METHODS: 12 patients were assessed through the questionnaire Harris Hip Score (modified/scale of 0-95 points) and goniometry in the pre- and post-surgery of total hip artroplasty. RESULTS: The pre-surgery questionnaire score was 39.7 and in the post-surgery the average score reached 74.1 points (p = 0.000001). The range of motion of the hip was incomplete in all the movement arches in the pre-surgery, but in the post-surgery this value increased significantly in flexion (p = 0.007), extension (p = 0.02), abduction (p = 0.001, medial rotation (p = 0.0002) and lateral rotation (p = 0.01); only the aduction movement was not significantly increased (p = 0.08). CONCLUSIONS: Total hip artroplasty reliefs patients pain, improves daily life activities and increases the hip range of motion; patients, however, relate fear and insecurity in the accomplishment of those activities.

Objective:Analyze level of dysphagia, oral ingestion, anxiety levels and nutritional status of patients with stroke diagnosis, before and after speech therapy. Method:Clinical assessment of dysphagia partially using the Protocol of Risk Assessment for Dysphagia (PARD), applying the scale Functional Oral Intake Scale for Dysphagia in Stroke Patients (FOIS), Beck Anxiety Inventory (BAI) and the Mini Nutritional Assessment MNA®. The sample consisted of 12 patients, mean age of 64.6 years, with a medical diagnosis of hemorrhagic and ischemic stroke and without cognitive disorders. All tests were applied before and after speech therapy (15 sessions). Statistical analysis was performed using the chi-square test or Fisher's exact test, McNemar's test, Bowker's symmetry test and Wilcoxon's test. Results:During the pre-speech therapy assessments, 33.3% of patients had mild to moderate dysphagia, 88.2% did not receive food orally, 47.1% of the patients showed malnutrition and 35.3% of patients had mild anxiety level. After the therapy sessions, it was found that 33.3% of patients had mild dysphagia, 16.7% were malnourished and 50% of patients had minimal level of anxiety. Conclusion:There were statistically significant evolution of the level of dysphagia (p = 0.017) and oral intake (p = 0.003) post-speech therapy. Although not statistically significant, there was considerable progress in relation to the level of anxiety and nutritional status.

Setting: Tertiary referral center.Patients: Twenty-one patients presenting chronic dizziness with uncompensated unilateral vestibular loss.Intervention: All patients underwent a regimen of 10 sessions of aquatic physiotherapy for vestibular rehabilitation.Main outcome measures: Patient evaluation prior to and following rehabilitation involved the application of the Brazilian version of the Dizziness Handicap Inventory (DHI), dynamic computerized posturography and the self-perception scale of dizziness intensity. Statistical analysis compared average variables prior to and following rehabilitation using the Student t test.Results: Brazilian DHI total scores were lower following rehabilitation (p = 0.001). Posturography revealed a reduction in the variation of body displacement following rehabilitation in the average stability and anterior/posterior stability indexes (p = 0.001) and in the average medial/lateral stability index (p = 0.003). Self-perception of dizziness intensity was lower following rehabilitation (p = 0.001). No association between age, time since symptom onset and use of antivertigo medication and rehabilitation therapeutic effects were found.Conclusion: Unilateral vestibular hypofunction patients undergoing aquatic physiotherapy for vestibular rehabilitation achieved an improvement in quality of life, body balance and self-perception of dizziness intensity, regardless of age, time since symptom onset, and use of antivertigo medication.
- ItemAcesso aberto (Open Access)Associação entre a ocorrência de quedas e o desempenho no Incremental Shuttle Walk Test em mulheres idosas(Associação Brasileira de Pesquisa e Pós-Graduação em Fisioterapia, 2012-08-01) Bardin, Marcela Grigol [UNIFESP]; Dourado, Victor Zuniga [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)BACKGROUND: Because the incremental shuttle walk test (ISWT) requires agility in its performance, we hypothesized that the test may be associated with balance and risk of falls in the elderly. Objective: To evaluate and compare the association between the performance on the ISWT, the timed up and go (TUG) and the occurrence of falls in the elderly. METHOD: Thirty-three elderly individuals (68±7 years) performed the TUG and the ISWT. Balance was assessed using the Berg Scale (BBS). Participants who fell at least twice in the last 12 months were placed in the falls group (FG) and all other participants comprised the control group (CG). RESULTS: There were seventeen elderly women in the FG and 16 in the CG. Participants from the FG had a significant worse performance (p<0.05) on the TUG (8.01±0.22 vs. 6.22±0.21 s), BBS (51±3 vs. 55±1 points) and ISWT [313±79 (92±15%pred.) vs. 395±75 m (113±19%pred.)] than participants from the CG. The ISWT significantly correlated with the TUG (r=-0.75, p<0.001), BBS (r=0.50, p=0.002) and number of falls (r=0.36, p=0.031). After logistic regression, the TUG was determinant (p=0.03) and the ISWT showed a tendency to determine the occurrence of falls (p=0.05). CONCLUSION: The ISWT was a valid measure to assess the risk of falls and balance and therefore, may be useful for the simultaneous assessment of cardiorespiratory fitness and balance in older women.

Almost 50% of the cases are caused by primary deficiency of a protein named merosin (MD), and present a homogenous phenotype with a severe motor and respiratory involvement. Eleven children with clinical and histological diagnosis of CMD-MD, aged of 3 to 15 years, were studied using the manual muscle testing (Medical Research Council), goniometric analysis, motor ability and day life activities (Barthel index) scales, with the objective to characterize the main motor function limitations. The muscular groups most affected were cervical flexors, paravertebral and proximal portions of limbs. The muscular groups of upper limbs were as affected as the lower limbs, and the extensors were more affected than the flexors groups. All children had severe muscular retractions on the hip, knee and elbow. Other frequent deformities were scoliosis and equinus-varum feet. No children presented the motor ability to walk, stand up and crawl; and all of them were classified as dependents or semi-dependents in the day life activities scale. Our findings confirm the severe and diffuse involvement of skeletal muscle in CMD-MD patients, producing serious motor limitations and deformities.

AIM: To study the recurrence and persistence of BPPV in patients treated with canalith repositioning maneuvers (CRM) during the period of one year. STUDY DESIGN: longitudinal contemporary cohort series. MATERIALS AND METHODS: One hundred patients with BPPV were followed up during 12 months after a treatment with CRM. Patients were classified according to disease evolution. Aquatic physiotherapy for vestibular rehabilitation (APVR) protocol was applied in cases of persistent BPPV. RESULTS: After CRM, 96% of the patients were free from BPPV's typical nystagmus and dizziness. During the follow up period of 1 year, 26 patients returned with typical BPPV nystagmus and vertigo. Nystagmus and vertigo were persistent in 4% of the patients. Persistent BPPV presented improvement when submitted to APVR. Conclusion: During the period of one year, BPPV was not recurrent in 70% of the patients, recurrent in 26% and persistent in 4%.

Design: longitudinal, descriptive and analytical study.Method: Elderly patients with clinical diagnosis of BPPV submitted to Balance Rehabilitation Unit static posturography in 10 sensory conditions at three time intervals: before and after the repositioning maneuver and 12 months after the treatment.Results: We studied 23 subjects with a mean age of 68.74 years. Posturography revealed that the stability limit was not significantly different when the three time intervals were compared (p = 0.405). the center of pressure (CoP) showed a significant change in condition 2 (stable surface and closed eyes), because after the repositioning maneuver, the CoP significantly differed vis-a-vis the results before and 12 months after the treatment (p = 0.003). the values of body velocity sway (BVS) were significantly different in six sensoly conditions in these three time intervals.Conclusion: 12 months after the treatment for BPPV, the static posturography showed balance abnormalities similar to those found before treatment.

Visual dynamic assessment could be applied to classify this alteration based on the clinical observation of the winging of the inferior medial scapular border (Type I) or of the prominence of the entire medial border (Type II), or by the excessive superior translation of the scapula (Type III).Objective:The aim of this study was to determine if there were differences in scapular rotations (Type I and II) and position (Type III) between a group of subjects with scapular dyskinesis, diagnosed by the clinical observation of an expert physical therapist, using a group of healthy individuals (Type IV).Method:Twenty-six asymptomatic subjects volunteered for this study. After a fatigue protocol for the periscapular muscles, the dynamic scapular dyskinesis tests were conducted to visually classify each scapula into one of the four categories (Type IV dyskinesis-free). The kinematic variables studied were the differences between the maximum rotational dysfunctions and the minimum value that represented both normal function and a small dysfunctional movement.Results:Only scapular anterior tilt was significantly greater in the type I dyskinesis group (clinical observation of the posterior projection of the inferior angle of the scapula) when compared to the scapular dyskinesis-free group (p=0.037 scapular and p=0.001 sagittal plane).Conclusions:Clinical observation was considered appropriate only in the diagnoses of dyskinesis type I. Considering the lower prevalence and sample sizes for types II and III, further studies are necessary to validate the clinical observation as a tool to diagnose scapular dyskinesis.

Methods: A prospective controlled intervention study carried out from 2006 to 2013 with 293 elders. The intervention group (n = 160) attended a cognitive cooperation group (20 sessions of 1.5 hours each). The control group (n = 133) received routine medical care. Outcome was the percent variation in the MMSE. Control variables included gender, age, marital status, schooling, hypertension, diabetes, dyslipidaemia, hypothyroidism, depression, vascular diseases, polymedication, use of benzodiazepines, exposure to tobacco, sedentary lifestyle, obesity and functional capacity. The final model was obtained by multivariate linear regression. Results: The intervention group obtained an independent positive variation of 24.39% (CI 95% = 14.86/33.91) in the MMSE compared to the control group. Conclusion: The results suggested that cognitive cooperation groups, mediated by computers and the internet, are associated with cognitive status improvement of older adults in memory clinics.

Arch Phys Med Rehabil 2004;85:162-5.Objective: To compare transmissivity data and thermal behavior of 4 coupling media.Design: Experimental.Setting: Postgraduate rehabilitation program in Brazil.Specimens: Four coupling media: gel, mineral oil, white petrolatum, and degassed water.Interventions: Not applicable.Main Outcome Measures: the transmission, attenuation, reflection coefficient, and acoustic impedance of gel, mineral oil, white petrolatum, and degassed water were measured with a density measurement cell. the temperature variation in the therapeutic ultrasound transducer was measured with a thermocouple.Results: the transmissivity data showed that the water and gel presented the highest transmission coefficient, the lowest reflection, and an attenuation coefficient and acoustic impedance close to that of the skin. the thermal data revealed the highest heating in the transducer during the insonation with white petrolatum and mineral oil, resulting from the thermal conductivity features of each medium.Conclusions: Transmissivity data obtained showed that water and-gel present the best acoustic features. in ultrasound therapy, with the direct contact technique using thin layers of coupling agents, any product may be used, because the effect of the attenuation coefficient does not play a significant role when layers are as thin as those used in this experiment.

SOURCES OF DATA: search of Medline and Lilacs databases; publications of scientific committees of institutions for children with special needs; and outpatient care reports of reference centers for the treatment of children and adolescents with mental and developmental disabilities. SUMMARY OF THE FINDINGS: this population presents health problems associated with the basic pathology and its consequences in addition to the usual problems of the specific age group. The etiology of developmental disorders and the main characteristics of each type of disability are discussed. Special attention is given to outpatient pediatric care. CONCLUSIONS: the most recent study of the Brazilian population, performed in 2000, showed that 14.5% of this population presented some type of developmental deficiency. Therefore, developmental problems are one of the most prevalent health problems among children and adolescents. Consequently, pediatricians need to be prepared to evaluate and identify factors that may influence normal children development. Pediatricians are responsible for prevention, early diagnosis and coordination of the multidisciplinary treatment of these patients in addition to basic medical assistance, which is essential to guarantee the patients' good quality of life.

[UNIFESP]; Ribeiro, PedroPost-polio syndrome (PPS) is a condition that affects polio survivors years after recovery from an initial acute attack of the poliomyelitis virus. Most often, polio survivors experience a gradual new weakening in muscles that were previously affected by the polio infection. The actual incidence of cardiovascular diseases (CVDs) in individuals suffering from PPS is not known. However, there is a reason to suspect that individuals with PPS might be at increased risk. Method: A search for papers was made in the databases Bireme, Scielo and Pubmed with the following keywords: post polio syndrome, cardiorespiratory and rehabilitation in English, French and Spanish languages. Although we targeted only seek current studies on the topic in question, only the relevant (double-blind, randomized-controlled and consensus articles) were considered. Results and Discussion: Certain features of PPS such as generalized fatigue, generalized and specific muscle weakness, joint and/or muscle pain may result in physical inactivity deconditioning obesity and dyslipidemia. Respiratory difficulties are common and may result in hypoxemia. Conclusion: Only when evaluated and treated promptly, somE patients can obtain the full benefits of the use of respiratory muscles aids as far as quality of life is concerned.

Thigh circumference was also measured and an incremental unilateral cardiopulmonary exercise test was performed separately for both involved and uninvolved lower limbs in order to compare heart rate, oxygen consumption, minute ventilation, and ventilatory pattern (breath rate, tidal volume, inspiratory time, expiratory time, tidal volume/inspiratory time) at three different workloads (moderate, anaerobic threshold, and maximal).Results:There was a significant difference between isokinetic extensor peak torque measured in the involved (116.5±29.1 Nm) and uninvolved (220.8±40.4 Nm) limbs, p=0.000. Isokinetic flexor peak torque was also lower in the involved limb than in the uninvolved limb (107.8±15.4 and 132.5±26.3 Nm, p=0.004, respectively). Lower values were also found in involved thigh circumference as compared with uninvolved limb (46.9±4.3 and 48.5±3.9 cm, p=0.005, respectively). No differences were found between the lower limbs in any of the variables of the incremental cardiopulmonary tests at all exercise intensities.Conclusions: Our findings indicate that, four months after ACL surgery, there is a significant deficit in isokinetic strength in the involved limb, but these differences in muscle strength requirement do not produce differences in the cardiorespiratory adjustments to exercise. Based on the hypotheses from the literature which explain the differences in the physiological responses to exercise for different muscle masses, we can deduce that, after 4 months of a rehabilitation program after an ACL reconstruction, individuals probably do not present differences in muscle oxidative and peripheral perfusion capacities that could elicit higher levels of peripheral cardiorepiratory stimulus during exercise.

There are several types of treatment, with varied results. The use of knee sleeve as an adjuvant resource has been controversial in the literature. OBJECTIVE: To assess the immediate efficacy of elastic knee sleeve on pain and functional capacity of individuals with KOA. METHODS: Seventy-four patients (132 knees) with symptomatic KOA were assessed by use of the Stair Climb Power Test (SCPT), Timed Up and Go (TUG) and 8-Meter Walk (8MW) tests, in addition to the VAS for pain. The tests were performed with and without knee sleeves, with a cover on the knees to hide knee sleeve. The order and the presence of the knee sleeve were randomized, and the investigator was blind. RESULTS: A statistically significant difference was found between the two compared circumstances (with and without knee sleeve) when using the VAS (P < 0.001), which showed a reduction in pain with the knee sleeve use. Analyses of the three functional tests under both circumstances were performed, resulting in statistically significant differences in 8MW and TUG tests (P < 0.05), but not in SCPT (P > 0.1339). CONCLUSION: The elastic knee sleeve proved to be effective to immediately improve the functional capacity and pain of individuals with KOA, because it enhanced performance during the tests proposed. Thus, the knee sleeve is an adjuvant resource for treating KOA, because it is practical, useful, and of easy clinical use, and can aid in the practice of therapeutic exercises.

Methods: Women who had pain in the shoulder girdle and upper limb region after three months of surgery , aged 18 years and pain level ? 3 in Visual Analog Scale were included. The patients were randomly divided into two treatment groups received weekly for 10 weeks. Kinesiotherapy group (G1): kinesiotherapy treated with standard pre-defined, lasting 30 minutes. Acupuncture Group + kinesiotherapy (G2): treated with the same protocol kinesiotherapy group followed by another 30 minutes of acupuncture used in pre-defined points. Both groups underwent a physical examination and completed questionnaires of upper limb function and depressive symptoms. Results: 48 patients completed the treatment, and 24 in each group. Regarding pain, the two groups had statistically significant improvement with p < 0,01 at all time points assessed. In the analysis of depression improved significantly only in G1 compared the 1st versus 10th session. The upper limb function had improved only when comparing G1 1st versus 10th session, and G2 showed improvement in all three evaluation times. The range of motion showed improvement in all evaluated movements, movements flexion, extension and internal rotation was improved in G1 and G2 in the comparison of the 1 st session versus 5 th and 1st versus 10th session. In adduction, there was significant improvement only in G2 compared the 1st versus 10th session. Already abduction was no difference in G1 compared to the 1st versus 10th session and G2 in the 1 st session versus 5 th and 1st versus 10th session. Regarding external rotation was improved only in G1 at the 1 st session versus 5 th and 1st versus 10th session. Conclusion: The kinesiotherapy with or without acupuncture was effective for pain relief and function of the upper limb. Depressive symptoms showed significant improvement in the group that did only kinesiotherapy.

Daoyin Qigong (DQ) is a rehabilitation technique in Traditional Chinese Medicine. Objective: verify the effects of DQ in PSS patients complaining of cold intolerance. Methods: Thirty-six patients, twenty-six women and ten men, ages between 35-60 years were submitted to DQ technique, during a 40-minute session two times a week for three consecutive months. Reassessed every three months after DQ intervention using a visual analogical scale (VAS), Fadigue Severity Scale (FSS) and Whoqol-bref. Results: A statistically significant difference was observed in the systemic VAS-Cold after the end of DQ intervention. Conclusion: DQ ameliorated CI complaint, pain and fatigue in the patients, positively reflecting the quality of life. They presented a low score of cold sensitivity at the end of the intervention.

The aim of the present study was to assess the effectiveness of belly dance as a treatment option for patients with fibromyalgia.Methods. Eighty female patients with fibromyalgia between 18 to 65 years were randomly allocated to a dance group (n=40) and control group (n=40). Patients in the dance group underwent 16 weeks of belly dance twice a week, while the patients in the control group remained on a waiting list. The patients were evaluated with regard to pain (VAS), function (6MWT), quality of life (FIQ and SF-36), depression (Beck Inventory), anxiety (STAI) and self-image (BDDE). Evaluations were carried out at baseline, 16 weeks and 32 weeks by a blinded assessor.Results. The dance group achieved significant improvements in VAS for pain (p<0.001), six-minute walk test (p<0.001), FIQ (p=0.003), BDDE (p<0.009)as well as the pain (p<0.001), emotional aspects (p<0.003) and mental health (p<0.021) subscales on the SF-36.Conclusion. Belly dance can be used in the treatment of fibromyalgia to reduce pain and improve functional capacity, quality of life and self-image.

Thirty handcapped people were evaluated: fifteen started with basketball and fifteen with swimming, according either to the specific preference of each one of them or to the degree and kind of physical impairment. They were submmited to the following evaluations: clinical examination, physiotherapy assessment, social interview and use of the Rivermead Social Scale, functional classification of the sport, use of the Barthel and Rivermead Functional Scales, and the psychological profile test (POMS). After two years, no relevant change in the moving evolution of the athletes were reported. Concerning the POMS psychological test, both basketball and swimming groups presented with high vigor and low depression levels. Considering the social aspects, both groups presented substantial improvement, specially regarding their relationship to one person or more people and also in the everyday activities (be it social, leisure, or domestic), thus leading them to better social integration. This essay shows that sport can bring people who are physically impaired a better social integration and physical conditions.

However, several aspects such as intensity and pain during therapy still differ among authors. OBJECTIVES: To compare the effectiveness of two protocols of eccentric exercise (performed with and without pain), in the improvement of knee function and pain intensity in athletes with patellar tendinopathy. METHODS: 7 athletes of both genders with diagnosis of patellar tendinopathy were selected and randomly assigned to two treatment groups. The first group of volunteers performed eccentric squat exercises on a slope with pain in the patellar tendon. The second group of volunteers was instructed to perform the same exercise, but without presenting pain in the patellar tendon during performance. The treatment lasted 12 weeks and the evaluation of pain and function was performed by the VISA-P and VAS before starting treatment, at eight weeks of intervention, and at treatment completion. RESULTS: There was improvement in both groups when results of evaluations carried out after eight and 12 weeks of beginning of the treatment were compared with the initial evaluation; however, no significant difference between the group that performed exercise with pain and the group that performed exercises without pain was found. Analysis of the probability of obtaining clinical improvement for VISA and VAS scores at eight and 12 weeks, there was no difference between the two groups. CONCLUSION: A program of eccentric exercise on an inclined plane using squat, performed with or without the presence of pain, was effective in improving pain and function in athletes with patellar tendinopathy.

Methods: We evaluated and compared in a descriptive study with 24 women, two groups: physiotherapy treatment (G1) and psychological treatment and physiotherapy (G2). Both groups underwent screening and were assessed through evaluation form by the multidisciplinary team (Doctors, Psychologists, Physiotherapists) sector of female sexuality Unifesp. To evaluate the physical therapy evaluation was performed bodily and physical assessment of pelvic floor muscle using palpation method and perineal body (subjective) so we could see any changes involuntary spastic contraction. We evaluated the physical and functional integrity of the pelvic floor in order to assess possible changes in muscle through digital examination and/or bidigital (objective evaluation). Were also collected during the evaluation questionnaires Sexual Quality of Life ? Female Sexual Quotient and Inventory and Beck ? BDI (psychological evaluation) for both the G1 Group as well as for the Group G2. Only underwent the treatment, patients with diagnosis of vaginismus. After signing the consent form, the same was referred for treatment with physical therapy and psychology individualized treatments. The number of sessions for therapy (physiotherapy and psychological) were 10 initial sessions the treatment may be extended for 10 more sessions if the multidisciplinary team thought very necessary. The treatment group G1 and G2 were started by working sex education (through the mirror technique and illustrative booklet (anatomy of the internal and external genitalia). Treatment Group G1 was accomplished through relaxation techniques and perineal body, respiratory training, normalization of body tone and perineal massage performed with and desensitization and perineal body and application of specific exercises (body awareness ? and acceptance verbalization through visualization and perineal body in the mirror) (mirror technique ? visualization body) and (autofocus ? visual and tactile perception of the body in the mirror). Results: The study included 15 patients in Group G1 and 9 patients in Group G2. With respect to gynecological assessment, initially seventeen (70.83%) failed to perform evaluation at the end of treatment the twenty-for (100.0%) have done it. In assessing physiotherapy, seven (46.67%) and nine in Group G1 (100.0%) in Group G2 conducted subjective evaluation and eight (53.33%) underwent objective evaluation the end of treatment the twenty-four (100.0%) of the patients began to perform objective evaluation. There was also improvement in relation to the scores of QSF (Female Sexual Quotient) and BDI (Beck Inventory of Psychological Assessment) and regarding the Partial and Total Penetration. Conclusions: physiotherapy should be an integral part of the multidisciplinary team treating sexual dysfunctions, we conclude also that the resources physiotherapeutic help produce physical changes in the body and perineal muscle, central nervous system, altering the mechanism of pain through the reeducation sensory motor.
