Visitas domiciliares em saúde mental: uma perspectiva dos usuários, trabalhadores e familiares

Resumo
Este estudo investiga a articulação entre os CAPS-Leste (Centro de Atenção
Psicossocial) e a UBS-Promissão (Unidade Básica de Saúde do Bairro Promissão),
especialmente as Visitas Domiciliares, tanto a partir da equipe de profissionais da
Saúde Mental e da Atenção Básica que as realizam, quanto dos usuários e
familiares que são seus beneficiários, no Bairro Promissão da cidade de
Diadema/SP. A estratégia metodológica, cujo referencial se apoiou em elementos da
pesquisa participante, se concretizou principalmente por meio de observação
participante, em realização de acompanhamento sistemático a reuniões, Visitas
Domiciliares e grupos de orientação. A análise do trabalho de campo permite
apontar que, apesar de ser recente a proposta de articular os serviços de Saúde
Mental e Atenção Básica no território em Diadema/SP, as Visitas Domiciliares, as
ações no território e as reuniões entre equipes surgem como estratégias
interessantes na aproximação dos profissionais e na elaboração de estratégias que
se proponham atender os usuários numa perspectiva da integralidade e da
intersetorialidade. Todavia, a complexidade e os desafios que estão postos às
demandas da atenção ao usuário da saúde mental e seus familiares no território,
bem como à gestão e organização do trabalho, ao trabalho interprofissional das
equipes e interesses profissionais influenciam diretamente as intervenções e
oferecem impasses quando na perspectiva de concretizar princípios e diretrizes das
políticas públicas de saúde.
The present study investigates the interplay among an institution of mental health care (CAPS-Leste) and an institution of primary health care (UBS Promissão), focusing specially on the home visits under the point of view of the health professionals and also of the mental health care patients and their families in the city of Diadema, state of São Paulo, in an specific neighborhood called Promissão. The research method adopted to develop the study was based in the participatory research and mainly executed by the participant observation. Due to that, the researcher attended meetings, home visits and groups accomplished by the health professionals. The results show that, even been a recent project, this interplay is an interesting way of team work in order to provide new strategies of health care that improve professional relations and the possibility of considering the patients entirely in an interprofessional perspective. However, the complexity and the challenges brought by the health attention to patients with mental disorder and their families in the territory, as well as to the management of public health, to the organization of a joint project, to the interprofessional team work and professional interests, act upon directly in the results of the actions and produce difficulties in the perspective of performing the principles and the guidelines of the public health politics.
