Caracterização das células T reguladoras circulantes em gestantes com diabetes Mellitus tipo 1

Abstract

Objetivos: Caracterizar o perfil de células Treg em sangue periférico de gestantes com DM1, comparando os padrões observados nos três trimestres entre si, e com o identificado em mulheres com DM1 não gestantes (controle). Métodos: Trata-se de um estudo tipo caso-controle, que incluiu 36 mulheres DM1, sendo 16 gestantes e 20 não gestantes. Foram coletadas amostras de sangue periférico nos 3 trimestres da gestação e uma amostra do grupo controle. Por meio de citometria de fluxo, foram avaliadas as porcentagens de linfócitos T CD3+, CD4+, CD8+, Treg CD4+ (CD4+CD25+CD127-FOXP3+), Treg CD8+ (CD8+CD25+FOXP3+) e a expressão das citocinas TGFb e IFNg em cada um dos grupos celulares citados. Além disso foram analisadas a expressão de CD4+CD25high e CD4+CD25low. Resultados: Não foram observadas diferenças significativas quando confrontamos os dados obtidos nos trimestres gestacionais entre si. Entretanto, foram identificadas menores porcentagens de células TCD4+CD25high, TCD4+CD25low, Treg CD8 (CD8+CD25+FOXP3+), CD3+TGFb+, TCD4+TGFb+, TCD8+TGFb+, TCD4+IFNg+ e Treg CD8+TGFb+ (CD8+CD25+FOXP3+TGFb+) nos subgrupos de gestantes (independente do trimestre) comparados aos controles. Além disso, foi detectada maior porcentagem de linfócitos TCD8+ em gestantes do que nos controles. Conclusão: Este estudo sugere que gestantes com DM1, independente do trimestre, apresentem perfil de células Treg na circulação, mais supressor do que as não gestantes com a mesma patologia. Estes achados precisam ser confirmados incluindo maior número de casos e testes funcionais.

Objective: To characterize the expression of Treg cells in peripheral blood of pregnant women with DM1, comparing the patterns observed in the three trimesters of pregnancy, and with those detected in non-pregnant women with DM1 (control). Methods: This is a case-control study, which included 36 T1DM women, 16 pregnant and 20 non-pregnant. Peripheral blood samples were collected in the 3 trimesters of pregnancy and one sample from the control group. Using flow cytometry, the percentages of CD3+, CD4+, CD8+, CD4+ Treg (CD4+CD25+CD127-FOXP3+), CD8+ Treg (CD8+CD25+FOXP3+) and the expression of the cytokines TGFb and IFNg in each of the mentioned cell groups. In addition, the expression of CD4+CD25high and CD4+CD25low was analyzed. Results: No significant differences were observed when comparing the data from the gestational trimesters with each other. However, lower percentages of TCD4+CD25high, TCD4+CD25low, CD8 Treg cells (CD8+CD25+FOXP3+), CD3+TGFb+, TCD4+TGFb+, TCD8+TGFb+ , TCD4+IFNg+ and CD8+TGFb+ Treg (CD8+CD25+FOXP3+TGFb+) were identified in subgroups of pregnant women (regardless of trimester) compared to controls. Also, a higher percentage of TCD8+ lymphocytes was detected in pregnant women than in controls. Conclusion: This study suggests that pregnant women with T1DM, regardless of the trimester, have a more suppressive profile of circulating Treg cells than non-pregnant women with the same pathology. These findings need to be confirmed including a greater number of cases and functional tests.
