Resumo
Objetivo: Verificar a viabilidade da avaliação de linguagem em sujeitos adultos e idosos saudáveis via teleatendimento utilizando a Western Aphasia Battery-Revised (WAB-R), por meio da comparação da avaliação presencial e remota. Metodologia: Estudo descritivo, observacional, prospectivo, transversal, aprovado pelo Comitê de Ética da instituição (parecer nº 0695/2023). A amostra foi composta por 59 indivíduos com 40 anos ou mais, sem queixas/alterações cognitivas, com escolaridade acima de 2 anos. Os sujeitos foram triados pelo Mini-Exame do Estado Mental, Escala de Depressão Geriátrica e Questionário de Atividades Funcionais, obtendo escores dentro dos parâmetros de normalidade para a população brasileira. Os indivíduos foram submetidos à avaliação de linguagem por meio da Western Aphasia Battery-Revised (WAB-R), em versão adaptada para a população brasileira (Neves et al., 2014), que examina as habilidades de fala espontânea, compreensão verbal auditiva, repetição, nomeação e acesso lexical, leitura e escrita, além de praxias e habilidades construtivas, visuoespaciais e cálculo. Para analisar o grau de satisfação dos participantes, aplicou-se um questionário adaptado de Theodoros e colaboradores (2008), pontuado por meio de escala Likert, com perguntas relacionadas ao teleatendimento - satisfação geral, qualidade do áudio/vídeo, facilidade do seguimento das instruções e comparação com a avaliação presencial. Os sujeitos foram avaliados de forma presencial e por teleatendimento por meio da plataforma Google Meet, com intervalo de 15 a 30 dias entre as avaliações, em ordem randomizada. O desempenho dos grupos foi comparado por estatística descritiva e inferencial. Resultados: Foram avaliados 59 sujeitos, com idade entre 40-85 anos (M=59,3 e DP=10,0) e escolaridade entre 2-22 anos (M=11,44 e DP=4,66) com escores normais nos testes de rastreio cognitivo. Quanto ao desempenho no WAB-R, não foram encontradas diferenças estatisticamente significativas nas pontuações obtidas em ambiente presencial versus online em nenhuma das tarefas e componentes linguísticos. Em relação ao grau de satisfação com o teleatendimento, 86,5% da amostra se sentiu muito satisfeita. Em relação à qualidade do áudio, 71,2% atribuiu nota excelente e quanto à qualidade do vídeo, 69,5% atribuiu nota excelente. Sobre a dificuldade em seguir as instruções, 71,2% considerou muito fácil. A grande maioria dos participantes, 98,3%, afirmou que participaria novamente de uma avaliação por teleatendimento e 64,4% considerou a sessão por teleatendimento mais conveniente do que a outra modalidade, mesmo assim 59,3% relatou que estaria mais satisfeito com uma avaliação presencial. Conclusão: A ausência de diferenças de desempenho na avaliação presencial e online demonstra equivalência entre as duas formas de avaliação, sinalizando a viabilidade da avaliação de linguagem por teleatendimento e alto grau de satisfação dos participantes.
Objective: To verify the feasibility of language assessment in healthy adults and elderly subjects via telecare using the Western Aphasia Battery-Revised (WAB-R), by comparing in-person and remote assessment. Methodology: Descriptive, observational, prospective, cross-sectional study, approved by the institution's Ethics Committee (opinion no. 0695/2023). The sample consisted of 59 individuals aged 40 years or older, without complaints/cognitive alterations, with more than 4 years of education. The subjects were screened by the Mini-Mental State Examination, Geriatric Depression Scale and Functional Activities Questionnaire, obtaining scores within the normal parameters for the Brazilian population. The individuals underwent language assessment using the Western Aphasia Battery-Revised (WAB-R), in a version adapted for the Brazilian population (Neves et al., 2014), which examines spontaneous speech skills, auditory verbal comprehension, repetition, naming and lexical access, reading and writing, as well as praxis and constructive, visuospatial and calculation skills. To analyze the degree of satisfaction of the participants, a questionnaire scored using a Likert scale was applied, developed by the researchers, with questions related to the telecare - general satisfaction, audio/video quality, ease of following instructions and comparison with the in-person assessment. The subjects were assessed in person and by telecare through the Google Meet platform, with an interval of at least 15 days between assessments, in a randomized order. Results: The performance of the groups was compared using descriptive and inferential statistics. To date, 40 subjects have been evaluated, aged between 40-85 years (M=60.8 and SD=9.9) and with 2-22 years of schooling (M=11.44 and SD=4.66) with normal scores in the cognitive screening tests. Regarding the performance in the WAB-R, no statistically significant differences were found in the scores obtained in face-to-face versus online environments in any of the linguistic tasks and components. Regarding the level of satisfaction with the telehealth service, 86.5% of the sample felt very satisfied. Regarding the audio quality, 71.2% gave it an excellent rating, and regarding the video quality, 69.5% gave it an excellent rating. Regarding the difficulty in following the instructions, 71.2% considered it very easy. The vast majority of participants, 98.3%, said they would participate in a telehealth assessment again, and 64.4% considered the telehealth session more convenient than the other modality. Even so, 59.3% reported that they would be more satisfied with an in-person assessment. Conclusion: The absence of differences in performance in the face-to-face and online assessment demonstrates equivalence between the two forms of assessment, signaling the feasibility of language assessment by telemedicine and a high level of satisfaction between the participants.
