyes had a latency response higher than 135 ms, and four eyes did not respond to stimuli. Correlating visual acuity with P100 latency for 15', in Pearson r correlation, r=0.85 with p=0.000000123, and for 1º, r = 0.87 with p=0.0000000338. CONCLUSION: Approximately 60.4% of patients presented abnormalities. However, the correlation between the P100 latency (15' and 1º) and visual acuity was statistically significant; therefore the better the visual acuity, the better the response to stimuli of the pattern-reversal VEP.
- ItemAcesso aberto (Open Access)Retinal nerve fiber layer atrophy as relevant feature for diffuse unilateral subacute neuroretinitis (DUSN): case series(Conselho Brasileiro de Oftalmologia, 2010-04-01) Casella, Antonio Marcelo Barbante; Farah, Michel Eid [UNIFESP]; Souza, Eduardo Cunha de [UNIFESP]; Belfort, Rubens Junior [UNIFESP]; Oguido, Ana Paula Miyagusko Taba; State University of Londrina; Universidade Federal de São Paulo (UNIFESP); UELPURPOSE: To report the findings of optical coherence tomography (OCT) in three cases of diffuse unilateral subacute neuroretinitis (DUSN). METHODS: Three young patients with confirmed diagnosis of diffuse unilateral subacute neuroretinitis were followed-up using Stratus® OCT. RESULTS: Optical coherence tomography findings included retinal nerve fiber layer atrophy and focal edema where the worm was initially lodged. In two patients we could identify the worm by optical coherence tomography as an area of hiper-reflectivity. The worms were laser photocoagulated and post-treatment optical coherence tomography scanning revealed improvement of edema, nerve fiber layer thinning and a retinal hyper-reflectivity where laser had been applied. CONCLUSION: The main finding reported here is the potential of optical coherence tomography in detecting the presence of retinal nerve fiber layer atrophy and focal retinal edema in areas affected by the worm. Optical coherence tomography can be used to distinguish diffuse unilateral subacute neuroretinitis from other mimicking diseases like punctate outer retinitis, when there are no retinal fiber layer atrophy.
- ItemAcesso aberto (Open Access)Seria o Ancylostoma caninum um dos agentes da neurorretinite sub-aguda difusa unilateral (D.U.S.N.) no Brasil?(Conselho Brasileiro de Oftalmologia, 2001-10-01) Casella, Antônio Marcelo Barbante; Machado, Roberto Augusto; Tsuro, Andréa; Hato, Maurício; Costa, Rogerio Alves [UNIFESP]; Farah, Michel Eid [UNIFESP]; Universidade Estadual de Londrina Setor de Oftalmologia; Universidade Federal de São Paulo (UNIFESP)The autors present a confirmed case of diffuse unilateral subacute neuroretinitis (D.U.S.N.) with identification of the worm in a 6-year-old child with symptoms of cutaneous larva migrans, as well as analysis of 7 other cases of D.U.S.N. and association with cutaneous larva migrans or positive sorology for Toxocara canis. This is the first described case in the literature of the association between D.U.S.N. and cutaneous larva migrans in its active form.
