- ItemAdaptação e correlação entre instrumentos neuropsicológicos para avaliação e acompanhamento da doença de Alzheimer em fases avançadas(Universidade Federal de São Paulo (UNIFESP), 2012) Wajman, José Roberto [UNIFESP]; Bertolucci, Paulo Henrique Ferreira [UNIFESP]Objetivo: foi objetivo principal deste projeto o estudo da correlacao entre instrumentos especificos para avaliacao de pacientes com demencia da doenca de Alzheimer em fases avancadas no que diz respeito a sua cognicao objetivamente ponderada e aspectos da capacidade funcional. Metodos: foram utilizadas nesta pesquisa as escalas CDR (Clinical for Dementia Rating), FAST (Functional Assessment Scale), BANS (Bedford Alzheimer Nursing Severity Scale), MMSE (Mini-Mental State Exame), MMSE-G (Mini-Mental State Exame-Grave), SIB-8 (Severe Impairment Battery) e TSI (Test for Severe impairment) para comparacao com uma escala ecologica que serviu como padraoouro, a PADL (Performance Activities of Daily Living). Como proposta estatistica foram aplicadas analises descritivas, analise de Variancia (ANOVA), o Teste de Kruskal-Wallis para efeito de comparacao, alem de ter sido adotado nivel de significancia de 5% para as analises inferenciais. Resultados: foram incluidos 95 individuos acompanhados junto ao Setor de Neurologia do Comportamento do Hospital São Paulo - UNIFESP, sendo 33 (34,7%) do genero masculino e 62 (65,3%) do feminino. A idade media dos 95 individuos foi de 74,7 anos, variando de 60 a 89 anos, com desvio padrao de 6,2 anos. Em media, o tempo de escolaridade foi de 4,2 anos, variando de 3 a 8 anos, com desvio padrao de 1,5 anos. O tempo medio de doenca dos individuos foi de 7,3 anos, variando de 5 a 12 anos, com desvio padrao de 1,7 anos. Conclusoes: as evidencias encontradas sugerem uma correlacao linear crescente e estatisticamente significante entre a PADL e os instrumentos MEEM-G, SIB-8 e TSI. Houve, ainda, relacao estatisticamente significante entre os resultados alcancados na escala PADL uma vez comparada com o MEEM-G, a SIB-8 e a TSI e a fase da doenca em que os pacientes se encontravam, na medida em que quanto mais prejudicados foram seus desempenhos nesses tres testes, maior o indice de progressao da doenca na escala funcional FAST e em suas subdivisoes. Estes resultados indicam que para populacoes com estas caracteristicas escalas especificas para demencia grave sao uteis na avaliacao mais acurada do estagio da doenca e sua progressao

- ItemAnálise comparativa entre as escalas funcionais do membro superior WMFT e ARAT utilizadas na avaliação da terapia por contensão induzida em pacientes com AVC isquêmico(Universidade Federal de São Paulo (UNIFESP), 2008-07-30) Assis, Rodrigo Deamo [UNIFESP]; Massaro, Ayrton Roberto [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)The Constraint-induced Movement therapy (CIMT) is an intervention which main goal is the recuperation of affected upper extremity in hemiparetic patients with acquired encephalic lesion. Its protocol consists by an intensive treatment of two consecutive weeks with six hours of exercises and the wear of arm sling in non affected upper arm during 90% of daily activities. The motor improvement of CIMT is validity by the functional evaluation scales of upper extremity (FESUE) Wolf Motor Function Test (WMFT) and Action Research Arm Test (ARAT). Objective: Realize a comparative analysis between both scales. Methodology: The FESUE had been applied during four different times in 17 patients with diagnosis of ischemic stroke, who made individually the CIMT during two weeks and six hours of exercises and after made a comparative analysis between the FESUE by time for application, time for preparation of the table, reproducibility and factorial analysis. Results: Decreased in score of FESUE WMFT and increased in score of FESUE ARAT after CIMT, no significant variation in the score between before and after CIMT of the FESUE, decreased in time to preparation the table and application of FESUE in both scales and the factorial analysis showed two and three qualitative components in FESUE ARAT and WMFT, respectively. Conclusions: Both FESUE can measure the therapeutics gains of CIMT, with high reproducibility, but the time for application and preparation of the table are minor in FESUE ARAT and only the FESUE WMFT shows the qualitative component “laterality”.
- ItemAnálise Da Interface Entre A Disfagia E As Implicações Nutricionais Nos Pacientes Com Doença Do Neurônio Motor(Universidade Federal de São Paulo (UNIFESP), 2017-01-31) Alves, Percilia Cardoso Lopes [UNIFESP]; Oliveira, Acary Souza Bulle [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: Amyotriphic Lateral Sclerosis (ALS)is a disease in which there is impairment of the lower and upper motor neurons and the Bulbar Progressive Palsy (BPP) of the lower motor neuron of the brainstem; Both are within the Motor Neuron Disease (MND) group. Muscle weakness is a common sign that can trigger dysphagia. Purpose: To analyze the interaction between dysphagia and nutritional implications in the patient with MND. Method: Through observational, transversal, analytical and prospectiveresearch, 59 patients were evaluated, being 42 (71.18%) with ALS (47.62% male and 52.38%, female) and 17 (28.82%) with BPP (7 (41.17%) male and 17 (58.83%) female. The patients underwent speech therapy and nutritional evaluation; moreover, have been applied: functional range of consistencies (FOIS), scales of functionality (ALSFRS-R and EGELA) and respiratory assessment (peak cough flow).Results: 100% of patients with BPP presented oral and pharyngeal phase changes, while patients with ALS had less changes in oral (66.67%) and pharyngeal phase (73.80%). In nutritional evaluation, malnutrition was observed in 35.71% of patients with ALS and 23.52% of the patients with BPP. 28% (ALS) and 41.17% (BPP) of the patients made use of enteral route.Conclusion: Dysphagia was present in all patients with diagnosis of BPP, relating to increased frequency of malnutrition. Body mass index and Protein Energy Malnutrition score showed correlation with ALSFRS-R functionality. There was correlation between body mass indexes, the scale of food consistency (FOIS) and the peak cough flow.

Inputs e outputs dependem da conectividade sináptica da rede neuronal e das propriedades individuais das sinapses, estando associadas à atuação de moléculas de adesão facilitadoras do processo. O cérebro em desenvolvimento é altamente excitável e susceptível às crises convulsivas por estar em um período de crescimento de axônios e dendritos, formação das sinapses e maturação das redes corticais, dependentes da idade. O Status epilepticus (SE) caracteriza-se como uma condição aguda de convulsões repetitivas e/ou prolongadas. Afim de mimetizar a condição humana, o SE pode ser induzido pelo modelo experimental de injeção de cloridrato de pilocarpina. Em animais aos 9 dias após o nascimento (P9), o SE como um insulto único pode acarretar consequências deletérias como aumento/diminuição da expressão protéica dos transportadores gabaérgicos, glutamatérgicos, cotransportadores de membrana e moléculas de adesão sináptica, que em última instância, pode conduzir à um prejuízo na funcionalidade cerebral. Frente à alta refratariedade ao tratamento farmacológico apresentada pelos pacientes com epilepsia, faz-se necessário a busca de tratamentos alternativos para lhes garantir melhores condições de vida. Evidências experimentais apontam a importância dos ácidos graxos poliinsaturados (PUFAs) no desenvolvimento normal do cérebro, manutenção de sua função e prevenção de algumas doenças neurológicas crônicas. Resultados prévios obtidos em nosso laboratório demonstraram que o tratamento com óleo de peixe com início logo após o SE, apresentou efeitos neuroprotetores e neuroplásticos. No intuito de avaliar a expressão protéica nas idades P3, P10, P17 e P60 de GABAT1, EAAT2, NKCC1, KCC2, PSD95, Neuroligina 2 e Neurologina 1 em animais submetidos ao SE neonatal em P9, foram formados 4 grupos experimentais: Grupo Pilo óleo de peixe (PO); Grupo Pilo Veículo (PV); Grupo Controle óleo de peixe (CO) e Grupo Controle Veículo (CV). Nossos resultados mostram que houve diminuição da expressão protéica no grupo experimental (SE) apenas em P10 para as proteínas NKCC1, EAAT2 e Neuroligina 2. Assim, afirmamos que o SE em fase precoce da vida tem relevante impacto na circuitaria excitatória/inibitória. Para as demais análises realizadas não verificamos diferenças significantes nem na comparação em relação ao tratamento proposto. Entretanto não podemos descartar que a suplementação com óleo de peixe exerça efeito relevante sobre o neurodesenvolvimento ou sobre as resposta adaptativas ao longo da vida.

This study had the following objectives: a) to analyze the motor and anxietyassociated behaviors of rats submitted to a protocol of chronic alcohol ingestion for four weeks; b) to analyze, in the brains of the same animals, the immunoreactivity to the neurotransmitters Dopamine and CRF. Material and Methods: Twentyfour Wistar rats from CEDEME/UNIFESP were submitted to a voluntary ingestion protocol of alcoholic solution 20% (twobottle choice paradigm) of an intermittent nature (alcohol was offered on Mondays, Wednesdays, and Fridays) for four weeks. The Open Field test was used for cerebral motor activity analysis and the Elevated Plus Maze test for the analysis of the behaviors associated with anxiety. After perfusion and histological preparation of brain tissue containing areas of interest of the dopaminergic and CRFergic systems, immunoreactivity analysis was performed on the respective systems utilizing the immunoperoxidase technique. Results: The Open Field test analysis allowed the classification of the alcohol consuming animals in high motor activity and low motor activity. The results obtained from the Elevated Plus Maze test showed that the high activity animals produced fewer fecal boli indicating an anxiolytic effect of alcohol on these animals. Immunohistochemical analysis showed that animals that chronically ingested alcohol solution showed lower immunoreactivity to dopamine when compared with the control group. Conclusions: Through the present study, it can be concluded that animals that consumed alcoholic solution in a chronic and intermittent manner could be classified as animals of high or low motor activity and those classified as high motor activity animals were those that consumed significantly more alcohol. No anxiolytic effect of ethanol was observed in the behavioral tests, as well as no effect on the brain stress system. However, we observed an inhibitory effect of chronic alcohol consumption on the dopaminergic reward brain system, regardless of the motor activity classification.
- ItemAnálise morfológica e funcional do coração de ratos submetidos ao modelo de Epilepsia induzida pela Pilocarpina(Universidade Federal de São Paulo (UNIFESP), 2009-03-25) Colugnati, Diego Basile [UNIFESP]; Scorza, Fulvio Alexandre [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Epilepsy is one of the most common neurologic problems worldwide. Approximately 3 percent of persons in the general population will have epilepsy at some point in their lives. Unfortunately, individuals with epilepsy are at higher risk of death than the general population and SUDEP is the most important direct epilepsy-related cause of death. SUDEP is responsible for 7.5% to 17% of all deaths in epilepsy and has an incidence among adults between 1:500 and 1:1,000 patient-years. A number of associated factors for SUDEP have been reported but the results are not wholly consistent between studies. These include refractoriness of the epilepsy, presence of generalized tonic-clonic seizures, polytherapy with antiepileptic drugs, young age, duration of the seizure disorder ranging from 15 to 20 years, early onset of epilepsy and winter temperatures. Additionally, potential pathomechanisms for SUDEP are unknown, but it is very probable that cardiac arrhythmia due to myocardial ischemia play a potential role. Based on this factor, we evaluated the cardiac function and morphology of rats during the three evolution periods (acute, silent and chronic) of the pilocarpine model of temporal lobe epilepsy. From functional point of view, we analyzed the electrocardiogram and the autonomic heart modulation. For that, both autonomic modulation and electrocardiogram were verified without, before seizure and during seizure. It is important to note that all analyses were compared to the age-matched control group. During seizure period, our results showed an intense cardiac rhythm alteration due an important influence of parasympathetic nervous system. On the other hand, in the seizure free period, we found a large sympathetic activity in the rats with chronic epilepsy in relation to the control group. From a morphological point of view, we found specific histological features in the rats with epilepsy, such as necrosis and mitochondrial proliferation. On the whole, our results suggest that chronic epilepsy condition lead to alterations on both autonomic modulation and cardiac morphology that may contribute to the higher risk of sudden unexpected death in epilepsy.

The lumbar fusion is one of the most commonly performed spinal procedures in the world, and interbody fusion has been one of the most effective strategies. There is a lack of cataloging of data regarding intervertebral disc height in Brazilian population and surgical implant materials are usually imported from other countries or are manufactured in Brazil based on foreign models. Objective: 1) To evaluate and describe the height pattern of normal intervertebral discs in a Brazilian population. 2) To analyze the adequacy of dimensions of inter-market devices available in the market to our population. 3) To analyze the difference in anterior and posterior height of the cited discs, evaluating the role of this difference in the formation of lumbar lordosis. 4) Analyze individuals of different genders, ethnic and age groups and describe the peculiarities of each subgroup. 5) To propose a coefficient to 6 calculate the ideal ratio between anterior and posterior height in the different segments studied, obtained by dividing the anterior and posterior height of the discs. Methods: A retrospective study based on analysis of images obtained in lumbar spine magnetic resonance imaging. We included male and female patients who underwent lumbar spine resonance. Patients with previous surgeries, advanced degenerative disc disease, suspected infection, deformities were excluded from the study. Results: 300 patients had their images analyzed for the anterior and posterior height of the L3-L4, L4-L5 and L5-S1 discs, totaling 900 intervertebral discs. The mean anterior disc height of L3-L4 was 9.30 mm, L4-L5 10.94 mm and L5-S1 12.41 mm. About 7% of the patients have a L3-L4 anterior height less than 7mm, which is usually the smallest available cage height. The ratio obtained by dividing the anterior height by the posterior height was 1,43 for L3-L4; 1,57 for L4-L5 and 1,93 for L5-S1. Conclusions: L3 to S1 discs present greater anterior height than posterior and this difference tends to decrease with aging. The L5-S1 disc is that most contributes for lumbar lordosis, followed by L4-L5. Populations with lower average height should have their disc dimensions carefully studied during surgical planning, especially in L3-L4 segment.
- ItemAnestesia para hipertermia maligna no contexto do transplante renal: avaliando a transferência de halogenados pelo órgão transplantado(Universidade Federal de São Paulo, 2022-10-24) Neder Neto, Calim [UNIFESP]; Silva, Helga Cristina Almeida da [UNIFESP]; Neville, Mariana Fontes Lima [UNIFESP]; http://lattes.cnpq.br/7466641350442181; http://lattes.cnpq.br/0844918862241102; http://lattes.cnpq.br/7462651305534896Introdução: hipertermia maligna (HM) é uma doença farmacogenética rara que ocorre em indivíduos geneticamente suscetíveis em resposta à exposição a anestésicos inalatórios halogenados ou succinilcolina. O quadro típico de hipermetabolismo e rabdomiólise pode ser mascarado no paciente submetido a transplante, principalmente renal, tornando o diagnóstico difícil. Além disso, mesmo que o paciente suscetível à HM não use halogenados, questiona-se a possibilidade dos agentes desencadeantes halogenados serem carreados com o tecido transplantado. Objetivo: verificar a possibilidade de anestésicos halogenados serem carreados do doador para o receptor de transplante renal e determinar fatores que influenciam esse processo. Métodos: o estudo foi do tipo observacional transversal controlado. A fração expirada de anestésicos inalatórios halogenados foi quantificada em pacientes receptores submetidos à anestesia geral do tipo venosa total para transplante renal, cujos respectivos doadores foram submetidos à anestesia geral do tipo balanceada com anestésicos inalatórios halogenados. Os pacientes receptores e doadores vivos de rim foram divididos da seguinte forma: 10 pacientes (5 duplas receptor/doador) para o grupo sevoflurano e 10 pacientes (5 duplas receptor/doador) para o grupo desflurano. A medição da fração expirada de anestésico halogenado no receptor foi feita por analisador de gases Vamos®. Resultados: os pacientes doadores que foram submetidos à anestesia combinada peridural e geral balanceada com desflurano eram em sua maioria pacientes do sexo feminino (4, 80%), apresentavam uma média de idade de 41,8 ± 7,79 anos (intervalo 32-51 anos), eram em sua maioria eutróficos (4 eutróficos e 1 sobrepeso) com média de IMC de 22,1 ± 2,14 kg/m2, avaliados em sua maioria como ASA I (4, 80%) e apenas um como ASA II (20%). O tempo médio de exposição dos rins dos pacientes doadores ao desflurano foi de 37,4 ± 5,50 minutos (intervalo 30-45 minutos). O tempo médio de duração total do procedimento dos pacientes doadores do grupo desflurano foi de 125,0 ± 16,95 minutos (intervalo 105-145 minutos). Nenhum receptor do grupo desflurano exalou halogenado após reperfusão renal. Os pacientes doadores que foram submetidos à anestesia combinada peridural e geral balanceada com sevoflurano eram em sua maioria pacientes do sexo feminino (4, 80%), apresentavam uma média de idade de 42,2 ± 9,09 anos (intervalo 29-54 anos), em sua maioria com sobrepeso (2 eutróficos e 3 sobrepesos) com média de IMC de 28,9 ± 4,20 kg/m2, avaliados em sua maioria como ASA I (3, 60%) e dois como ASA II (40%). O tempo médio de exposição dos rins dos pacientes doadores ao sevoflurano foi de 49,6 ± 14,77 minutos (intervalo 29-68 minutos). O tempo médio de duração total do procedimento dos pacientes doadores do grupo sevoflurano foi de 145,0 ± 30,62 minutos (intervalo 120-145 minutos). Em dois receptores do grupo sevoflurano foram identificadas leitura de halogenado após a reperfusão do órgão transplantado (valor de 0,1%). Conclusões: no presente estudo, foi possível mostrar que, possivelmente, o órgão sólido transplantado armazenou e transportou anestésico inalatório halogenado que foi liberado na corrente sanguínea do receptor após sua reperfusão. Esse transporte pode ser influenciado por fatores como massa corpórea e tipo e duração da exposição ao halogenado. Esses fatos devem ser levados em consideração em pacientes considerados suscetíveis que serão submetidos a transplante renal de doador vivo, de forma a não empregar agentes desencadeantes.

- ItemAneurismas do segmento intracavernoso da artéria carotida interna(Universidade Federal de São Paulo (UNIFESP), 1997) Frudit, Michel Eli [UNIFESP]; Cavalheiro, Sergio [UNIFESP]Foram estudados 22 pacientes portadores de aneurismas do segmento intracavernoso da arteria carotida interna. Ocorreu predominancia do sexo feminino em relacao ao sexo masculino numa proporcao de 4,5:1. Os sintomas predominantes foram cefaleia (59,1%) e compressao de nervos cranianos adjacentes (VI, III, V, IV). O VI nervo foi o mais frequentemente acometido (68,2%), seguido do III nervo (45,5%). Estes achados apresentaram significancia pela analise estatistica. O diagnostico foi firmado pela TC ou RM de cranio e pela angiografia cerebral por subtracao digital. Todos os pacientes foram submetidos ao teste de oclusao da arteria carotida interna com balao. O tratamento realizado foi a oclusao proximal da arteria carotida interna utilizando-se baloes destacaveis. O impacto do tratamento evidenciou-se pela melhora da sintomatologia na grande maioria dos pacientes. Ocorreram complicacoes em 2 casos, sendo uma temporaria e a outra definitiva. Os pacientes foram submetidos a controles por RM de cranio a partir do 3° mes pos-embolizacao que demonstrou trombose dos aneurismas em todos os casos e diminuicao das dimensoes dos aneurismas na maioria dos pacientes. A revisao da literatura e os achados do autor permitiram concluir que a oclusao proximal da arteria carotida interna foi eficaz no tratamento dos aneurismas intracavernosos.
- ItemAplicação de terapias celulares em um modelo experimental de esclerose lateral amiotrófica(Universidade Federal de São Paulo (UNIFESP), 2011-11-24) Barnabé, Gabriela Filoso [UNIFESP]; Mello, Luiz Eugenio Araujo de Moraes [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease characterized by progressive degeneration of upper and lower motor neurons, to which no efficient treatment is currently available able of increasing life span. Because ALS is a multifactorial disease, it is supposed that the most promising therapies may be the ones capable of playing multiple roles. Despite some aspects remained unknown, stem cells (SC) may be able to modify the lesion microenvironment, modulating multiple systems and, under this aspect, they show up as candidates for neurodegenerative disease therapies. In this work we used a model of transgenic rats that superexpress a mutaded form of human superoxide dismutase I (SOD1) as a tool to study ALS. In the first step of this work we revealed some of the cellular and molecular events that took place during the neuronal degeneration process in ALS rat model. We noticed that motor neuron death was accompanied by gliosis, increased expression of inflammatory cytokines and some growth factors, besides the mobilization of endogenous neural precursor cells. Accordingly to the data acquired in the first step, two disease stages were chosen to study cell therapy in SOD1 rats: the pre-symptomatic stage and the initial symptomatic stage. The second step experimental design has been organized accordingly to two distinct questions: I) Can SC therapies retard disease progression? II) How SC behave to neurodegenerative environment in different stages? The effects of various types of SC were compared: neural, mesenchymal and the combination of both. SC transplantation did not promoted any motor function improvement, possibly due to the great variability of ALS rat model and to the fact that SC did not went far from the transplantation site. Each type of SC used here resulted in distinct effects over ALS degenerative microenvironment, highlighting gliosis modulation, mobilization of endogenous neural precursors and differential expression of chemokines and cytokines in the spinal cord. These effects depends on disease stage when cell transplantation happened, illustrating a complex interaction between donor and host cells. Finally, neurospheres derived from SOD1 transgenic fetal spinal cord, that consists in an ALS in vitro study model, indicated that some alterations promoted by mutated SOD1 can be early noticed in the development. A main contribution of this work is in the conception that the development of cell therapies for ALS must consider a large comprehension of the degenerative mechanisms and the biological properties of the several sources of SC.

However how early physical activity promotes such effects are not well understood. A possible explanation is that physical exercise can stimulate neuronal growth, resulting in a neural reserve which could be extracted throughout the life course. Based in this hypothesis, the present study was designed to investigate the neural reserve induced by early physical activity. To do this, we evaluated the cortical and hippocampal levels of BDNF, ACTH and corticosterone during the aging course of rats submitted to physical exercise during adolescent period. Methods: Thirtyfive male Wistar rats were distributed into exercise (EX, n=18) and control (CTL, n=17) groups. Rats from EX group were submitted to an aerobic exercise program during the 21st and 60th postnatal day (P21P60). Afterward, cortical and hippocampal levels of BDNF, ACTH and corticosterone from the EX and CTL groups were investigated at different life stages: 0 (P60), 30 (P90), 60 (P120) and 90 (P150) days after last exercise session. The BDNF, ACTH and corticosterone levels were quantified by mean of Luminex xMAP system (MAGPIX technology). Results: No change was detected in the cortical and hippocampal levels of ACTH and corticosterone (p > 0.05). After the last physical training session (at P60), it was noted a significant increase of hippocampal BDNF (p < 0.05). However, this effect induced by early exercise was not maintained throughout the sedentary life of rats (ie, BDNF levels returned to control levels at P90 and P120). Conclusion: Our results indicate that changes in hippocampal BDNF levels may occur in rats exercised in youth, especially at P60. However, no persistent and longlasting effect of early physical exercise, which could be related to the neural reserve, was found in this study.

Method: Thirty-nine (39) underwent evaluation of the pre-sleep questionnaire and polysomnography, thirty-three (33) underwent actigraphy and data from these examinations were correlated with the scales of the Chalder fatigue, answered by thirty-eight (38) patients and Piper, answered by thirty (30) patients in three periods of the day (sent by mail). Results: This study allowed us to identify the quality of sleep is affected in two populations, both PS and PPS, with increased severity of the situation faced by the PPS group. Fatigue in fact is related to the time of the day, appearing in the population of circadian PPS. Conclusion: The circadian fatigue character takes on well-defined population of PPS, according to the assessment of the scale of Piper. The fatigue is correlated with sleep parameters, especially considering the variables of apnea and hypopnea index, movement of legs and sleep architecture. The intensity of fatigue is greater in patients with PPS. The Piper fatigue scale proved to be very sensitive and can measure the circadian fatigue in PPS. The intensity of fatigue is strongly correlated with the degree of muscular weakness.
- ItemAvaliação da contribuição do estudo de perfusão por ressonância magnética para o diagnóstico pré-operatório de anaplasia em tumores supratentoriais com aspecto sugestivo de glioma sem impregnação pelo agente paramagnético na ressonância magnética convencional(Universidade Federal de São Paulo (UNIFESP), 2005) Maia Junior, Antonio Carlos Martins [UNIFESP]; Malheiros, Suzana Maria Fleury [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Objetivo: O diagnóstico de glioma de baixo grau não pode ser baseado exclusivamente em imagens convencionais de RM e, particularmente nos tumores localizados em áreas eloqüentes, a adequada seleção de alvos para biópsias estereotáxicas é crucial, devido ao alto risco de erros de amostragem. Nosso objetivo é utilizar as seqüências de perfusão por RM, com a finalidade de avaliar a microcirculação dos tumores supratentoriais que não apresentam impregnação pelo agente paramagnético no exame de RM convencional. Método: Entre fevereiro de 2001 e fevereiro de 2004 foram incluídos em nosso estudo todos os pacientes adultos com lesão expansiva supratentorial sugestiva de glioma de baixo grau, sem impregnação pelo gadolínio na RM convencional, avaliados no setor de neurooncologia da Disciplina de Neurologia da UNIFESP. Foram realizados estudos pré- operatórios de perfusão por RM da primeira passagem do gadolínio pela rede capilar cerebral utilizando seqüência spin-eco eco-planar. O volume sangüíneo cerebral relativo (rCBV) das lesões foi expresso em relação à substância branca contralateral de aparência normal. Nos pacientes que apresentavam lesões com mapas de perfusão heterogêneos foram realizadas biópsias estereotáxicas dos focos de maior rCBV antes da ressecção cirúrgica da lesão. Resultados: Dos 21 pacientes incluídos (16 homens, média etária de 36 anos; variação de 23 a 60 anos) 10 apresentaram diagnóstico de astrocitoma difuso (grau II da OMS) e 11 foram agrupados como outros gliomas de baixo grau (oligoastrocitomas e oligodendrogliomas) ou gliomas anaplásicos. Nos pacientes com mapas de rCBV heterogêneos os focos de alta perfusão corresponderam a áreas de oligodendroglioma ou astrocitoma anaplásico na biópsia estereotáxica, enquanto a histologia predominante do restante do tumor era de astrocitoma de baixo grau. Os valores de rCBV dos astrocitomas difusos foram significativamente menores do que aqueles observados nos demais grupos (p <0,01). O valor de corte de rCBV que permitiu melhor discriminação entre os astrocitomas difusos e os demais foi de 1,2 (80% sensibilidade e 100% especificidade). Conclusão: A seqüência de perfusão por RM é um método útil, que pode contribuir para minimizar os erros de amostragem nos procedimentos de biópsia estereotáxica em gliomas que não apresentam impregnação pelo gadolínio e pode ser utilizada como critério adicional aos critérios morfológicos anteriormente descritos para a graduação pré-operatória dos tumores gliais.
