- ItemSomente MetadadadosAtraso E Regressão Do Desenvolvimento(Universidade Federal de São Paulo (UNIFESP), 2017-05-25) Silva, Mayara Cantalice Vogel Da [UNIFESP]; Rodrigues, Marcelo Masruha [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Neurological development consists of a continuous and sequential process in which the human being acquires skills ranging from simple and disorganized movements to highly complex activities.1,2 Developmental disorders are highly prevalent and are estimated to occur in 1% 3% of children under 5 years of age.3 Thus, they represent one of the main manifestations of neurological diseases in the pediatric age group and are among the main complaints in the specialist's office. Early diagnosis is very important, allowing the implementation of strategies that enable the patient reach their maximum potential.4 With regard to the pattern of different areas of neurodevelopment, the changes can be classified as being part of: 1. Global delay (impairment of two or more areas); 2. Dissociation (isolated delay or much more marked in one of the areas); 3. Deviation from the expected sequence of acquisitions.5 As for temporal progression, development can be classified as: Regular (constant speed of acquisition of skills); Stagnant (when no new gains are made); Regressive (when there is loss of milestones already achieved) .6 Lesions located in different regions of the child's nervous system may manifest with developmental delay. However, peripheral lesions will fundamentally impair motor development, as well as myopathies. Brain lesions, which are the main cause of developmental changes, are most often manifested by a number of changes, such as delayed motor development, language impairment, intellectual impairment, and impairment of other higher cortical functions (such as attention, memory, executive functions), among others. Patients with signs and symptoms of brain dysfunction caused by permanent and non-progressive lesions are classified syndromically as having a static, non-progressive or non-progressive encephalopathy, with overall developmental delay being one of their main characteristics. On the other hand, those individuals who present signs and symptoms of brain dysfunction caused by progressive lesions are framed, also syndromic, as having progressive or progressive encephalopathy, and developmental regression is their main characteristic. The differentiation between static and progressive frames is not always easy. For example, progressive diseases often present initially, and sometimes for long periods, with developmental delays. Similarly, when this type of disease manifests itself in the first weeks or months of life, there may be a serious impairment of development, preventing any type of gain and making it impossible to perceive the loss of any previous acquisition. Otherwise, static frames can evolve with pseudoregression, a phenomenon that will be discussed later in this chapter.
