Surgical success in 51 patients with primary hyperparathyroidism (PHPT)

A retrospective analysis of 51 patients with primary HPT who was underwent surgical procedure from January/83 to April/98 in the Head and Neck Service of University Federal of Sao Paulo was performed. The patients ranged in age from 18 to 73 (mean of 56 years); 39were women and 11 were men. Bone disease was present in 80%, nephrolithiasis in 38%, other symptom in 31% and 5.9% had no symptoms. The average preoperative serum calcium was 12.6mg/dl, ionized calcium was 1.57mmol/l; serum phosphorus was 2.3mg/dl; alkaline phosphatase was 568U/l and parathyroid hormone was 152pmol/l. A parathyroid adenoma was found in 72.6%, parathyroid hyperplasia in 19.6% and parathyroid carcinoma in 7.8%. We had 86.3% surgical success with the first surgery and 94.1% after a second cervical exploration with no permanent surgical complications.
