Revisão sistemática sobre efetividade das intervenções odontológicas utilizadas no tratamento de traumatismo de luxação em dentes permanentes anteriores

Contexto: O trauma dental é comum, ocorrendo principalmente em crianças e adolescentes. Um dos grupos destas injurias são classificadas como luxação dental. Este grupo inclui diferentes injurias, que envolvem trauma onde o dente é deslocado da sua posição original. Estes traumas de luxação são subdivididos em: intrusão, extrusão e luxação lateral de acordo com a direção do deslocamento. Várias intervenções tem sido utilizadas para o tratamento do traumatismo de luxação em dentes permanentes anteriores, mas essas intervenções não são suportadas por forte evidência científica. Objetivos: O objetivo desta revisão sistemática foi avaliar a efetividade das intervenções utilizadas para o tratamento do traumatismo de luxação em dentes permanentes anteriores. Estratégia de pesquisa: Estratégias de pesquisa foram desenvolvidas para a MEDLINE e elaboradas para as seguintes bases de dados: Cochrane Oral Health Group Trials Register, PubMed (1966-2006); EMBASE (1980-2006); Lilacs (1982-2006); BBO (Biblioteca Brasileira de Odontologia) (1986-2006);
SciELO (1997-2006). Critérios de Seleção: Todos os ensaios clínicos controlados randomizados ou quasi-randomizados de tratamentos de traumatismo de luxação que apresentem um acompanhamento de no mínimo 12 meses. Coleta de dados e análise: Dois revisores, independentemente, avaliaram a elegibilidade de todos os artigos identificados. Principais resultados: Quatorze estudos foram eleitos, e nenhum ensaio clínico controlado randomizado foi incluído. Obtivemos um grande número de publicações que envolvem relato de casos, série de casos, estudos de prevalência e revisões da literatura, estes trabalhos demonstram, na maioria das vezes, seqüência de tratamento realizada em pacientes traumatizados, utilizando as intervenções
existentes, descartando a hipótese da não efetividade das mesmas. Conclusões dos revisores: Não encontramos evidências suficientes para concluir sobre a efetividade das intervenções utilizadas para o
tratamento do traumatismo de luxação dental de dentes permanentes anteriores. Mais ensaios clínicos randomizados são necessários para elucidar as intervenções para tratamento de trauma dental.

This group encompasses a range of injuries where following the trauma, the tooth is displaced from its original position. These injuries are further subclassified into: intrusion, extrusion and lateral luxations according to the direction of displacement. Several interventions have been proposed to treating luxated permanent front teeth, but interventions are not supported by strong scientific evidence. Objectives: The aim of this systematic review was to evaluate the effectiveness of a range of interventions for treating luxated permanent front teeth. Search strategy: Search strategies were developed for MEDLINE and revised appropriately for the following databases: Cochrane Oral Health Group Trials Register; The Cochrane Central Register of Controlled Trials (CENTRAL): The Cochrane Library , current Issue; PubMed (1966 to 2006); EMBASE (1980 to 2006); Lilacs (1982 to 2006); BBO (Bibliografia Brasileira de Odontologia) (1986 to 2006); SciELO (1997 to 2006). Selection criteria: All randomised or quasi-randomised controlled trials of interventions for treating luxated permanent front teeth that include a minimum follow-up period of 12 months. Data collection & analysis: Two authors independently assessed the eligibility of all reports identified. Main results: Fourteen trials were chosen, and no randomised controlled trials were included. We have got a great number of publications including: cases report, series of cases, studies of prevalence and revisions of literature. These works demonstrate, in most cases, a sequence of treatment carried through traumatized patients, using the available interventions, discarding the hypothesis of non-effectiveness of them. Reviewers' conclusions: We did not find sufficient evidences to conclude about the effectiveness of a range of interventions for treating luxated permanent front teeth. More randomised controlled trials are needed to elucidate the interventions for treating dental trauma.
