Fome hedônica, depressão e terapia cognitivo-comportamental: estudo clínico randomizado para adultos com obesidade

No Brasil, mais de 50% por cento dos adultos estão com excesso de massa corporal e 20% estão com obesidade. Existem fortes evidências do ambiente obesogênico na contribuição do aumento desta pandemia. O estilo de vida moderno colabora para o sedentarismo, depressão e compulsão alimentar, que estão amplamente associados a baixa qualidade de vida. A terapia interdisciplinar tem sido considerada um tratamento eficaz para obesidade, e os aspectos cognitivos, estão ganhando cada vez mais atenção neste contexto. Objetivos: 1-). Analisar e comparar os efeitos dos programas: Educação e Saúde (ES); Exercício Físico (EF); Terapia Interdisciplinar com Terapia cognitivo-comportamental (TI+TCC), de longo prazo para adultos com obesidade; 2-). Verificar associação entre frequência alimentar e compulsão alimentar nos programas: Educação e Saúde (ES); Exercício Físico (EF); Terapia Interdisciplinar com Terapia cognitivo-comportamental (TI+TCC), de longo prazo para adultos com obesidade. Métodos: 98 voluntários foram randomizados nos programas: ES (n =33) e receberam palestras sobre temas de saúde; EF (n = 34), participaram de exercícios físicos; TI+TCC (n=31), participaram de exercício físico, orientação nutricional, terapia psicológica e de fisioterapêutica. Resultados: Objetivo 1: foi observado que a qualidade de vida total aumentou significativamente no ES. Embora o EF tenha diminuído a massa corporal, apenas IT + TCC melhorou o perfil antropométrico; e a qualidade de vida; e diminuiu o comportamento alimentar e os sintomas de depressão em adultos com obesidade. Para o objetivo 2. Verificamos associação positiva entre QFAP com BES; e QFAU com BES. Conclusão: Os três programas trouxeram benefícios psicológicos para adultos com obesidade, melhorando de forma qualitativa a ingestão alimentar e diminuindo os sintomas de compulsão alimentar.
In Brazil, more than 50% per cent of adults are overweight and 20% are obese. There is strong evidence from the obesogenic environment in the contribution to the increase of this pandemic. The modern lifestyle contributes to sedentary lifestyle, depression and binge eating, which are widely associated with low quality of life. Interdisciplinary therapy has been considered an effective treatment for obesity, and cognitive aspects are gaining more and more attention in this context. Objectives: 1-). Analyze and compare the effects of the programs: Education and Health (ES); Physical Exercise (EF); Long-term Interdisciplinary Therapy with Cognitive-behavioral Therapy (IT + CBT) for adults with obesity; two-). Check the association between eating frequency and binge eating in the programs: Education and Health (ES); Physical Exercise (EF); Long-term Interdisciplinary Therapy with Cognitive-behavioral Therapy (IT + CBT) for adults with obesity. Methods: 98 volunteers were randomized into the programs: ES (n = 33) and received lectures on health topics; EF (n = 34), participated in physical exercises; TI + CBT (n = 31), participated in physical exercise, nutritional guidance, psychological and physical therapy. Results: Objective 1: it was observed that the total quality of life increased significantly in ES. Although EF has decreased body mass, only IT + CBT improved the anthropometric profile; and the quality of life; and decreased eating behavior and symptoms of depression in adults with obesity. For goal 2. We found a positive association between QFAP and BES; and QFAU with BES. Conclusion: The three programs brought psychological benefits to adults with obesity, qualitatively improving food intake and decreasing the symptoms of binge eating.
