Introduction Arterial calcification is a negative predictor of morbidity and mortality in end stage renal disease. Traditional factors like hypertension, dyslipidemia and non-traditional like mineral disturbs are involved in the calcification. Generally they are classified as promoters and inhibitors. Possible changes in inhibiting factors, such as fetuin A, are responsible for the pathophysiology of vascular calcification. Method Arterial calcification was evaluated by withdrawing fregments of the artery to be held in the first arteriovenous fistula for hemodialysis. Four histological staining was used to identify presence of calcification while the blood to the determination of fetuin was collected at the same time. Demographic, clinical and laboratory data were analyzed to test possible relations in the prevalence of vascular calcification. Results 34 patients were enrolled and in eight vascular calcification was detected. Predictors of calcification, after statistical analysis, were age and the presence of diabetes. The correlation between arterial calcification and fetuin 0.4 ( r = of 0.033). Conclusion There is an tendency of inverse correlation between arterial calcification and fetuin A levels in patients with chronic renal failure patients beginning hemodialysis.
Introdução A calcificação arterial é um preditor negativo de morbidade e mortalidade na Insuficiência renal crônica terminal tendo como principal causa na piora da sobrevida destes pacientes os fatores não tradicionais que são classificados em promotores e inibidores. Possíveis alterações nos fatores inibidores, como a fetuína A, são responsáveis pela fisiopatologia da calcificação vascular. Método A calcificação arterial foi avaliada pela retirada de fragmento da artéria do membro em que seria realizada a primeira fístula arteriovenosa para hemodiálise utilizando quatro colorações histológicas enquanto que o sangue para dosagem da fetuína A foi colhido no momento em que o paciente foi puncionado para que fosse realizada a fístula arteriovenosa. Dados sócio demográficos, clínicos e laboratoriais foram analisados para testar possíveis relações na prevalência da calcificação vascular. Resultados Foram incluídos 34 pacientes e em oito foi detectada a presença da calcificação vascular. Fatores preditores da calcificação, após análise estatística, foram a idade e a presença de diabetes. A correlação entre a calcificação arterial e a dosagem de fetuína A verificada pelo coeficiente de Pearson foi de 0,4 com valor de r de 0,033. Conclusão Existe uma correlação inversa de nível moderado entre a calcificação arterial e os níveis de fetuína A em pacientes com insuficiência renal crônica terminal em início da terapia hemodialítica
