Abstract Introduction: Knee osteoarthritis (OA) is a very prevalent joint disease and common cause of pain, functional loss and disability in adults. However, there are a few therapeutic options available for the management of this disease. Intra-articular injections (IIA) with corticosteroids (CE) are known to be effective in the short term providing symptomatic relief of the pain. Nevertheless, patients with some comorbidities may have a contraindication to this procedure. Botulinum toxin type A (TBA) has local analgesic effect, and has been used sporadically by the intra-articular route (IA) for this purpose. However, only a few well designed scientific studies, support IA use of TBA for the treatment of knee OA. Purpose: To compare the effectiveness of intra-articular injection (IIA) of TBA versus triacinonone hexacetonide (HT) in improving joint pain and goniometry, function, quality of life and sonographic measurement of synovial hypertrophy in the short (4 weeks) and medium (12 weeks) term in patients with symptomatic primary knee OA. Methods: A prospective randomized double-blinded controlled trial was conducted in 105 patients with symptomatic primary knee OA. Patients with a diagnosis of knee OA according to the American College of Rheumatology (ACR) criteria of both genders, with a visual analogue scale (VAS 0-10 cm) of knee pain between 3 and 8 (cm) were included. The patients were randomized into three groups of 35 patients subjected to IIA: 1) TBA, 2) HT and 3) Saline solution (SS) - 0.9% isotonic saline solution (control group). All patients received a IIA in the most symptomatic knee at a single moment. Clinical and ultrasound asssessments were performed by blinded evaluators using the following assessment instruments: pain at rest (VASr) versus pain at movement (VASr), functional questionnaire WOMAC and their domains, 6-minute walk tests and Time To Go Test (TUG), SF-36 generic quality of life questionnaire and its domains, perception of improvement (percentage -%) and longitudinal and transverse quantitative sonographic measurement (in millimeters) of synovial hypertrophy in the largest synovial knee pocket. Four evaluation periods were used: at the beginning of the study (T0) and at 4 (T0), 8 (T8) and 12 (T12) weeks after the IIA. Results: 105 patients were evaluated, 96 (91.4%) were women and 9 men (8.6%), with a mean age of 64 (6.9) years, and a disease time of 6.3 (7.0) years. In the baseline period, the groups were homogeneous for all parameters evaluated in the study. The analysis between groups at T12, using the ANOVA test for repeated measures, showed a statistically significant difference between the groups only for the VASm (p = 0.024) in favor of the HT group, despite a strong statistical tendency of superiority also observed in the HT group for WOMAC-pain (p = 0.051) and WOMAC-total (p = 0.052). In addition, a subgroup analysis at the short term (T0-T4), showed statistically significant improvement for the VASm (p = 0.002), WOMAC-pain (p = 0.003), WOMAC-stifness (P = 0.015), WOMAC-total (p = 0.007), and quantitative sonographic measurement of synovial hypertrophy (p = 0.049) in favor of the HT group. Conclusion: IIA with HT was more effective than TBA in the improvement of joint pain, WOMAC functional questionnaire and ultrasound measurement of synovial hypertrophy in patients with knee OA at 4 weeks (short term), while at 12 weeks (medium term), the superiority of IIA with HT, against IIA with TBA and IIA with SS, was only observed for the pain at the knee movement analysis.
Introdução: A osteoartrite (OA) de joelho é uma doença articular muito prevalente e causa comum de dor, perda funcional e incapacidade em adultos. No entanto, são poucas as opções terapêuticas disponíveis para o manejo dessa enfermidade. As injeções intra-articulares (IIA) com corticosteroide (CE) são sabidamente efetivas em curto prazo para o alívio sintomático dessa enfermidade. No entanto, pacientes com algumas comorbidades, podem ter contraindicação a esse procedimento. A toxina botulínica tipo A (TBA) tem efeito analgésico local, já tendo sido utilizada esporadicamente pela via intra-articular (IA) com essa finalidade. Existem poucos trabalhos bem desenhados, no entanto, que suportem o uso IA da TBA para o tratamento da OA de joelho. Objetivo: Comparar a efetividade da injeção intra-articular (IIA) da TBA versus a IIA de hexacetonide de triancinolona (HT) na melhora da dor e goniometria articulares, função, qualidade de vida e medida ultrassonográfica de hipertrofia sinovial em curto (4 semanas) e médio (12 semanas) prazo em pacientes com OA primária sintomática de joelho. Métodos: Foi realizado um estudo controlado randomizado prospectivo duplo-cego com intenção de tratar em 105 pacientes com OA primária sintomática de joelho. Foram incluídos pacientes com diagnóstico de OA de joelho segundo critérios do American College of Rheumatology - ACR, de ambos os gêneros, com escala visual analógica (EVA 0-10 cm) de dor no joelho entre 3 e 8 centímetros (cm). Os pacientes foram randomizados em três grupos de IIA, cada um com 35 pacientes: grupo TBA, grupo HT e grupo solução salina (SS- solução salina isotônica a 0,9%) (grupo controle). Todos os grupos receberam uma IIA no joelho mais sintomático em um único momento. As avaliações foram realizadas por avaliadores, clínico e ultrassonografista, “cegos”, através dos seguintes instrumentos de avaliação: EVA de dor ao repouso (EVAr) e ao movimento (EVAm), questionário funcional WOMAC e seus domínios, testes de caminhada de 6 minutos e teste Time Up and Go (TUG), questionário genérico de qualidade de vida SF-36 e seus domínios, percepção de melhora (porcentagem -%) e medida ultrassonográfica quantitativa (em milímetros) longitudinal e transversal de hipertrofia sinovial no maior bolsão sinovial supra-patelar do joelho. Utilizaram-se quatro tempos de avaliação: no início do estudo (T0) e T4, T8 e T12 semanas após a IIA. Resultados: Foram estudados 105 pacientes, 96 (91,4%) mulheres e 9 homens (8,6%), com média de idade de 64,2 (6,9) anos e de tempo de doença de 6,3 (7,0) anos. Na avaliação inicial, os grupos foram homogêneos para todos os parâmetros. A análise entre os grupos ao T12, usando o teste ANOVA para medidas repetidas, mostrou diferença estatisticamente significante entre os grupos apenas para a EVAm (p=0,024) a favor do grupo HT, apesar de uma forte tendência estatística de superioridade também para o grupo HT para o WOMAC-dor (p=0,051) e WOMAC-total (p=0,052). Em subanálise intergrupo de curto prazo (T0-T4), no entanto, observou-se melhora estatística da EVAm (p=0,002), WOMAC-dor (p=0,003), WOMAC-rigidez (0,036), WOMAC-função (p=0,015), WOMAC-total (p=0,007), e medida ultrassonográfica quantitativa de hipertrofia sinovial (p=0.049) a favor do grupo HT. Conclusão: A IIA com HT foi mais efetiva que a IIA de TBA em curto prazo (4 semanas) na melhora da dor articular, questionário funcional WOMAC e medida ultrassonográfica de hipertrofia sinovial em pacientes com OA de joelhos. Em médio prazo (12 semanas), a superioridade da IIA com HT em relação à IIA de TBA e à IIA de SS só se manteve para a dor articular ao movimento.
