In children and adults with chronic kidney disease (CKD) increased levels of the orexigenic hormone ghrelin are often found. However, no data exits in relationship to concentration of acylated ghrelin in pediatric patients with CKD. Methods Cross-sectional study of acylated and total ghrelin plasma levels in pediatric patients with mild CKD undergoing conservative treatment (MCKD group, n = 19) and patients with end stage renal disease undergoing hemodialysis (ESRD group, n = 24) compared with healthy controls (n =20). The correlations between total or acyl ghrelin with leptin, GH, IGF-I, glomerular filtration rate (GFR) and anthropometric and nutritional measurements were also undertaken. Results ESRD patients had significantly lower BMI Z-score and energy intake while both ESRD and MCKD groups had lower height-for-age Z-score than control group. ESRD patients also exhibited higher total ghrelin levels (2009.7±1278.0 pg/ml, mean±SD) than either MCKD (1117.5±891.9 pg/ml) or controls (655.3±255.6 pg/ml). However, plasma acyl ghrelin levels did not differ between groups. The ESRD group had normal GH but low IGF-I levels. When all 43 uraemic subjects were combined, total ghrelin correlated positively with GH (r =0.340, p=0.0255) and negatively with IGF-I (r= - 0.415, p=0.0057) and GFR (r= -0.534, p<0.0002). Both total and acyl ghrelin correlated negatively with nutritional status. Conclusion The present findings suggest that most of the increased total ghrelin in CKD pediatric patients is desacylated. As desacyl ghrelin has been shown to inhibit feeding, its high levels may contribute to malnutrition and growth deficit in CKD patients.

After Highly Active Antiretroviral Therapy (HAART) was started, malnutrition has declined and HIV-associated lipodystrophy syndrome has emerged as an important metabolic disorder. Aids is also characterized by hormonal disturbances, principally in growth hormone/insulin-like growth factor 1 (GH/IGF-1) axis. The use of recombinant human GH (hrGH) was formerly indicated to treat wasting syndrome, in order to increase lean body mass. Even though the use of hrGH in lipodystrophy syndrome has been considered, the decrease in insulin sensitivity is a limitation for its use, which has not been officially approved yet. Diversity in therapeutic regimen is another limitation to its use in Aids patients. The present study has reviewed the main HIV-related endocrine-metabolic disorders as well as the use of hrGH in such conditions.

ESRD patients had higher total ghrelin levels than those with mild CKD or control individuals. However, acyl ghrelin did not differ between groups, indicating that the excess circulating ghrelin was desacylated. Since desacyl ghrelin has been shown to inhibit appetite, increased levels might contribute to protein-energy wasting in pediatric renal patients. When all 43 renal patients were combined, multiple regression analysis found age and glomerular filtration rate (GFR) to be significant negative predictors of total ghrelin. Acyl ghrelin was influenced negatively by age and positively by energy intake. Acyl to total ghrelin ratio related positively to GFR and energy intake. the results indicate that total but not acyl ghrelin is influenced by low GFR in children with CKD and suggests that ghrelin activation may be impaired in these patients. Since energy intake is a positive predictor of acyl ghrelin, the physiological control of ghrelin secretion appears to be altered in pediatric renal patients.

METHODS: Twenty-four samples (Graves' ophthalmopathy patients) underwent lateral rectus muscle and surrounding fibrous and adipose tissue biopsy. The control group was obtained by strabismus surgery. Correlation between clinical- ophthalmologic, endocrinological, ultrasonographic findings, and immunohistochemical expression was performed. RESULTS: IGF-1: There were 7 positive cases (29.2%). There was a direct relation with higher CAS (clinical activity score) in all of them and if only CAS equal or higher than 5 was considered, this was 54.5%. FGF: There was expression in 5 cases (20.8%) with a direct relation in all those with higher CAS (>5) (45.4%). VEGF: There were two positive cases (8.3%) for VEGF in endothelial cells, in these cases the patients also presented CAS higher than 5. There was no expressions of all growth factors in the control group. CONCLUSIONS: All patients, except one, with positive expression of FGF, IGF-1 and VEGF showed CAS greater than 5, suggesting in this way an important role of these growth factors in the pathogenesis and severity of Graves' ophthalmopathy. However, statistical analysis revealed only significant association between IGF-1 and male sex (P=0.034). Low ultrasound reflectivity and endocrine status may not correlate directly with disease activity or with immunoexpression of growth factors and c-erbB-2/HER-2/neu.
