METHODS: One hundred patients were studied, with ages ranging between 30 and 90 years old (median = 55 years old). There were 44 males and 56 females. We divided them in two groups. One group had fifty patients with skin cancers, equal or larger than 10mm in the greater axis, and the pathological status was confirmed by previous biopsy. The control group had 50 patients without skin cancer. Rosenberg/UNIFESP-EPM's Self-esteem Scale was used to evaluate self-esteem. Data were analyzed by Mann - Whitney's test and Kruskal Wallis test. RESULTS: There was no significant difference between the two groups in relation to age (p = 0.331), sex and education. The mean tumor size was 20.9 ± 2 mm. Skin cancer patients presented significantly lower self-esteem, with average score of 6.84 and 3.90 for the control group (p = 0.002). CONCLUSION: Patients with skin cancer of the head and neck presented lower self-esteem than individuals without cancer.

Clinically it presents as a slow-growing, well-defined, hyperpigmented plaque, and should be included as a differential diagnosis of other pigmented lesions. The authors describe a challenging case of pigmented Bowen's disease with non-diagnostic dermscopy findings.

It occurs in young adults of both genders. Lesions can be single or multiple and more frequently involve extremities. Pain may occur spantaneously or after physical stimulation. We describe a case of unilateral multiple piloleiomyoma in a young woman, complaining of itching lesions.

A degeneração causada pela radiação ultravioleta (UV) sobre diversas estruturas celulares e moleculares determina diversas alterações cutâneas, entre elas, o câncer de pele. As queratoses actínicas (QAs) múltiplas, uma das características do fotoenvelhecimento avançado, são consideradas lesões pré neoplásicas e constituem o chamado campo de cancerização (CC). Fotoenvelhecimento avançado e CC são fenômenos cutâneos que se desenvolvem paralelamente. O uso tópico de 5-Fluorouracil (5-FU) é aprovado para o tratamento de QAs. No entanto, suas limitações de uso estão associadas aos efeitos colaterais como eritema, descamação, crostas e prurido. Atualmente, o desenvolvimento de medicações tópicas empregando novas tecnologias, como a homogeneização de alta velocidade, visa melhor ou igual eficácia das formulações, com menos eventos adversos. Objetivos: 1) Avaliar a eficácia, perfil de segurança e tolerabilidade através dos efeitos clínicos e histológicos epidérmicos e dérmicos do 5-FU a 5% em emulsão tópica manipulada com homogeneização de alta velocidade (fórmula inovadora), comparada à emulsão tradicional no tratamento do CC e no fotoenvelhecimento avançado dos antebraços de pacientes imunocompetentes. 2) Analisar marcadores envolvidos no CC por meio da análise de imuno-histoquímica e expressão gênica. Métodos: Estudo clínico de fase II de intervenção terapêutica, randomizado, prospectivo, comparativo e duplo-cego. Após aprovação pelo Comitê de Ética em Pesquisa – Unifesp, assinatura do Termo de Consentimento Livre e Esclarecido (TCLE) e do Termo de Autorização para uso de imagens, foram incluídos 41 participantes com fotoenvelhecimento grave ou avançado dos antebraços (classificação de Guimarães, 2015) com QAs múltiplas. O esquema terapêutico compreendeu o uso do 5-FU 5% em emulsão tradicional em um antebraço (grupo Clássico) e em emulsão preparada com homogeneização de alta velocidade no antebraço contralateral (grupo Turrax), definido por randomização, duas vezes ao dia, durante quatro semanas. A avaliação da eficácia incluiu os seguintes parâmetros: 1) Clínicos: avaliação do investigador, através da Escala de Fotoenvelhecimento dos Antebraços (EFA) e contagem de QAs; e de dois observadores independentes por comparação de fotografias padronizadas; 2) Medidas instrumentais não invasivas: corneometria, cutometria e ultrassonografia (USG) de alta frequência. Todas as medidas foram realizadas antes (T0) e após 12 semanas do início do tratamento (T12); 3) Biópsias de pele em T0 e T12, uma em cada antebraço para exames histológico, imuno-histoquímico e análise da expressão gênica e outra biópsia em T0 na região ventral do braço esquerdo em área não fotoexposta para comparação das vias associadas ao envelhecimento extrínseco e intrínseco (controle). A avaliação de segurança baseou-se na observação e relatos de efeitos colaterais. Resultados: Dos 41 participantes incluídos 40 completaram o estudo. Os achados clínicos confirmaram os benefícios do 5-FU 5% tópico, independentemente da formulação utilizada: 1. EFA (média grupo Turrax: T0=100/T12=88, p<0,01; média grupo Clássico: T0=101/T12=87, p<0,01; sem diferença entre os tratamentos); 2. Contagem de QAs (média grupo Turrax: T0=8/T12=0, p<0,001; média grupo Clássico: T0=9/T12=0, p<0,001; sem diferença entre os tratamentos); assim como as medidas não invasivas: corneometria (média grupo Turrax: T0=27,7/T12=33,1, p<0,01; média grupo Clássico T0=29,5/T12=32,6, p=0,04; sem diferença entre os tratamentos); cutometria (número amostral de 19, mediana R7 grupo Turrax T0=0,29/T12=0,51, p<0,01; mediana R7 grupo Clássico T0=0,27/T12=0,47, p<0,01, sem diferença entre os tratamentos); USG: SLEB (subepidermal low echogenic band) (n amostral de 19, Média Grupo Turrax T0=0,36/T12=0,36, p = 0,97; Média Grupo Clássico T0=0,39/T12=0,43, p = 0,21; com diferença entre eles p =0,04). Os achados histológicos (número amostral de 21) demonstraram melhora da classificação da neoplasia intraepidérmica de queratinócitos (KIN) e a tendência da redução da expressão imuno-histoquímica de p53 e Ki-67 epidérmicas em T12 em ambos os grupos. Os efeitos colaterais observados pelo investigador (eritema, descamação, prurido, ressecamento e crostas) ocorreram em todos os casos, em ambos os antebraços, porém, foram de intensidade leve a moderada e não apresentaram diferença estatística entre os tratamentos utilizados. Conclusão: Os tratamentos tópicos com 5-FU a 5% em creme ou preparado com homogeneização de alta velocidade foram eficazes no tratamento do CC e na melhora dos sinais de fotoenvelhecimento dos antebraços.

Statistical knowledge about the biological behavior of this tumor is essential for guiding daily outpatient practice and aiding public health policies.OBJECTIVES:To analyze the profile of patients with cutaneous melanoma attending a pathology department in Teresina (state of Piauí) between 2000 and 2010.METHODS:Retrospective study of melanoma patients diagnosed between 2000 and 2010 in the São Marcos Hospital in the city of Teresina. The pathology laboratory reports were studied and all the statistical analyses performed using SPSS 19.0.RESULTS:A total of 25 in situ, 199 invasive and 89 metastatic melanomas of unknown primary site were observed. Histological types found were nodular (52.8%), superficial spreading melanoma (18.6%), acral (10.6%) and lentigo maligna (9.5%). In 144 (73.4%) cases the Breslow thickness was >1 mm. Metastasis was found in 28.6% of invasive melanomas and nodular melanoma, Clark IV/ V, Breslow > 1 mm, mitotic index > 6 and ulcerated lesions were more likely to metastasize.CONCLUSION:Most melanomas presented Breslow> 1mm. The main factors associated with metastasis were nodular type, Clark IV / V, Breslow> 1mm, mitotic index > 6 and ulcerated lesions.

Oligosaccharides generated by heparanase induce tumor progression. Basal cell carcinoma and squamous cell carcinoma comprise types of nonmelanoma skin cancer. Objectives: Evaluate the glycosaminoglycans profile and expression of heparanase in two human cell lines established in culture, immortalized skin keratinocyte (HaCaT) and squamous cell carcinoma (A431) and also investigate the expression of heparanase in basal cell carcinoma, squamous cell carcinoma and eyelid skin of individuals not affected by the disease (control). Methods: Glycosaminoglycans were quantified by electrophoresis and indirect ELISA method. The heparanase expression was analyzed by quantitative RT-PCR (qRTPCR). Results: The A431 strain showed significant increase in the sulfated glycosaminoglycans, increased heparanase expression and decreased hyaluronic acid, comparing to the HaCaT lineage. The mRNA expression of heparanase was significantly higher in Basal cell carcinoma and squamous cell carcinoma compared with control skin samples. It was also observed increased heparanase expression in squamous cell carcinoma compared to the Basal cell carcinoma. Conclusion: The glycosaminoglycans profile, as well as heparanase expression are different between HaCaT and A431 cell lines. The increased expression of heparanase in Basal cell carcinoma and squamous cell carcinoma suggests that this enzyme could be a marker for the diagnosis of such types of non-melanoma cancers, and may be useful as a target molecule for future alternative treatment.

METHODS: Hospital-based case-control study with individuals residing in Taubaté, treated between January 2005 and December 2006. The subjects were matched 1:1 according to gender and age. Age, gender, phototype, European descent, time of residence, sun exposure, time (in years), number of hours and time of occupational and non-occupational sun exposure, photoprotection, family history and photodamage were independent variables. A hierarchical logistic regression was used at three levels. The model adjustment was performed using the Hosmer-Lemeshow test and its accuracy was verified by ROC curve. The significance level was p < 0.05. RESULTS: There were 132 cases and 132 controls. Phototype with OR = 3.14 (95% CI 1.79-5.49), hours of occupational 1.76 (1.04-2.99) and non-occupational sun exposure 1.80 (0.98-3.29) and family history 2.10 (1.13-3.93) were the variables in the final model. Hosmer-Lemeshow test, p = 0.97. Accuracy 70% (95% CI 63-76). CONCLUSION: We concluded that fair skin, family history and occupational and non-occupational sun exposure were associated with non-melanoma skin cancer in Taubaté-São Paulo.

Currently, the treatment of actinic keratosis addresses not only the destruction of individual lesions (using methods such as liquid nitrogen cryosurgery, electrocoagulation, trichloroacetic acid, and shaving), but also the treatment of field cancerization (using drugs such as 5-fluorouracil, imiquimod, ingenol mebutate, oral and topical retinoids, chemical peelings, and photodynamic therapy). Most studies involving retinoids have as purpose and primary outcome the prevention of nonmelanoma skin cancer. Therefore, few data are available about their use for the treatment of actinic keratosis. Oral and topical retinoids can be used for prevention and treatment of multiple actinic keratoses and field cancerization, especially in association with sunscreen and liquid nitrogen cryosurgery. Objectives: The primary objective of this research was to evaluate the clinical, histopathological, and immunohistochemical effects on the epidermis and dermis of the studied drugs for the treatment of multiple actinic keratoses of the face and forearms in immunocompetent patients. The secondary objectives were: 1) to evaluate the tolerability and safety of the medications; 2) to demonstrate the impact of the treatments on quality of life; 3) to investigate the reproducibility of lesion count as a parameter of efficacy; 4) to verify the role of cryosurgey associated to daily use of sunscreen in the control of actinic keratosis. Methods: This is an open, randomized, propective, and comparative clinical trial, including 61 participants, from 50 to 75 years of age, from both genders, with 5 to 60 visible and/or palpable actinic keratoses on the face and forearms. At the beginning (T0) and after 120 days (T120), all participants underwent liquid nitrogen cryosurgery to treat their actinic keratoses. Posteriorly, they were randomized in two treatment groups: 30 took oral isotretinoin, at the dose of 10 mg/day (ISO group), and 31 used 0.05% topical tretinoin cream every other night (TRE group), for 6 months (T300). All the subjects used SPF 60 sunscreen during the whole study period. At the beginning of the randomization (T120) and at the end of the study (T300), two biopsies were performed in the left forearm. The efficacy was evaluated using clinical (number of actinic keratoses) and histopathological parameters (hematoxilin-eosin and Weigert-Van Gieson staining, the latter for elastic fibers), as well as immunohistochemical markers of carcinogenesis (proteins p53, Bcl-2, and Bax) before and after the treatments. The safety evaluation included the report and observation of side effects and biochemical laboratory tests for the participants of the ISO group. Results: A decrease in the number of actinic keratoses related to liquid nitrogen cryosurgery and the use of SPF 60 sunscreen was observed between T0 and T120 (-39%) and to the treatments for field cancerization with oral isotretinoin (-33%) and topical tretinoin (-23%), cryosurgery and SPF 60 sunscreen between T120 and T300. The histopathological and immunohistochemical findings demonstrated: thinning of the stratum corneum and reduction of p53 and Bax expression between T120 and T300. Thickening of the basal to the granular layer and increased expression of Bcl-2 protein were observed in both groups. No modifications were found in the elastic tissue. In general, the treatment groups did not differ (p > 0.1). Only slight side effects were observed in both groups, except severe xerophthalmia in one patient of the ISO group. Between T120 and T180, the levels of total cholesterol and triglycerides increased in 76% and 61% of the participants, respectively, and this increase was around 30% of the initial value, returning to normal in T300 without the need to change or interrupt the treatment. Conclusion: Both low-dose oral isotretinoin and 0.05% topical tretinoin cream improved clinical, histopathological, and immunohistochemical parameters in patients with multiple actinic keratoses and field cancerization, without differences between them. The therapeutic choice of retinoids for field cancerization should take into consideration the patient?s profile, particularly in relation to the risk of side effects.

The purpose was to identify these workers knowledge about skin cancer and describe protection/prevention measures adopted for them. Descriptive and qualitative nature research, developed with 50 workers of a great load building site firm at municipal district of Rio de Janeiro, Brazil, which the data was collected through a form. From descriptive and statistical analysis, the results showed that the protection measures adopted for the workers to skin cancer prevention are insufficient and the way that they are used are not lined with the literature. Important changes needed at daily of these workers are related to life styles and at access ease to informations and knowledges about skin cancer. Thus, nursing's interventions along the Worker's Health stuff can contribute to preventive's implementation of occupational cancer.

Polypoid melanoma is considered an exophytic variant of the nodular subtype. The incidence of polypoid melanoma is extremely variable, most likely because of the different criteria used for its characterization. We presented a rare case of polypoid melanoma and superficial spreading melanoma in the same lesion.

Assessing quality of life and self-esteem outcomes in skin cancer patients is important because these are indicators of the results of the treatment, translating how patients face their lives and their personal relationships.OBJECTIVE:To assess the late impact of the surgical treatment of head and/or neck skin carcinomas on quality of life and self-esteem of the patients.METHODS:Fifty patients with head or neck skin carcinomas were enrolled. Their age ranged between 30 and 75 years, 27 were men and 23 were women. Patients were assessed with regard to quality of life and self-esteem, preoperatively and five years postoperatively. Validated instruments were used: the MOS 36-item Short-form Health Survey (SF-36) and the Rosenberg Self-esteem/EPM-UNIFESP Scale. The Wilcoxon signed-rank test was used for the statistical analysis.RESULTS:Twenty-two patients completed the five-year follow-up, 54.5% women and 45.5% men. Compared to the preoperative assessment, patients had an improvement in mental health (p=0.011) and in self-esteem (p=0.002). There was no statistical difference with regard to the other domains of the SF-36.CONCLUSION:Patients submitted to surgical treatment of skin carcinoma improved mental health and self-esteem in the late postsurgical testing.

OBJECTIVES: 1. To report our experience in surgical treatment of multiple periorbital syringomas excised by Castroviejo scissors followed by healing by secondary intention. 2. To present a literature review with emphasis on therapeutic aspects. MATERIAL AND METHOD: During 68 months we treated 38 cases of periorbital syringoma. All of them were submitted to surgical excision using Castroviejo scissors, followed by healing by secondary intention. Evaluation was done 7, 30 and 90 days after surgery. The mean follow-up period was 33 months, ranging from 3 to 62 months. RESULTS: The results were excellent in 24 patients (63.1%), good in 12 patients (31.6%) and regular in two patients (5.3%). The only immediate complication observed was edema; late complications were hypochromia (12 patients), depressed scar (one patient) and hypertrophic scar (one patient). Recurrences were not observed during the follow-up period. CONCLUSIONS: We concluded from our results that excision of periorbital syringomas by Castroviejo scissors followed by healing by secondary intention is a low cost, straightforward outpatient procedure. Results are good, with no recurrences, although transient hypochromia may occur.
- ItemSomente MetadadadosSquamous cell carcinoma arising in a multiple verrucous epidermal nevus(Soc Brasileira Dermatologia, 2016) Yarak, Samira [UNIFESP]; Machado, Taila Yuri Siqueira [UNIFESP]; Ogawa, Marilia Marufuji [UNIFESP]; Almeida, Mirian Luzia da Silva; Enokihara, Mílvia Maria Simões e Silva [UNIFESP]; Porro, Adriana Maria [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Verrucous epidermal nevi are hamartomatous lesions of the epidermis that, unlike other epidermal nevi (such as sebaceous nevus or nevus comedonicus), are rarely associated with malignant neoplasms. The majority of squamous cell carcinoma develop in linear or multiple epidermal nevus and rarely in solitary epidermal nevus. In general, the prognosis is favorable. We report a case of well-differentiated invasive squamous cell carcinoma arising from a multiple verrucous epidermal nevus. Although there is no consensus on prophylactic removal of epidermal nevus, its removal and biopsy should be considered if changes occur.

METHODS: we included seventy-one patients recorded their demographic and operation data. For analysis of direct costs we considered the period of patient hospitalization, including human and material costs. RESULTS: The average equipment cost per procedure was R$ 324.70, and the mean cost of hospital service, according to the table of SUS, was R$ 193.66. Thus, we obtained an average total cost of R$ 518.36 per procedure. However, the average amount refunded by the SUS per hospital procedure was R$ 429.19. CONCLUSION: Surgical treatment of cutaneous carcinomas generated for the hospital an average deficit of R$ 89.16 per procedure.

There are few cases reported in the literature, and we could only identify one description of trichoadenoma occurring in the eyelid area. We describe the case of a patient with trichoadenoma in the outer corner of the lower eyelid that we treated with surgical excision associated with blepharoplasty.
