Introduction: Violence against women is an important public health issue that occurs in various social classes, races/ethnic groups, religions, ages and educational backgrounds, being historically and socially neglected, mitigated and taken as natural, as one of the forms of gender control and power. Purpose: To identify the prevalence of intimate partner violence among puerperal women and verify the possible associations. Method: Cross-sectional survey with 207 women who attended the Centro de Incentivo e Apoio ao Aleitamento Materno/Banco de Leite da Universidade Federal de São Paulo (Center for Incentive and Support to Breastfeeding/Milk Bank of the São Paulo Federal University) for post-natal and breastfeeding appointment with nurses in 2011. The women bottlefeeding who declared themselves as illiterate and/or with cognitive deficit irrespective of their educational background presented hearing and/or visual disorders; disoriented in relation to time, space or people; with multiple pregnancies; and gave birth to children with abnormalities, especially orofacial fissures were excluded from the survey. To collect data, three instruments were used to cover the variables established. The first one was developed especially for the survey and identified the sociodemographic characteristics and life styles of the puerperal women and their partners, obstetric and neonatal characteristics, as well as those related to breastfeeding. The second one included the collection of data in relation to the mother-baby interaction by means of the Shermann & cols Mother-0 to 6 month-old baby Observation Protocol (POIMB 0-6) in an adapted manner. To identify intimate partner violence (IPV), a third instrument was chosen and it was based on the model proposed by Schraiber et al., adapted by Marinheiro and Rodrigues and by the researcher. The collection of data started in January 2011 and concluded in February, 2012. The interviews were carried out at three different moments and by different professionals, thereby ensuring the blind survey. The first appointment took place around 7 to 10 days after the birth and include the collection of data related to the characterization of the population, in addition to obstetric, neonatal and breastfeeding data. The second appointment was scheduled for 30 days after the first one and an invitation was made to take part of the research and collection of data in relation to the mother-baby interaction, and at the end of the appointment, a third one was appointed for 5 to 30 days later. During the third appointment, the collection of data relating to IPV took place. This survey observed the ethical principles that govern researches involving human beings, and the collection of data started after approval by the Comitê de Ética em Pesquisa da UNIFESP (CEP 1978/10) (UNIFESP’s Research Ethical Committee (CEP 1978/10)), signature of Authorization for Collection of Data by CIAAM/BLH’s coordinators and signature of the participants of the TCLE. Results: The research identified a prevalence of IPV at some stage of life of 60.4%. Of the women exposed to IPV, 94.4% mentioned psychological violence, 43.2% physical violence, and 8.8% sexual violence. In the period prior to the last pregnancy, 79.2% of the pregnant women referred to IPV, of them, 76.3% mentioned psychological IPV, 75.9% physical IPV, and 81.8% sexual IPV. During the last pregnancy, 43.2% of the pregnant women declared IPV, of this group, 44.9% declared psychological IPV, 25.9% physical IPV, and 18.2% sexual IPV. In the post-natal period, the percentage was 41.6%, of these women, 42.4% mentioned psychological IPV, 16.7% physical IPV, and 9.1% sexual IPV. Only 47.2% of the women acknowledge having been victims of violence at some point of their life. In the multivariate analysis, having higher education and having prior breastfeeding experience represented protection from violence, and not having a partner was a risk 2.8 times for this issue and the puerperal women breastfeeding in an inadequate position represented a risk 2.1 higher for IPV. In connection with the obstetric and neonatal variable, a multivariate model of risk factors could not be adjusted because none of the parameters selected for analysis remained significant in the presence of another variable. It happened the same way in relation to the analysis between mother-baby interaction and violence data because only one parameter represented a ≤ 0.20 significance, not being justified a new multivariate analysis. Conclusion: The results found allowed to deepen reflections in relation to IPV and breastfeeding. They show the need of early detection and follow up by health staff on women who undergo IPV situations, in addition to qualitative researches that deepen the IPV phenomenon and its relationship with breastfeeding.
Introdução: A violência contra a mulher é um importante problema de saúde pública, incidindo nas diversas classes sociais, raças/etnias, religiões, idades e graus de escolaridade, sendo histórica e socialmente negligenciada, mitigada e naturalizada, como uma das formas de controle e poder de gênero. Objetivo: Identificar a prevalência da violência por parceiro íntimo entre puérperas e verificar as possíveis associações. Métodos: Estudo transversal com 207 mulheres que compareceram ao Centro de Incentivo e Apoio ao Aleitamento Materno/ Banco de Leite da Universidade Federal de São Paulo para consulta de enfermagem de revisão pós-parto e aleitamento materno, em 2011. As mulheres que estavam em aleitamento artificial; que se declararam não alfabetizadas e/ou com déficits cognitivos, independente da escolaridade; apresentavam deficiências auditivas e/ou visuais, desorientadas quanto ao tempo, espaço ou pessoas; com gestação múltipla; e, tiveram filhos com anormalidades, especialmente, malformações orofaciais foram excluídas do estudo. Para a coleta dos dados, três instrumentos foram usados, a fim de contemplar as variáveis estabelecidas. O primeiro, foi desenvolvido especificamente para o estudo e identificou as características sociodemográficas e hábitos de vida das puérperas e seus companheiros, características obstétricas e neonatais e as relacionadas à amamentação. O segundo instrumento, contemplou a coleta de dados a respeito da interação mãe-bebê por meio do Protocolo de Observação Mãe-bebê 0 a 6 meses de Shermann & cols (POIMB 0-6) de forma adaptada. Para a identificação da violência por parceiro íntimo, optou-se por utilizar o terceiro instrumento, baseado no modelo proposto por Schraiber et al., adaptado por Marinheiro e Rodrigues e pela pesquisadora. A coleta de dados iniciou-se em janeiro de 2011 e foi concluída em fevereiro de 2012. As entrevistas foram realizadas em três momentos distintos e por profissionais diferentes, garantindo o cegamento do estudo. A primeira consulta ocorreu em torno de 7 a 10 dias após o parto e contemplou a coleta de dados referente à caracterização da população, além das características obstétricas, neonatais e da amamentação. A segunda consulta marcou-se 30 dias após a primeira, na qual houve o convite para a participação no estudo e a coleta dos dados referente à interação mãe-bebê e, ao final da consulta, agendou-se a terceira consulta para 15 a 30 dias posteriores. Na terceira consulta, ocorreu a coleta dos dados referentes à VPI. Esta pesquisa respeitou os princípios éticos que regem estudos envolvendo seres humanos, e a coleta dos dados iniciou-se após a aprovação pelo Comitê de Ética em Pesquisa da UNIFESP (CEP 1978/10), assinatura do Termo de Autorização para Coleta de Dados pela coordenação do CIAAM/BLH e assinatura das participantes do TCLE. Resultados: O estudo identificou uma prevalência de VPI em algum momento da vida de 60,4%. Das mulheres expostas à VPI, 94,4% mencionaram violência psicológica, 43,2% física e 8,8% sexual. No período anterior à última gestação, 79,2% das puérperas referiram VPI, destas, 76,3% citaram VPI psicológica, 75,9% VPI física e 81,8% VPI sexual. Durante a última gestação, 43,2% das puérperas declararam VPI, destas, 44,9% declararam VPI psicológica, 25,9% VPI física e 18,2% VPI sexual. No período pós-parto, o índice encontrado foi de 41,6%, destas mulheres, 42,4% citaram VPI psicológica, 16,7% VPI física e 9,1% VPI sexual. Apenas 47,2% das mulheres reconheceram ter sido vítimas de violência alguma vez na vida. Na análise multivariada, ter ensino superior e não ter experiência anterior em amamentação representaram proteção para a violência, e não ter companheiro foi um risco 2,8 vezes para esse problema e a puérpera amamentar em posição inadequada representou um risco 2,1 vezes maior para VPI. Quanto às variáveis obstétricas e neonatais, não foi possível ajustar um modelo multivariado de fatores de risco, visto que nenhum dos parâmetros selecionados para a análise permaneceu significante na presença de outra variável. Da mesma forma ocorreu para a análise entre os dados de interação mãe-bebê e violência, pois só um parâmetro apresentou significância ≤ 0,20, não se justificando a realização de análise multivariada. Conclusão: Os resultados encontrados permitiram aprofundar as reflexões quanto à VPI e amamentação. Mostraram a necessidade da detecção precoce e acompanhamento das mulheres que vivenciam situações de VPI pela equipe de saúde, além de estudos qualitativos que aprofundem o fenômeno da VPI e sua relação com a amamentação.
