Programa Personalizado de Atividades (TAP-BR): proposição de uma versão ambulatorial e avaliação dos seus impactos na demência

Introdução: O Programa Personalizado de Atividades (TAP-BR) é um programa de intervenção em Terapia Ocupacional, que traz resultados promissores na redução dos sintomas comportamentais e psicológicos na demência (SCPD). Objetivos: propor adaptações ao TAP-BR para uma versão ambulatorial, avaliar seu impacto e a sua aplicabilidade. Materiais e Métodos: O TAP-BR foi adaptado para uso e aplicação em uma perspectiva ambulatorial e foi feita a avaliação da aplicabilidade desta versão a partir da percepção dos terapeutas ocupacionais e dos cuidadores familiares. Por fim, foi avaliado o impacto do programa sobre os SCPD e sobre o estado emocional dos cuidadores familiares. Resultados: O TAP-BR versão ambulatorial sofreu as seguintes alterações: adequações no Manual de Intervenção e Pasta de Documentação; a avaliação ambiental foi feita a partir de entrevista com o cuidador familiar; as sessões diminuíram de 90 minutos para 60 minutos cada; o livro “Você não está sozinho... nós continuamos com você” não foi utilizado. A partir da visão dos terapeutas ocupacionais e dos cuidadores familiares o programa apresentou-se como sendo de fácil aplicação. Em relação as medidas de desfecho, houve melhora no desgaste do cuidador em relação aos SCPD (Cohen’s d= 0,23) e em seu estado emocional (Cohen’s d= 0,27), principalmente na variável depressão (Cohen’s d= 0,37). Conclusão: A partir dos resultados, podemos concluir que o TAP-BR versão ambulatorial é um Programa de fácil aplicação, com impacto sob os SCPD e melhora no estado emocional dos cuidadores familiares, com ênfase na variável depressão, com tamanho de efeito pequeno.
Introduction: The Tailored Activities Program (TAP-BR) is an intervention program in Occupational Therapy, which brings promising results in reducing behavioral and psychological symptoms in dementia (BPSD). Objectives: to propose adaptations to the TAP-BR for an outpatient care version and to evaluate its impact and its applicability. Materials and Methods: TAP-BR was adapted for use and application in an outpatient care perspective and the applicability of this version was assessed based on the perception of occupational therapists and family caregivers on it. Finally, the impact of the program on BPSD and on the emotional state of family caregivers was evaluated. Results: The TAP-BR outpatient care version has undergone the following changes: adjustments to the Intervention Manual and Documentation Folder; the environmental assessment was made based on an interview with the family caregiver; the sessions decreased from 90 minutes to 60 minutes each; the book “You are not alone... we are still with you” was not used. From the perspective of occupational therapists and family caregivers, the program was perceived as being easy to apply. Regarding the outcome measures, there was an improvement in the weariness of the caregiver in relation to BPSD (Cohen's d = 0.23) and their emotional state (Cohen's d = 0.27), especially in the depression variable (Cohen's d = 0.37). Conclusion: From the results, we can conclude that the TAP-BR outpatient care version is a program that is easy to apply, with an impact on the BPSD and overall improvement in the emotional state of family caregivers, with emphasis on the variable depression, with a small-sized effect.
