- ItemAcesso aberto (Open Access)Atenção às mulheres no período do climatério: a construção de diálogos com profissionais de saúde no município de Santos, São Paulo(Universidade Federal de São Paulo (UNIFESP), 2019-09-23) Luz, Milene Mori Ferreira [UNIFESP]; Frutuoso, Maria Fernanda Petroli [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Women's health has historically been one of the priority focuses of the health sector, especially with regard to actions in the reproductive period, with little emphasis on the climacteric, the transformation period, from the reproductive to the non-reproductive phase, which may cause various symptoms affecting women’s every day. With population aging and increased life expectancy, new approaches to care, disease prevention and climacteric women’s health promotion are needed. This study aimed to know the professionals’ practices and discuss the assistance provided to women in climacteric period in health primary care in Santos. This is an intervention research with teams of the Family Health Strategy (FHS) and the Extended Family Health Center (NASF), with nine professional categories. The data were produced from workshops encouraging the active participation of the subjects. They were recorded, transcribed and analyzed by content analysis. Professionals point out there are no actions focused on this population and that the complaints of women who are in the climacteric period are not perceived or are minimized by the lack of knowledge and reflection on the issue by the team. The FHS actions are still directed to the priority groups (hypertensive and diabetic patients, pregnant women) and some women's complaints are understood as a problem related to mental health (depression) giving invisibility to climacteric women. The workers recognize the difficulty for women to experience the climacteric syndrome with the actions offered by the services, which are not prepared for the approach in the climacteric period and the conditions of vulnerability of women in the studied region. The NASF team can contribute to multidisciplinary actions, which qualify care for women throughout the life cycle. The actions of permanent education in health, in the daily work of the FHS and NASF teams can extend the professionals' understanding of the women care, as well as rethink professional practices in the unique contexts of the territories. The data production from this research may support the construction of public policies in the area of Women's Health in the city of Santos aiming at comprehensive health care and humanized assistance to women in the climacteric period.

Since the last century, due to the aging of the population, there has been a growing institutionalization of such practice in home care services, which had assumed very different models and work processes in different countries. The main objective of this research was to know the process of care in the Home Care Service (SAD) of a medium-sized municipality in the State of São Paulo as a component of the Emergency and Urgency Network (RUE). Method: It is a case in which I take on a cartographic ethos in every way of the research, participating in the routine of SAD and recording in a field diary the daily scenes. I also have recorded interviews with caregivers and a hospital nurse, and I have conducted a focus group with Primary Care nurses (AB). As a data analysis strategy, the first approximation of the empirical material was the definition of Cutting Plans, that is, intentional cuts made from elements or components of home care policy. In the second analytical approach, there are the Visibility Plans, that are scenes of the daily life that reveal the complex relationships between the prescribed work (that the official policy defines) and the actual work (that is how care is performed by the teams in their concrete conditions of work). Results and Discussion: Through the cutting plans and visibility plans, it was possible to identify problems related to management, in particular those that establish limits for the team work; limitations in the use of the information system; the positive aspects and also the tensions present in the multiprofessional work; the conflicts present in the daily work; deficiencies in the training of workers; the isolation of SAD in relation to other health equipment; the limitations of AB to effectively support SAD; the overload of the caregiver and the inescapable need of strategies to support their work and elements of the patient universe, marked by the loss of autonomy and often by the lack of perspective of future building, showing human vulnerability in different perspectives and the sense that each family gives to the suffering experienced by the dependent family member of long-term care. Conclusions: The study evidenced that home care is a model of innovative care, if assumed by the health system with sufficient human resources, materials, equipment, transportation and agile and flexible information system to include real information. Caregivers suggested that care must to be integrated among professionals and with other points of the Health Care Network (RAS). There are difficulties for AB to assume this model of care, and the experience indicated the need of social support services to contribute to the quality of life of the caregiver.

A pesquisa teve como inspiração as abordagens teórico-metodológicas da pesquisa-intervenção, da Análise Institucional e da pesquisaformação. Participaram seis gestoras e gestores, vinculados à gestão da Educação Permanente em Saúde na Baixada Santista e recorremos aos diários e as narrativas como dispositivos de intervenção e de envolvimento das pessoas na proposta. Foram dois tipos de diários: diário de pesquisa e diários institucionais. O diário de pesquisa foi produzido pela pesquisadora durante todo o percurso, com os acontecimentos, as reflexões e os sentimentos produzidos no processo e também, por meio da escrita, pôde ir reconhecendo/analisando suas implicações. Os diários institucionais foram feitos pelas e pelos participantes, escritos e/ou falados, compartilhando experiências relacionadas ao trabalho durante o período em que estiveram envolvidas e envolvidos com a pesquisa. Os dispositivos suscitaram exercícios de autoanálise, avaliação e significação do vivido. Ao acolher nossas experiências nos afetamos, em pensamento, em sentimento, em ação, criamos rede. Tendo como referência a análise de implicação e analisador, conceitos–ferramenta da Análise Institucional, a produção desta pesquisa reforça a necessidade de criar espaços/situações em que a gestão possa ser problematizada e as práticas e os movimentos institucionais existentes sejam observados e analisados, de produzir – constantemente – Educação Permanente em Saúde para a própria gestão, na busca por desnaturalizar e desacomodar os fazeres para gerar movimentos, deslocamentos e transformações.
- ItemAcesso aberto (Open Access)Influência dos modelos inovadores paulistas na Política Brasileira de Saúde Mental(Assoc Brasileira Enfermagem, 2016) Vieira, Marcia do Nascimento [UNIFESP]; Marcolan, Joao Fernando [UNIFESP]Objective: to analyze the influence of two Sao Paulo State experiences in the choice of psychosocial care centers as guiding service providers for the national mental health policy. Method: qualitative, exploratory, and descriptive study using oral history as a methodology and theoretical reference. Results: eight professionals involved in the deployment of the mental health policy in the cities of Santos and Sao Paulo between the years 1989 and 1992 were interviewed. Data were analyzed after treating the narratives and grouping the most significant content. Two central themes emerged: development of the local-regional model, and the model's influence on the choice of the psychosocial care center. Conclusion: due to greater insertion of the Santos experience group in the spheres of the federal government, its influence on the choice of the substitute model was higher in ideological terms, whereas the Sao Paulo model had its influence restricted for political reasons.
- ItemAcesso aberto (Open Access)Problematizando As Práticas Profissionais De Uma Unidade De Saúde Da Família: O Encontro Dos Trabalhadores(Universidade Federal de São Paulo (UNIFESP), 2017-11-17) Oliveira, Ludmila Sterci de [UNIFESP]; Uchôa-Figueiredo, Lúcia da Rocha [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Abstract: The main objective of this research was to problematize the professional practices of a Family Health Unit located in the municipality of São Bernardo do Campo. And the specific objectives were to provide encounters and speech possibilities, in which professionals could analyze their implications regarding the work performed in this Family Health Unit. It was a qualitative field research carried out and developed according to the theoretical-methodological framework of Institutional Analysis. The participants were sixteen professionals of this service. For the production of the data, there were conversational wheels among the participants and for the analysis the concept-tool of the analysis of the implications and the micropolitics of work and health care were used. The results have showed that the work is burdened by the affection and the caring in health, is conducted by the subjectivity from the participants involved. It has been clear that the absorption of the daily events at the Family and Health Strategy, is pervaded with complexity and life, and the professionals need and wish to participate in a creative enviroment altogether with discussions which permit the analysis of the implications and reflections concerning upon life and the health care work universe.
