- ItemAcesso aberto (Open Access)Complicações intra e pós-operatória da dacriocistorrinostomia externa(Sociedade Brasileira de Oftalmologia, 2011-12-01) Lorena, Silvia Helena Tavares [UNIFESP]; Silva, João Amaro Ferrari [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)PURPOSE: The research project goal is to determine the incidence of complications during the external dacryocystorhinostomy and post surgery done by the author, in São Paulo Hospital, from april 10th , 2009 to january 15th, 2010. METHODS: This is a retrospective study of the records of the patients submitted to external dacryocystorhinostomy in São Paulo Hospital, from april 10th 2009 to january 15th, 2010.The patient were submitted to the ophtahalmologic evaluation: Ocular inspection, biomicroscopiy, Milder Test, Test of Jones I and II and dacryocystograph exam. It is important point out, that the included patients in this study, the patients were examined by the author of this work. RESULTS: This study was verified that out of 15 external dacryocystorhinostomy done, the complications were: bleeding (46,66%), oddment loss of the nasal mucosa (26,66%,utilização) of the hammer in the osteotomy (6,66%).The pos-operatory complications: cutaneous fistula (6,66%), hypertrophic (6,66%). Among the 15 surgeries there was only 1 failure corresponding to the 6,66%. The average time of surgery follow-up was of 4 months. CONCLUSION: The external dacryocystorhinostomy offers great results,according this study.
- ItemAcesso aberto (Open Access)Complicações na dacriocistorrinostomia transcanalicular com laser diodo: complications(Conselho Brasileiro de Oftalmologia, 2009-08-01) Garcia, Eduardo Alonso [UNIFESP]; Cintra, Pedro Paulo Vivacqua Cunha; Universidade Federal de São Paulo (UNIFESP)PURPOSE: To evaluate the complications of the use of diode laser in the treatment of acquired nasolacrimal obstruction. METHODS: Forty four procedures (transcanalicular dacryocystorhinostomy with diode laser with bicanalicular silicone tube intubation and local anesthesia) where performed from February 2002 to November 2007 in 41 patients (3 bilaterally), 32 women and 9 men. RESULTS: The most common intraoperative complications were disability to pass the Crawford probe (13.6%) and the laser probe (11.3%). Regarding postoperative complications, epiphora was the event of higher frequency (15.9%) followed by the non-intentional silastic extrusion by the patient (11.3%). CONCLUSION: Intraoperative and postoperative complications rate were similar of others articles that demonstrated the same surgical technique (with same laser).

In the case of the acquired form, the etiology classifies in not-specific (it involves the coating epithelium and/or the vascular plexus submucosae of the lacrimal duct) and specific (infections, inflammations, tumours and iatrogenies). The diagnosis is established by means of the anamnesis and clinical evaluation based on semiology of the lacrimal sack. The patients with lacrimal obstruction present epiphora, acute pains and swelling, mostly in the region of the lacrimal sack causing ectasye of various degrees whichn culminate with cutaneous fistulization. The image exams routinely proposed for dacryocysto-graphy, which represent a great help in showing the best surgical technique for dacryocystorhinostomy, will be indicated as soon as the acute inflammatory state is treated.
- ItemAcesso aberto (Open Access)Estudo epidemiológico da dacriocistite crônica(Sociedade Brasileira de Oftalmologia, 2011-12-01) Lorena, Silvia Helena Tavares [UNIFESP]; Silva, João Amaro Ferrari [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)PURPOSE: The goal of this project is to document the epidemiological characteristics of chronic dacryocystitis cases assisted in a public hospital. METHODS: This is a retrospective study of medical records of patients who developed chronic dacryocystitis assisted in the period of january 12th , 2009 to june 9th, 2010 .The patients were ophthalmologist evaluated: ocular inspection, biomicroscopy, Milder Test, Test of Jones I and II. RESULTS: The gender with chronic dacryocystitis belonged to 55 years. There were 73 % female and 27 % male.The unilateral involvement occurred in 95% of the cases and only 5 % were bilateral. In this work, the side of this pathology was on the right in 45% and on the left in 55%. Negroid races have a lower rate of illness than caucasians.Regarding the chronic dacryocystitis ethiology, there was a predominance of the idiopathic in 95% of the cases and only 5% of trauma. CONCLUSION: The dacryocystitis is more frequent in women, rare in blacks and presents hereditary predisposition, increasing the incidence from the 6th decade
- ItemAcesso aberto (Open Access)Estudo retrospectivo da obstrução congênita do ducto lácrimonasal(Sociedade Brasileira de Oftalmologia, 2011-04-01) Lorena, Silvia Helena Tavares [UNIFESP]; Silva, João Amaro Ferrari [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)PURPOSE: The goal of this research is to relate the results of congenital lacrimonasal duct obstruction treatment and epidemic aspects in children. METHODS: This is a retrospective study of 21 children with congenital lacrimonasal duct obstruction attended in the period of 9 months approximately (april 2009 to january 2010). The children were evaluated regarding the sex, age, and side of obstruction (right, left or bilateral). RESULTS: Nine of the children were female and 12 were male (42.85% versus 57.14%. Therefore there was no significant statistically difference related to gender. The obstruction occurred bilaterally in 38% of the cases, on the right in 28,57% and on the left in 33,33%. Seven children were operated on for the first time before 1 and a half year old (1st Group) with a 100% of success. In 10 children the surgery were performed between 2 an 3 years old (2nd Group) with 80% of success and on the 3rd group there was 3 children (above 3 and till 7 years old) that were treated surgically with 100% of unsuccess. CONCLUSION: The congenital lacrimonasal duct obstruction were not related to the gender of the children. The treatment were very succefull during the 1st year of age.

To analyze the various etiologies of the obstruction was the goal in the present study. Methods: The study was carried out on 63 lacrimal apparatus of 59 patients who were submitted to external examination, biomicroscopy, Jones' I and II tests and canaliculus measurement. Results: The most important etiologies were chronic dacryocystitis and failure of dacryocystorhinostomy (53.9%) followed by traumatisms caused by aggression, car accidents and a dog bite. Agenesis contributed with 9.5% and less frequent were canaliculus inflammations and infections, surgeries, radiotherapy and idiopathic obstructions. Conclusions: There is no diference between the findings of this study and those re-ported in the international literature.
- ItemAcesso aberto (Open Access)Resultados da conjuntivodacriocistorrinostomia com implante de prótese lacrimal de polietileno de baixa densidade(Conselho Brasileiro de Oftalmologia, 2002-01-01) Bison, Simone [UNIFESP]; Soccol, Ovídio; Scarpi, Marinho Jorge [UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Santa Casa de São Paulo Departamento de OftalmologiaPurpose: To analyze the postoperative complications of conjunctivodacryocystorhinostomy with low-density polyethylene tube and the possible correlations between the various etiologies and complications and surgical success rate. Methods: The study was carried out on 35 lacrimal apparatus of 34 patients who were submitted to conjunctivodacryocystorhinostomy with low-density polyethylene tube during a six-year period, from August 1991 to November 1997. With the aim of studying the possible correlations between the findings (etiology x complications and etiology x success rate) exact Fisher's test was carried out. Results: The most important etiologies were chronic dacryocystitis and failure of dacryocystorhinostomy (53.9%) followed by traumatisms (16.3%). The complication rate was 74.3%. The surgical success rate in one intervention was 25.7%. After two or more surgeries, 17 tubes became pervious and well-located (48.6%). Unsuccessful procedures occurred in 25.7% of the lacrimal apparatus. There was no significant correlation between etiology and complication occurrence neither between etiology and surgical success. Conclusions: There is no difference between complications and success in this study and those reported in the international literature. These findings were not correlated with the obstruction etiology. The advantages of low-density polyethylene tube are its great availability, the easy manufacture in the intraoperative period according to the lenght and width needs of and, finally, it can be exchanged for borosilicate glass when the edema regress.

Methods: A retrospective study of 10 subjects with a normal lacrimal system (Group 1) and 10 with chronic dacryocystitis (Group 2) diagnosed according to B-mode ultrasound with a 10-MHz transducer and the direct-contact technique (AVISO, Quantel Medical) for lacrimal sac assessment. We analyzed the dimensions, features, and content of the sacs. Characteristics of the population: female: 6, Group 1; 8, Group 2; mean age 48.4 years (SD=19.9; range, 22-80 years), Group 1; 50.5 years (SD=15.5; range, 25-75 years), Group 2. Results: The dimensions of the lacrimal sac were as follows: anteroposterior 1.86 and 10.99 mm in Groups 1 and 2, respectively, p<0.0001; vertical 9.79 and 14.13 mm in Groups 1 and 2, respectively, p=0.049. Qualitative evaluation of the lacrimal sac contents showed hypoechogenic content in Group 1 (10, 100%) and hyperechogenic punctiform content in Group 2 (10, 100%) with partial filling in seven cases (70%). Conclusions: Ultrasonography can differentiate normal lacrimal sacs from sacs compromised by chronic dacryocystitis, thus being useful as an adjunct to clinical examination and surgical planning.
