Introduction: The impacts of institutional racism on the health-disease-care process are evidenced in several epidemiological studies that demonstrate the vulnerabilities and inequities in health of the Brazilian black population. It is necessary to study its mechanisms and effects in order to describe it as a problem to be solved in the context of racial justice. Objective: To understand the perceptions and actions of health professionals in relation to institutional racism in the health of black university students.Method: Exploratory, descriptive study with a qualitative approach to institutional racism in the university environment. The population was composed of health professionals from the Student Support Centers and the Student Health Service of a public university in the state of São Paulo. Results: Seven interviews were conducted using a semi-structured questionnaire. Content analysis was used and three categories emerged: 1) Presence and absence of Critical Racial Literacy; 2) Racialized versus universal care practice; and 3) Institutional Racism in university life. The people interviewed presented diverse perceptions of racial awareness and literacy that impact the way they carry out care practices and their understanding of institutional racism in the lives of black students. Final considerations: Institutional racism can be tackled to the extent that people in institutions have the ability to recognize it. The absence of racial literacy can reproduce the racism in force in society.
