A.; Universidade Federal de São Paulo (UNIFESP)106 infants which received artificial feeding and belonging to the lowest socio-economical levels of the city of Sorocaba were studied.The ages varied from 6 days to 12 weeks.52 males and 54 females, weighing from 800 to 4200 g.They were followed-up to the 20th week of life.All patients were visited by the pediatric staff in their respective home every week,receiving advice on Puericulture.Parasitological tests andcoprocultures of the feces were done at the beginning of the observation,every 4 weeks,and during diarrhoea outbreaks. Results: 55.6% of the infants had their nutritional state within normal limits;17.0% improved their nutritional state during the observation period;12.2%,which at the beginning of the study present with 1st.undernutrition,remained unchanged,and 10.3% with undernutrition of the 2nd and 3rd degrees remained unchanged during the study.2.8% of the cases with weight within normal level,evoluted into 1st degree undernutrition and 1.84% with first degree undernutrition finished with 2nd degree undernutrition.287 protoparasitological tests and 287 coprocultures were performed.Enteropathogenic bacteria were isolated in the feces of 23 patients without diarrhoea and in 7 patients with diarrhoea.In 10 patients with diarrhoea,no enteropathogenic bacteria were isolated.The results obtained stress the importance of continued “in loco” medical assistance.
- ItemSomente Metadadados3d shape tracking of minimally invasive medical instruments using optical frequency domain reflectometry(Soc Brasileira Pneumologia Tisiologia, 2016) Parent, Francois; Mandal, Koushik Kanti; Loranger, Sebastien; Watanabe Fernandes, Eric Hideki [UNIFESP]; Kashyap, Raman; Kadoury, SamuelWe propose here a new alternative to provide real-time device tracking during minimally invasive interventions using a truly-distributed strain sensor based on optical frequency domain reflectometry (OFDR) in optical fibers. The guidance of minimally invasive medical instruments such as needles or catheters (ex. by adding a piezoelectric coating) has been the focus of extensive research in the past decades. Real-time tracking of instruments in medical interventions facilitates image guidance and helps the user to reach a pre-localized target more precisely. Image-guided systems using ultrasound imaging and shape sensors based on fiber Bragg gratings (FBG)-embedded optical fibers can provide retroactive feedback to the user in order to reach the targeted areas with even more precision. However, ultrasound imaging with electro-magnetic tracking cannot be used in the magnetic resonance imaging (MRI) suite, while shape sensors based on FBG embedded in optical fibers provides discrete values of the instrument position, which requires approximations to be made to evaluate its global shape. This is why a truly-distributed strain sensor based on OFDR could enhance the tracking accuracy. In both cases, since the strain is proportional to the radius of curvature of the fiber, a strain sensor can provide the three-dimensional shape of medical instruments by simply inserting fibers inside the devices. To faithfully follow the shape of the needle in the tracking frame, 3 fibers glued in a specific geometry are used, providing 3 degrees of freedom along the fiber. Near real-time tracking of medical instruments is thus obtained offering clear advantages for clinical monitoring in remotely controlled catheter or needle guidance. We present results demonstrating the promising aspects of this approach as well the limitations of using the OFDR technique.

Several local treatments can be used, including topical 5-Fluorouracil (5-FU).ObjectivesTo determine the effectiveness and safety of 5-FU topical treatment for genital warts in nonimmunocompromised individuals.Search strategyDatabases searched were Cochrane Central Register of Controlled Trials (The Cochrane Library 2009 Issue 3), MEDLINE (1966 to August 2009), EMBASE (until August 2009), LILACS (1982 to August 2009). the search had no language or publication restrictions.Selection criteriaThe review included randomised controlled trials (RCTs) among women, men, or both sexes, aged 18 years and older, comparing: 5-FU versus placebo or no treatment; 5-FU in any dose versus other isolated treatment, topical or systemic; 5-FU in any dose associated with other treatment versus placebo; 5-FU in any dose associated with other treatment versus other isolated treatment, topical or systemic; 5-FU in any dose associated with other treatment versus other associated treatment, topical or systemic.Data collection and analysisTwo authors independently assessed trial quality and extracted data from the original publications.Main resultsSix trials involving 988 patients (645 women and 343 men) and reporting eight comparisons were found. Two studies reported withdrawals and dropouts, but none mentioned analysis by intention to treat (ITT). 5-FU presented better results for cure than placebo or no treatment (relative risk (RR) 0.39, 95% confidence interval (CI) 0.23 to 0.67), meta-cresol-sulfonic acid (MCSA) (RR 2.11, 95% CI 0.83 to 5.37), Podophylin 2%, 4% or 25% (RR 1.26, 95% CI 0.86 to 1.82). There were no statistical differences for treatment failure for 5-FU versus CO2 Laser (RR 0.69, 95% CI 0.43 to 1.11) versus 5-FU + INF alpha-2a (low dose) (RR 1.02, 95% CI 0.87 to 1.119). Worse results were found for 5-FU versus 5-FU + INF alpha-2a (high dose) (RR 10.78, 95% CI 1.50 to 77.36), and 5-FU + CO2 Laser INF alpha-2a (high dose) (RR 7.97, 95% CI 2.87 to 22.13).Authors' conclusionsThe reviewed trials were highly variable in methods and quality, and the evidence provided by these studies was weak. Cure rates with several treatments were variable, and although 5-FU presents therapeutic results that are inferior to those seen with 5-FU + Inf alpha-2a (high dose) and 5-FU + CO2 Laser + Inf alpha-2a (high dose), the treatment should not be abandoned. Topical treatment with 5-FU has a therapeutic effect; however, the benefits and risks have not been determined clearly and further studies are needed.

[UNIFESP]; Universidade de São Paulo (USP); Universidade Federal de São Paulo (UNIFESP)Objectives: the aims of this review were to summarize a) the consensus definitions of normal and pathologic intra-abdominal pressure (IAP); b) the techniques to measure IAP; c) the risk factors for intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS); d) the pathophysiology of ACS; and e) the current recommendations for management and prevention of ACS.Data Sources. PubMed was searched using the following terms: ACS, IAH, IAP, and abdominal decompression.Data Synthesis. ACS represents the natural progression of end-organ dysfunction caused by increased IAP and develops if IAH is not recognized and treated appropriately. Although the reported incidence of ACS is relatively low in critically ill children (0.6%-4.7%) it may be under-recognized and under-reported. the diagnosis of IAH/ACS depends on a high index of suspicion and the accurate and frequent measurement of IAP in patients at risk. Mortality from ACS remains high (50%-60%) even when decompression of the abdomen is performed early, which highlights the importance of detection and treatment of elevated IAP before end-organ damage occurs.Conclusions. A widespread awareness of the recognition and current approach to management and prevention of IAH and ACS is needed among pediatric intensivists, so outcome of these life-threatening disease processes might be improved. - ItemSomente MetadadadosAbility of vitamin D to reduce inflammation in adults without acute illness(Wiley-Blackwell, 2014-02-01) Zanetti, Mayra [UNIFESP]; Harris, Susan S.; Dawson-Hughes, Bess; Universidade Federal de São Paulo (UNIFESP); Tufts UnivIn vitro studies and some clinical studies suggest that vitamin D plays an important role in reducing inflammation. the objective of this review was to examine recent evidence that vitamin D status influences the level of inflammation in adults without acute illness or injury. Five large cross-sectional studies and two randomized controlled trials are the focus of this review. Associations between 25-hydroxyvitamin D (25OHD) and inflammation markers are significant and inverse in study populations with low 25OHD levels (<21ng/mL). They are also inverse in adults with relatively high inflammation levels. These associations in the few available randomized controlled vitamin D intervention trials have been null; this may be because they were not examined in populations with sufficiently low levels of 25OHD or high levels of inflammation.
