EFEITOS DA ESTIMULAÇÃO TRANSCRANIANA POR CORRENTE CONTÍNUA SOBRE OS SINTOMAS, HABILIDADES COGNITIVAS E COGNIÇÃO SOCIAL EM CRIANÇAS E ADOLESCENTES COM TRANSTORNO DO ESPECTRO AUTISTA: Ensaio clínico cruzado, controlado, randomizado e duplo cego

Transtorno do Espectro Autista (TEA) é uma condição do neurodesenvolvimento caracterizada por déficits na comunicação, interação social, padrões restritos e repetitivos de comportamentos e interesses. Apresenta alterações funcionais em diversas estruturas cerebrais causando prejuízos significativos na vida dos indivíduos. Portanto, faz-se necessário ampliar as pesquisas nas áreas de tratamento que se apresentam promissoras, tais como a Estimulação Transcraniana por Corrente Contínua (ETCC). Dessa forma, o presente estudo teve como objetivo comparar os efeitos neuromodulatórios da ETCC anódica aplicada sobre o córtex pré-frontal dorsolateral esquerdo (CPFDL) com efeitos da estimulação placebo nos sintomas, comportamento, funcionamento global, cognição social, linguagem e funções executivas de crianças e adolescentes com TEA. Trata-se de um ensaio clínico cruzado, controlado, randomizado e duplo cego, do qual participaram 23 crianças e adolescentes na faixa etária de 6 a 12 anos com diagnóstico de TEA. O protocolo de intervenção foi formado por dois períodos de tratamento compostos por dez sessões de ETCC anódica sobre o CPFDL esquerdo (ativa ou placebo) com intensidade de 1mA, por 20 minutos cada sessão, associadas a atividades de reconhecimento de emoções e resolução de situações problemas. Por se tratar de um estudo cruzado, foi respeitado um período de 3 meses entre as intervenções (washout). O estudo envolve avaliações pré-intervenção, pós-intervenção e um follow-up de 3 meses após cada etapa, a fim de verificar se os efeitos se mantêm a longo prazo. Para avaliação dos sintomas, foram utilizadas as escalas Childhood Autism Rating Scale (CARS), Autism Behavior Checklist (ABC) e Autism Treatment Evaluation Checklist (ATEC). Os problemas de comportamento foram avaliados com o Child Behavior Checklist (CBCL), a cognição social com o NEPSY-II, a linguagem oral com o Teste Infantil de Nomeação (TIN) e as funções executivas com o Five Digit Test (FDT). Para verificar se existiam diferenças entre os tempos de avaliação e entre os tipos de intervenção (ativo vs placebo) foram conduzidas análises intragrupo e intergrupo. O método utilizado foi o Generalized Equations Estimating (GEE) e o teste de Wilcoxon. Na ABC, a análise intergrupo demonstrou que, em comparação com o momento placebo, a ETCC ativa ocasionou redução estatisticamente significante dos problemas relacionados a estímulos sensoriais na avaliação de follow-up. Por outro lado, ao contrário do que era esperado, na ATEC, a comparação intergrupos demonstrou diferença significante no domínio “sociabilidade” na avaliação de follow-up com menores médias para o grupo placebo. Tanto na ABC quanto na ATEC, notaram-se reduções significantes em diversos subdomínios nos dois grupos, indicando efeito de tempo entre as avaliações. Ainda, o teste de Wilcoxon evidenciou redução significante na gravidade dos sintomas em doze dos quinze domínios mensurados pela CARS, assim como melhora no funcionamento global após todas as etapas de estimulação. Nas demais medidas avaliadas, encontrou-se apenas efeito do tempo, mas não do tipo de estimulação. Até onde se sabe, esse foi o primeiro ensaio clínico que investigou os efeitos da ETCC sobre habilidades cognitivas e sociais na população pediátrica com TEA, sendo assim, são necessários mais estudos para avaliar a eficácia da técnica nesse público.
Autistic Spectrum Disorder (ASD) is a condition of neurodevelopment characterized by deficits in communication and social interaction, as well as restrictive and repetitive patterns of behaviors and interests. It presents functional alterations in several brain structures, causing significant damage to individuals' lives. Therefore, it is necessary to expand research in treatment areas that are promising, such as Transcranial Direct Current Stimulation (TDCS). Thus, the present work aims to compare the neuromodulatory effects of anodic (TDCS) applied to the left dorsolateral prefrontal cortex (DLPFC) with the effects of placebo stimulation on symptoms, behavior, global functioning, social cognition, language and executive functions in children and adolescents with ASD. This is a crossover, controlled, randomized, double-blind clinical trial, in which 23 children and adolescents aged 6 to 12 years with a diagnosis of ASD participated. The intervention protocol consisted of two periods of treatment consisting of ten sessions of anodic tDCS on the left DLPFC (active or placebo) with 1mA, for 20 minutes each session, associated with activities of emotional recognition and problem-solving situations. As this is a crossover study, a 3-month period between interventions (washout) was respected. The study involves pre-intervention and post-intervention assessments and a 3-month follow-up after each step to see if the effects are sustained in the long term. To assess symptoms, the Childhood Autism Rating Scale (CARS), Autism Behavior Checklist (ABC) and Autism Treatment Evaluation Checklist (ATEC) were used. Behavior problems were assessed with the Child Behavior Checklist (CBCL), social cognition with NEPSY-II, oral language with the Child Naming Test (CNT) and executive functions with the Five Digit Test (FDT). To verify if there were differences between the evaluation times and between the types of intervention (active vs placebo) intragroup and intergroup analyzes were conducted. The used method was the Generalized Equations Estimating (GEE) and the Wilcoxon test. At ABC, the intergroup analysis showed that, compared to the placebo moment, active tDCS caused a statistically significant reduction in problems related to sensory stimuli in the follow-up assessment. On the other hand, contrary to what was expected, in the ATEC the intergroup comparison showed a significant difference in the “sociability” domain in the follow-up assessment with lower means for the placebo group. Both ABC and ATEC showed significant reductions in several subdomains in both groups, indicating an effect of time between assessments. Furthermore, the Wilcoxon test showed a significant reduction in symptom severity in twelve of the fifteen domains measured by the CARS, as well as an improvement in global functioning after all stages of stimulation. In the other measures evaluated, only the effect of time was found, but not of the type of stimulation. As far as we know, this was the first clinical trial that investigated the effects of tDCS on cognitive and social skills in the pediatric population with ASD, therefore, further studies are needed to assess the effectiveness of the technique in this audience.
