- ItemSomente MetadadadosA expressão da desigualdade social na mortalidade infantil, no município do Embu(Universidade Federal de São Paulo (UNIFESP), 2003) Ventura, Renato Nabas [UNIFESP]; Oliveira, Eleonora Menicucci de [UNIFESP]O objetivo deste trabalho foi construir uma explicacao processual, a luz das classes sociais, para o comportamento da mortalidade infantil no municipio do Embu, estabelecendo uma sintese explicativa para o fenomeno, dentro do marco conceitual da desigualdade social e propondo formas de enfrentamento do problema por meio de acoes de Saúde Publica. E um estudo que se da no campo da medicina e busca outras abordagens, como a das Ciências humanas, em especial a da sociologia, atraves da utilizacao de uma abordagem quantitativa e qualitativa referenciada numa concepcao metodologica denominada paradigma emergente, que se contrapoe ao modelo de racionalidade da Ciência moderna. Parte de um estudo realizado em 1996, que avaliou os diferenciais intra-urbanos atraves da construcao de indicadores compostos para estudo das condicoes de vida nas diferentes areas do municipio e agrega um segundo estudo que utilizou a realizacao de entrevistas fechadas as familias das criancas que faleceram antes de completar um ano de vida, residentes no Embu, nos anos de 1996 e 1997, construindo, com os dados obtidos de fonte secundaria (declaracoes de obito) e de fonte primaria (entrevistas dos dois estudos), categorias analiticas, segundo o lugar que a familia ocupa na estrutura produtiva e as condicoes de reproducao social. Mostrou, apos a analise das categorias - condicoes de vida, trabalho e renda, escolaridade, cenario reprodutivo, acesso a servicos de Saúde, condicoes do meio ambiente - que a forma de ocorrencia dos obitos no primeiro ano de vida e uma expressao das desigualdades sociais que surgem em decorrencia da divisao social do trabalho, da dominacao do capital sobre o trabalho e agravadas pelas condicoes economicas e sociais determinadas pela hegemonia neoliberal. E por fim, discute que o entendimento das inter-relacoes que envolvem o fenomeno da mortalidade infantil tem o objetivo de politizar o enfrentamento do problema, devendo considerar as diferentes oportunidades de acesso aos bens e servicos de cada classe social e, propoe uma intervencao de carater intersetorial e interdisciplinar, como forma de minorar os efeitos da desigualdade, em contraposicao as politicas de focalizacao que visam, unicamente, a diminuicao do coeficiente de mortalidade infantil

However, although the term ‘care network’ has been widely spread, there is little research on the underlying principles of this approach, what points out to the relevance of understanding this concept since the perspective of those who use it in their daily practice. Purpose: The purposes of this research were: 1) to understand the social representations of professionals working in public services about networks as a strategic approach to assist people with substance-related disorders; and 2) to observe the main challenges they face in their daily practice and the resources used to overcome them. Methods: To reach the research purpose, it was conducted two qualitative studies, with professionals from different services of the public system, working with people with substance-related disorders. The participants were taking part in a course on addiction issues, promoted by the CRR-DIMESAD-UNIFESP, in 2016. The study 1 observed the social representations of 179 professionals about the network concept and was carried out from self-completion questionnaires, submitted to subsequent correspondence analysis. The Study 2 investigated, through focus groups, the challenges and overcoming strategies used by professionals in the network care assistance; 24 professionals took part in this research phase and the data were generated through the content analysis of the focus groups. Results: According to the data collected in both studies, the care network was associated with the idea of the multidisciplinary teamwork, and the main action usually conducted on the care network was the patient referral to other services. The results also pointed out the professional´s perception of the users as needy people. Concerning the obstacles that hamper the care network system, it was identified structural obstacles (lack of material and human resources) and proceeding obstacles (poor communication between services; high turnover of the staff; professional´s poor training on addiction issues; stigma and prejudice against people who use drugs). Training courses on the theme, the provision of supervision to professionals and evaluations of the care network actions emerged as some possibilities of overcoming these obstacles. Discussion: The common procedure of referrals between services, which emerged as one of the main actions in the care network system, seems to impair its potential of promoting the integration of important actors on his field. However, despite the innumerable challenges faced by the professionals in their daily routine (both structural and proceeding obstacles) – source of a huge suffering – a positive perception of the care network system remained to the professionals. Both studies pointed out the relevance of training courses focused on addiction issues to professionals, in order to help them to deal with the complexity involved on this phenomenon, contributing to reduce the stigma and the prejudice that exists around this theme.
