In addition, 1,25D3 has been reported to be a modulator of cell growth and differentiation in many tissues. Recently, a suppressive effect of 1,25D3 on CT secretion and synthesis in C cells Was demonstrated in vivo and also in vitro, but there are no data about its effects on thyroid C cell growth. We investigated the effects of 1,25D3 on basal and stimulated CT secretion and on [H-3]-thymidine incorporation, using a human medullary thyroid carcinoma cell line (TT cells). After a 4-day exposure to 1,25D3, TT cells showed a dose-dependent inhibition of basal CT secretion (64% of the value for the control group at 100 nM 1,25D3). Calcium (3 mM) plus K+ (50 mM) greatly increased CT secretion in both the control and vitamin D-treated groups. However, in the cells preincubated with 1,25D3 the stimulated CT levels were less than observed in controls. A dose-dependent increase in [H-3]-thymidine incorporation (200% of the value for the control group at 100 nM 1,25D3) and in cell number (150% of the value for the control group at 100 nM 1,25D3 after 72 h) was observed in the groups treated with 1,25D3. 24,25D3 had no effect on CT secretion or cell growth compared to the control group. These data show that 1,25D3 decreased basal and Ca2+-stimulated CT secretion, a specialized function of these cells, and stimulated their growth. Hence, in contrast to its effects on other cell lines, 1,125D3 appears to induce a dedifferentiation on TT cells.

Several patients worldwide with severe MAC have been treated successfully with transcatheter mitral valve replacement (TMVR) using balloon-expandable aortic transcatheter valves. The TMVR in MAC Global Registry is a multicenter registry that collects data on outcomes of these procedures. OBJECTIVES The goal of this study was to evaluate 1-year outcomes in this registry. The slides were reviewed for 60 seconds. Suspect smears were fully checked by 2 reviewers to determine the final diagnoses. A total of 2,954 sequential cytologic results were considered in this study. Of the 2,954, 2,568 (86.9%) were considered initially negative according to our internal QC, and these cases underwent PR. Also, 10% were randomly selected from these negative cases for full reviewing. The internal QC in our laboratory includes review of cases selected according to clinical and cytomorphologic criteria..Results Among the 2,954 total cases, QC detected 386 (13%) atypias with final diagnoses reported according to The Bethesda, System 2001 as follows: 82 (2.18%) low grade squamous intraepithelial lesions (LSILs), 35 (1.18%) high grade squamous intraepithelial lesions (HSILs), 2 (0.06%) squamous cell carcinomas, 105 (3.5%) atypical cells of undetermined significance (ASCUS), 4 (0.12%) atypical endocervical cells (AECs) and 158 (5.3%) unsatisfactoiy samples. RR of 2,568 smears initially considered negative selected 194 (7.5%) slides. Of the 194, 146 (75.3%) were negative, 28 (14.4%) ASC-US, 5 (2.6%) AEC, 1 (0.5%) LSIL and 14 (7.2%) unsatisfactory. Full review of a 10% randomfraction of the 2,568 cases interpreted as negative did not detect lesions but did detect 5 (1.95%) unsatisfactory samples.Conclusion Internal QC used in our laboratory based on clinical and cytomorphologic criteria to select cases for review proved to be an efficient method of detecting HSIL and cervical cancer. The consensus basis of this program strongly limits the false positive and false negative rates and also provides subjects with continuing education. One hundred percent RR is more efficient than 10% full reviewing in detecting cervical abnormalities.

[UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Universidade de São Paulo (USP)Objective: To build a life table and determine the factors related to the time of treatment of undernourished children at a nutrition rehabilitation centre (CREN), São Paulo, Brazil.Design: Nutritional status was assessed from weight-for-age, height-for-age and BMI-for-age Z-scores, while neuropsychomotor development was classified according to the milestones of childhood development. Life tables, Kaplan-Meier survival curves and Cox multiple regression models were employed in data analysis.Setting: CREN (Centre of Nutritional Recovery and Education), São Paulo, Brazil.Subjects: Undernourished children (n 228) from the southern slums of São Paulo who had received treatment at CREN under a day-hospital regime between the years 1994 and 2009.Results: the Kaplan-Meier curves of survival analysis showed statistically significant differences in the periods of treatment at CREN between children presenting different degrees of neuropsychomotor development (log-rank = 6.621; P = 0.037). Estimates based on the multivariate Cox model revealed that children aged >= 24 months at the time of admission exhibited a lower probability of nutritional rehabilitation (hazard ratio (HR) = 0.49; P = 0.046) at the end of the period compared with infants aged up 12 months. Children presenting slow development were better rehabilitated in comparison with those exhibiting adequate evolution (HR = 4.48; P = 0.023). No significant effects of sex, degree of undernutrition or birth weight on the probability of nutritional rehabilitation were found.Conclusions: Age and neuropsychomotor developmental status at the time of admission to CREN are critical factors in determining the duration of treatment.
- ItemSomente Metadadados17 alpha-HYDROXYLASE DEFICIENCY IS AN UNDERDIAGNOSED DISEASE: HIGH FREQUENCY OF MISDIAGNOSES IN A LARGE COHORT OF BRAZILIAN PATIENTS(Amer Assoc Clinical Endocrinologists, 2018) Fontenele, Rafaela [UNIFESP]; Costa-Santos, Marivania [UNIFESP]; Kater, Claudio E. [UNIFESP]Objective: 17 alpha-Hydroxylase deficiency (P450c17D) is characterized by hypogonadism and mineralocorticoid hypertension. We aimed to estimate the relative incidence and spectrum of preliminary misdiagnoses in Brazilian P450c17D patients. Methods: In this cross-sectional study, we reviewed, updated, and analyzed data of 40 P450c17D patients (21 XY, 19 XX). Results: Complete data were unavailable for 2 patients. Seven patients were relatives of an index case. Of the 31 index cases, 29 (94%) received a total of 16 misdiagnoses (1-4 per patient) before confirmation of P450c17D. Essential hypertension (55%), pure gonadal dysgenesis (35%), and androgen resistance syndrome (21%) were the most frequent misdiagnoses. Median ages at initial and final diagnosis were 13.2 and 16.5 years, respectively, with an average interval to diagnosis of 3.2 years. Initially, 38 (95%) patients had hypertension, and 75% had hypokalemia. Primary amenorrhea and sexual infantilism were present in 95% patients, and 73% were at Tanner stage I. All had low-to-undetectable estrogens and androgens with elevated gonadotropins and progesterone (580 +/- 53 ng/dL). Several had recurrent infections in childhood and neurological issues prior to final diagnosis and/or had siblings who died of infectious diseases or unknown causes before puberty. Conclusion: The high percentage of prior misdiagnoses in P450c17D patients may be attributable to the rarity of and relative unfamiliarity with the disease, its varied clinical presentation, and the limited access to critical steroid dosages and genotyping. Reduced sex steroids, and elevated gonadotropins and progesterone levels, in addition to mineralocorticoid hypertension, are pathognomonic of P450c17D. CYP17A1 gene mutations provide a definitive diagnosis.

However, H2O2 signals also fine-tune responses to growth factors and cytokines controlling cell division, differentiation, and proliferation. Because 17 beta-estradiol (E-2) also plays important roles in these processes, and is considered a major risk factor in the development and progression of endometriosis, this study evaluated whether E-2 has an antiapoptotic effect on oxidative stress in endometrial cells in combination with steady-state H2O2 levels ([H2O2]ss). Endometrial stromal cells were prepared from the eutopic endometrium of 18 women with and without endometriosis to produce primary cells. These cells were stimulated with E-2 for 20 h, exposed to [H2O2]ss, and examined for cell viability, proliferation, and apoptosis. the endometrial cells from women with endometriosis maintained the steady state for 120 min at high H2O2 concentrations. When they were pretreated with E-2 and exposed to [H2O2]ss, a decrease in apoptosis level was observed compared to the control cells (p < 0.01). the endometrial cells from patients with endometriosis subjected to both E-2 and [H2O2]ss showed increased ERK phosphorylation. These findings suggested that H2O2 is a signaling molecule that downregulates apoptosis in endometrial cells, supporting the fact that endometriosis, albeit a benign disease, shares some features with cancer such as decreased catalase levels. These results link the E-2 effects on [H2O2]ss to resistance to apoptosis and progression of endometriosis. (C) 2013 Elsevier Inc. All rights reserved.

RT-PCR detected transcripts for the estrogen receptors ESR1 and ESR2 in cultured immature Sertoli cells and in the testis of 15-, 28-, and 120-day-old rats. the expression of ESR1 and ESR2 was confirmed in Sertoli cells by immunofluorescence and Western blot. Immunohistochemistry with cryosections of testes from immature and adult rats revealed that ESR1 is present in Sertoli, Leydig, and some peritubular myoid cells, and ESR2 is present in multiple cell types, including germ cells. Treatment of Sertoli cells with 17beta-estradiol (E-2) induced a translocation of ESR1 and ESR2 to the plasma membrane and a, concomitant phosphorylation of MAPK3/1. Both effects reached a maximum after 10 min and were blocked by PP2, an inhibitor of the SRC family of protein tyrosine kinases, and by the antiestrogen ICI 182,780 (10). MAPK3/1 phosphorylation was also decreased in the presence of AG 1478, an inhibitor of the epidermal growth factor receptor (EGFR) kinase, and in the presence of MAP2K1/2 inhibitor UO126. Treatment with 2 for 24 h increased the incorporation of [methyl-H-3]thymidine, which was blocked by ICI. These results indicate that E2 activates an SRC-mediated translocation of estrogen receptors to the plasma membrane, which results in the activation of EGFR and the mitogen-activated protein kinase signaling pathway. in addition, activation of ESR1 and/or ESR2 by E 2 is involved in proliferation of immature Sertoli cells. the estrogen actions in Sertoli cells might be a key step mediating cellular events important for spermatogenesis and fertility.

- ItemSomente Metadadados17Beta-Estradiol Signaling and Regulation of Proliferation and Apoptosis of Rat Sertoli Cells(Soc Study Reproduction, 2012-04-01) Royer, Carine [UNIFESP]; Lucas, Thais Fabiana Gameiro [UNIFESP]; Lazari, Maria de Fatima Magalhaes [UNIFESP]; Porto, Catarina Segreti [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)The aim of the present study was to investigate the intracellular signaling events downstream of the classical estrogen receptors (ESRs) and G protein-coupled estrogen receptor 1 (GPER) involved in regulation of proliferation and apoptosis of rat Sertoli cells, in which we have previously described ESR1, ESR2, and GPER. ESRs play a role in Sertoli cell proliferation, and GPER, but not ESRs, plays a role modulating gene expression involved with apoptosis. the present study shows that 17beta-estradiol (E2) and the GPER-selective agonist G-1 rapidly activate phosphatidylinositol 3-kinase (PIK3)/serine threonine protein kinase (AKT) and cyclic AMP response element-binding (CREB) phosphorylation. E2 and the ESR1-selective agonist 4,4',4 ''-(4-propyl-(1H)-pyrazole-1,3,5-triyl)trisphenol (PPT) increase the expression of cyclin D1 (CCND1), whereas the ESR2-selective agonist 2,3-bis(4-hydroxyphenyl)-propionitrile (DPN) and G-1 do not change the expression of this protein, suggesting that ESR1 is the upstream receptor regulating Sertoli cell proliferation. E2- or PPT-ESR1, through activation of epidermal growth factor receptor (EGFR)/mitogen-activated protein kinase 3/1 (MAPK3/1) and PIK3 pathways, induces upregulation of CCND1. KG-501, the compound that disrupts the phospho-CREB/CREB binding protein (CBP) complex, does not change E2-or PPT-ESR1-mediated CCND1 expression, suggesting that phospho-CREB/cyclic AMP response element/CBP is not involved in the expression of this protein. E2- or G-1-GPER, through activation of EGFR/MAPK3/1 and PIK3 pathways, may be involved in the upregulation of antiapoptotic proteins BCL2 and BCL2L2. E2- or G-1-GPER/EGFR/MAPK3/1/phospho-CREB decreases BAX expression. Taken together, these results show a differential effect of E2-GPER on the CREB-mediated transcription of proapoptotic and antiapoptotic genes of the same BCL2 gene family. ESR1 and GPER can mediate the rapid E2 actions in the Sertoli cells, which in turn can modulate nuclear transcriptional events important for Sertoli cell function and maintenance of normal testis development and homeostasis. Our findings are important to clarify the role of estrogen in a critical period of testicular development, and to direct further studies, which may contribute to better understanding of the causes of male infertility.

Within a year, Torek performed the first transpleural resection for cancer in the thoracic esophagus, Zaaijer successfully resected the distal esophagus through the transpleural route, Heller performed the first myotomy for achalasia, and it was also the year of Ach's pioneering transmediastinal esophagectomy. Previously, in 1912, after a series of animal experiments by Beck and Jianu, Roepke successfully used the greater curvature of the stomach as a presternal conduit. Other previous approaches included reconstruction with jejunum (Roux1907), colon (Kelling1911), and skin tube (Bircher1907). Several technical advances made these operations possible, most of all were the giant leaps in the perioperative medicine.

- ItemSomente Metadadados1º Levantamento nacional sobre padrões de consumo de álcool na população brasileira(Universidade Federal de São Paulo (UNIFESP), 2009) Zaleski, Marcos José Barreto [UNIFESP]; Pinsky, Ilana [UNIFESP]Introdução: este estudo apresenta duas etapas distintas: a metodologia do 1º levantamento nacional sobre os padrões de consumo de álcool na população Brasileira, seguida pela análise dos dados sobre violência entre parceiros íntimos (VPI) e consumo de álcool durante o evento. Objetivos: descrever a metodologia do trabalho de campo e os desafios de sua realização. Analisar os índices de prevalência de violência entre parceiros íntimos (VPI) entre casais brasileiros, avaliando a contribuição do álcool e de fatores sócio-demográficos ao risco de VPI. Metodologia: a metodologia foi desenhada para ter abrangência do território nacional, apresentando um perfil da população brasileira a partir de 14 anos de idade. A amostra populacional total foi composta por 2.522 indivíduos de 14 anos e mais, e uma cota extra de 485 indivíduos entre 14 e 17 anos de idade, totalizando 3.007 entrevistas, com a sub-amostra analisada para os dados de VPI foi composta por 1445 homens e mulheres casados ou vivendo em união estável. Foram selecionados aleatoriamente 143 municípios brasileiros totalizando 325 setores censitários. A amostra foi feita em 03 estágios. Estágio 1: sorteio sistemático dos municípios, selecionados probabilisticamente pelo método PPT (Probabilidade Proporcional ao Tamanho), sendo a população de cada município tomada como base para tal seleção. Estágio 2: seleção probabilística dos setores censitários também pelo mesmo método (PPT). Estágio 3: contagem e sorteio aleatório simples dos domicílios e sorteio do respondente, dentro dos domicílios sorteados. As entrevistas foram realizadas entre novembro de 2005 e abril de 2006. Resultados: a taxa de resposta final global obtida foi de 66%, variando de 80 a 56% nas diferentes regiões do Brasil. A maior parte dos casos de não-resposta estava associada a dificuldades no acesso a domicílios em condomínios fechados. Procedimentos especiais de trabalho-de-campo efetuados nas áreas de baixa taxa de resposta tiveram um efeito positivo, aumentando a taxa de resposta final global em cerca de 11 pontos percentuais. Com relação à aos dados de VPI, as análises indicaram uma prevalência de 10,7% de violência de parceiro intimo homem contra mulher e uma prevalência de 14,6% de violência de parceiro íntimo mulher contra homem (incluindo episódios classificados como agressão mútua, somente perpetração e somente vitimização). Os homens consumiam álcool em 38,1% dos casos de VPI e mulheres em 9,2%. A parceira do homem consumia álcool em 30,8% dos atos de VPI e o parceiro da mulher em 44,6%. Foram encontradas associações bivariadas para fatores sócio-demográficos como idade e renda familiar (para ambos os sexos), e ainda para nível educacional com relação à violência perpetrada pela mulher. A análise de regressão logística indicou que o grupo a idade mais jovem (14-29 anos) é um significante fator preditor tanto de perpetração de violência de parceiro homem contra mulher (HMVP) quanto de violência de parceiro mulher contra homem (MHVP). Além do mais, homens que informaram não ter religião têm maior envolvimento em HMVP quando comparado aos Católicos, o maior grupo religioso da amostra. Finalmente, foi observado que ser dona-de-casa é um fator protetor para MHVP, quando comparado ao grupo de mulheres empregadas. Conclusões: o estudo apresenta uma metodologia pioneira nessa área e sua dificuldade de implantação em âmbito nacional, além de trazer um maior conhecimento do relacionamento dinâmico entre VPI e consumo de bebidas alcoólicas durante o evento. Esses dados fornecem importantes evidências científicas, auxiliando na implantação de políticas públicas para prevenção da violência, assim como para a prevenção e tratamento do uso abusivo do álcool e redução do seu custo social.

Objective: To estimate the prevalence of depressive symptoms in the Brazilian population and establish their sociodemographic correlates. To study the associations between depressive symptoms and alcohol-related disorders, drinking patterns and other characteristics of alcohol use and search for related socio-demographic correlations in men and women. Method: A cross-sectional study was conducted between November 2005 and April 2006. Data were collected in face-to-face interviews using a standardized questionnaire. The sample consisted of 3,007 interviews with individuals 14 years of age and older and followed a probabilistic design covering the Brazilian national territory. Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale and alcohol dependence was assessed using the Composite International Diagnostic Interview. Associations assessed using bivariate analysis were tested using Rao-Scott measures. Gender specific multinomial logistic regression models were developed. Results: The observed prevalence of depressive symptoms was 28.3% (13% mild/moderate; and 15.3% major/severe; p<0.01). Increased depressive symptoms rates were associated with being a female, being 45 years of age and older, having lower education, being single, having family income of up to 2.5 times minimum wage, and living in the Northern Region of Brazil (p<0.05). Among the participants with alcohol dependence, 46% had depressive symptoms (17.2% mild/moderate and 28.8% major/severe; p<0.01); 35.8% (p=0.08) of those with alcohol abuse and 23.9% (p<0.01) of those with a binge-drinking pattern also had depressive symptoms. Alcohol abstainers and infrequent drinkers had the highest prevalence of major/severe depressive symptoms, whereas frequent heavy drinkers had the lowest prevalence of major/severe depressive symptoms. In women, alcohol dependence and the presence of one or more problems related to alcohol consumption were associated with higher risks of major/severe depressive symptoms. Among men, alcohol dependence and being ≥45 years old were associated with higher risks of major/severe depressive symptoms. Conclusions: In Brazil, the prevalence of depressive symptoms is high and is strongly related to alcohol dependence. The strongest association was between major/severe depressive symptoms and alcohol dependence in women. This survey supports the possible association of biopsychosocial distress, alcohol consumption and the prevalence of depressive symptoms in Brazil. Investing in education, social programs, and care for those with alcohol dependence and major/severe depressive symptoms, especially for such women, and the development of alcohol prevention policies may be components of a strategic plan to reduce the prevalence of depression and alcohol problems in Brazil.
