Aspectos epidemiológicos, clínicos, radiológicos e características renotípicas do Mycobacterium tuberculosis associados a fatores de risco para cura da tuberculose multidrogarresistente

Resumo
Objective: To identify risk factors among demographic, clinical and radiological variables associated with multidrug-resistant tuberculosis´s cure, abandonment, failure and death after the first treatment, describe the variability of the sensitivity pattern of the multidrug-resistant bacillary population and associate bacillary hetero-resistance over time by the drug sensitivity test mean with the events of cure, failure and death. Method: Retrospective cohort study from 2006 to 2010 at a health unit in São Paulo,Brazil. The data were obtained from medical records. Phenotypic sensibility tests were performed in the automated system BACTECTMMGITTM SIRE kit (SAT/SIRE) and methodology for determining minimum inhibitory concentration for second line drugs. Phenotypic heterogeneity was defined when sputum samples from the same patient ranged between sensitive or resistant compared to the binomial Rifampicin / Isoniazid during treatment of disease. Results: A total of 156 patients and their 460 sputum samples with in vitro bacilli resistant to rifampicin and isoniazid were isolated by means of a phenotypic sensitivity test and eligible for the study. In this cohort of patients with MDR-TB, mean age was 39.5 years (SD ± 12.5), the majority was male (60%). Of the 85 patients cured after the first treatment, 89.4% had conversion of the culture between the first and the sixth month of treatment. It was possible to classify the bacillary resistance in 144 patients, where 58% were considered as of acquired resistance and 42% were classified as of primary resistance. When measured by chest X-ray, 78% had bilateral parenchymal lesion and 39% bilateral pulmonary cavitation. A logistic regression, showed that patients presenting with a greater initial weight (RC = 1.05),without comorbidities (RC = 2.47) and with primary multidrug-resistant tuberculosis (RC = 2.67) were associated with cure at the end of first treatment. Cure occurred in 54% of patients after the first treatment and in 58% at the final outcome. At the final event, 80% of patients remained classified as multidrug-resistant tuberculosis and with a higher cure rate than those who migrated to pre extensively drug-resistant and extensively drug-resistant tuberculosis. There was a high rate of bacillary resistance to the anti-tuberculosis drugs used. The presence of phenotypic hetero-resistance to the binomial rifampicin / isoniazid was associated with negative events, failure and death.(P = 0.037). There were six cases (7%) of relapse. Conclusion: High initial weight, no comorbidity and with primary multidrug-resistant tuberculosis were considered as predictive factors associated with multidrug-resistant tuberculosis cure in the first treatment. In the evolution of the resistance pattern over time, it was observed that patients who remained multidrug-resistant tuberculosis had a higher proportion of cure and a lower death rate. The presence of phenotypic hetero-resistance presented by the binomial rifampicin/isoniazid in relation to all samples from each patient was associated with negative events (failure and death)
Objetivo: Identificar fatores preditivos entre variáveis demográficas, clínicas e radiológicas associados aos eventos, cura, abandono, falência e óbito na tuberculose multidrogarresistente após o primeiro tratamento; descrever a variabilidade do padrão de sensibilidade da população bacilífera multidrogarresistente e associar a heterorresistência bacilar ao longo do tempo com os eventos cura, falência e óbito, no desfecho final. Método: Estudo observacional do tipo coorte retrospectivo de 2006 a 2010 em uma unidade de saúde em São Paulo, Brasil. Os dados foram obtidos pormeio de revisão de prontuários médicos. Os testes de sensibilidade fenotípicos foram realizados no sistema automatizado BACTECTMMGITTM SIRE kit (SAT/SIRE) e a metodologia de determinação da concentração inibitória mínima, para os fármacos de segunda linha. Heterorresistência fenotípica foi definida quando as amostras de escarro do mesmo doente variaram entre sensível ou resistente, em relação ao binômio Rifampicina/Isoniazida, durante o tratamento da doença multidrogarresistente. Resultados: Foram isoladas por meio de teste de sensibilidade fenotípico e elegíveis para o estudo 156 doentes e suas 460 amostras de escarro com presença de bacilos resistentes in vitro à rifampicina e isoniazida. Nessa coorte a média de idade foi de 39,5 anos (DP± 12,5), a maioria era de sexo masculino (60%). Dos 85 doentes que curaram após o primeiro tratamento, 89,4% deles apresentaram conversão da cultura entre o primeiro e o sexto mês de tratamento. Em 144 doentes foi possível classificar a resistência bacilar, onde 58% foram considerados como portador de resistência adquirida e 42% foram classificados como resistência primária.Avaliados pela radiografia de tórax, 78% deles apresentaram lesão parenquimatosa bilateral e 39% cavidade bilateral. Pela regressão logística os doentes que se apresentaram com maior peso inicial (RC= 1,05), sem comorbidade (RC= 2,47) e com tuberculose ultidrogarresistente primária (RC=2,67) foram associados com cura ao final do primeiro tratamento para TB-MDR. Após o primeiro tratamento de tuberculose multidrogarresistente o evento cura ocorreu em 54% dos doentes e no desfecho final 58%. No evento final 80% dos doentes permaneceram classificados como tuberculose multidrogarresistente e com maior taxa de cura em relação aos que migraram para tuberculose pré extensivamente resistente e tuberculose extensivamente resistente.Houve elevada taxa de resistência bacilar aos fármacos rotineiramente utilizados. A presença de heterorresistência fenotípica ao binômio rifampicina/isoniazida associou-se aos eventos negativos, falência e óbito. (p=0,037). Houve seis (7%) casos de recidiva. Conclusão: Maior peso inicial, sem comorbidade e com tuberculose multidrogarresistente primária foram considerados como fatores preditivos associados à cura da tuberculose multidrogarresistente no primeiro tratamento. Na evolução do padrão de resistência ao longo do tempo, observou-se que os doentes que permaneceram tuberculose multidrogarresistente tiveram maior proporção de cura e menor proporção óbito. A presença de heterorresistência fenotípica apresentada pelo binômio rifampicina/isoniazida em relação a todas as amostras de cada paciente se associou aos eventos negativos (falência e óbito)
