A cirurgia é a etapa crucial do tratamento.

Methods: Cross sectional study with infants in the first year of life, born from February/2007 to December/2008, with gestational age <37 weeks, birth weight <2000g and followed at the Premature Clinic of the Federal University of São Paulo. Major malformation, intraventricular hemorrhage grade III/IV and/or periventricular leucomalacia were excluded. Physical exam was used as a reference standard to determine the frequency and factors associated with thoracic alterations and for accuracy analysis, by ROC curve. Photographs were analyzed by a computer program to detect shoulder elevation and thoracic retraction by the measurement of manubrium/acromion process/trapezius angle and the greatest thoracic retraction depth. The study was approved by the Ethical Committee of the Institution and parents/guardians signed terms of informed consent. Results: Of 121 studied infants (gestational age: 31.1+2.8 weeks, birth weight: 1400+338g), 81 (66.9%) presented thoracic alterations. By logistic regression, factors associated with thoracic alterations were: days of corrected age (OR=1.018, CI95%: 1.004-1.019) and days on oxygen therapy (OR=1.096, CI 95%: 1.031-1.164). Compared to physical exam, the manubrium/acromion process/trapezius angle presented: accuracy: 0.793, sensitivity: 71.4%, specificity: 78.4%, intraclass and interclass correlation coefficient (ICC): 0.922 and 0.947. The greatest thoracic retraction depth showed: accuracy: 0.895, sensitivity: 96.6%, specificity: 75.9%, (ICC): 0.841 and 0.838. Conclusions: The frequency of thoracic alterations was high and associated with higher corrected age and longer oxygen therapy. The photogrammetry provided an objective, accurate and reliable test to detect thoracic alterations in preterm infants.
- ItemSomente MetadadadosAngry posture(Elsevier Science Bv, 2016) Rosario, Jose Luis [UNIFESP]; Bezerra Diogenes, Maria Suely [UNIFESP]; Mattei, Rita [UNIFESP]; Leite, Jose Roberto [UNIFESP]Postural abnormalities can affect the emotions and vice-versa. The aim of the present study was to investigate the existence of a relationship between subjective anger and body posture in 28 women, aged between 20 and 39 years, with a normal body mass index (or underweight) and an absence of neurological, psychiatric or musculoskeletal disorders. The postural parameters photographed were the inclination of the shoulders, protrusion of the head, hyperextension of the knees and shoulder elevation. The degree of anger was rated by analogue scales representing current and usual anger. The results indicated that a relationship exists between current anger and the inclination of the shoulders (p = 0.03), protrusion of the head (p = 0.05) and hyperextension of the knees (p = 0.05). Correlations were found between usual anger, shoulder elevation (p = 0.05) and hyperextension of the knees (p = 0.05). In conclusion, posture is associated with emotions, and usual anger can lead to shoulder protraction. (C) 2016 Elsevier Ltd. All rights reserved.

- ItemAcesso aberto (Open Access)Autoavaliação da postura por idosos com e sem hipercifose torácica(ABRASCO - Associação Brasileira de Saúde Coletiva, 2012-03-01) Gasparotto, Lívia Pimenta Renó [UNIFESP]; Reis, Camila Costa Ibiapina [UNIFESP]; Ramos, Luiz Roberto [UNIFESP]; Santos, José Francisco Quirino dos [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)This article lists the differences between self-perception of body posture among the elderly suffering from postural alterations or not, in order to ascertain whether self-evaluation of posture can lead to preventive measures. Eighteen cases from the elderly population participated in the EPIDOSO project at UNIFESP and were subjected to postural evaluation. Postures were photographed and copies given to the participants and their subsequent comments were analyzed by the qualitative method. The narratives were taped and cataloguedusingthe technique of theoretical axial and selective coding from the perspective of symbolic interactionism. A passive attitude was identified among the elderly in relation to postural alterations. There is a distortion of body image by those with postural deviation. Participants with adequate spinal alignment were more conscious about body posture and the importance of this being assimilated in the phases prior to aging. The adoption of postural self-care seems to occur in the earlier stages of aging and preventive measures should be implemented at this stage. Lack of concern about posture is linked to the concept of the elderly regarding the notion that aging is, in itself, the accumulation of inevitably simultaneous or successive dysfunctions.
- ItemAcesso aberto (Open Access)Avaliação clínica da relação entre postura, respiração e deglutição em paciente pós-acidente vascular cerebral na fase crônica: relato de caso(CEFAC Saúde e Educação, 2013-10-01) Simão, Sináira Santos Seixas; Romero, Vivian Urbanejo; Baraldil, Karen; Oda, Adriana Leico; Viana, Celiana Figueiredo [UNIFESP]; Chiappetta, Ana Lúcia de Magalhães Leal [UNIFESP]; Pieri, Alexandre [UNIFESP]; Universidade de São Paulo (USP); Centro Universitário São Camilo; Universidade Nove de Julho; Universidade Federal de São Paulo (UNIFESP)The Stroke is a neurological disorder that disables more adults. The breathing, posture and swallowing have a direct relationship in this disease, thus requiring a multidisciplinary approach. The goal of this study was to conduct a clinical evaluation integrated to analyze the relationship between these disorders. This case is a female patient aged 37 with six years of diagnosis of ischemic stroke with right hemiparesis complete predominantly crural, hypertension, dysphagia and dysarthrophonia. In history, reported symptoms related to dyspnea, pain in the lumbar spine, the presence of coughing and choking and drooling to food, especially during the speech. The patient has scoliosis, thoracic kyphosis and abdominal weakness. The evaluation showed a respiratory expiratory force below the expected range. The peak cough flow is lower than expected, demonstrating not have the strength to cough. The research revealed changes in speech increased time to eat, need to drink to help swallow solid food before any leaks, the need for multiple swallows due to the sense of stasis in laryngeal-pharyngeal cavity and decreased taste and thick and viscous saliva. Using physical and speech therapy evaluations, we concluded that a patient with stroke in chronic respiratory presents alterations that may prevent it from effectively protecting the lower airways, which, added to postural changes and swallowing show the importance of integrated assessment for future therapeutic interventions more effective.

METHODS: Two groups were evaluated: 16 post-mastectomy women with lymphedema of the upper limb and 14 post-mastectomy women without lymphedema. Patients were submitted to analysis made by software, one for posture and the other to measure ranges of movement of the shoulder, elbow, and wrists. The results obtained were compared between the right and left sides, and operated and non-operated sides, and then were submitted to statistical tests. RESULTS: Both groups presented with anteriorization of the trunk. The women with lymphedema had head rotation to the right, protrusion of the left shoulder, and trunk inclination angle smaller on the operated side, besides bilateral elevation of the scapula when compared to the group with no lymphedema. Changes in range of motion were also smaller on the operated side in terms of flexion, abduction, and external rotation of the shoulder for all women, and for those with lymphedema, elbow extension and wrist flexion had a smaller range of motion. CONCLUSION: Women submitted to mastectomy presented with asymmetries and modifications in posture, and lymphedema seemed to worsen this condition. Additionally, they had deficits in range of motion in the shoulders on the operated side. Women with lymphedema also showed deficits in the elbows and wrist.
- ItemSomente MetadadadosAvaliação do controle de tronco, equilíbrio e funcionalidade em pacientes com Distrofia Muscular de Duchenne(Universidade Federal de São Paulo (UNIFESP), 2019-04-29) Santos, Ana Lucia Yaeko da Silva [UNIFESP]; Sá, Cristina dos Santos Cardoso de [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: The main consequence of Duchenne muscular dystrophy (DMD) is progressive muscular weakness, which affects the trunk control and upper limb motor function (UULL) of patients. Accurate assessment tools are needed to determine the best therapeutic approach and monitor its effectiveness. However, there are few studies to verify trunk control in relation to the UULL function of ambulatory and non-ambulatory patients. Objective: To verify trunk control and UULL motor function in patients with ambulatory, non-ambulatory DMD and children without dystrophinopathy. Methods: Cross-sectional study. The sample was selected according to a sample calculation, composed of children without dystrophinopathy and patients diagnosed with DMD, divided into ambulators (Vignos ≤ 6) and non ambulators (Vignos ≥ 7) from the Vignos score, which evaluates the staging of the disease. Subsequently the volunteers were evaluated by the following functional scales: (1) Segment Assessment Trunk Control (SATCo-Br), trunk control, (2) Performance of Upper Limb (PUL) and (3) Jebsen Taylor Test (TJT), function of the UULL. The data was based on the SPSS program, version 22 or R, version 3.5.2. To compare numerical and categorical variables, the Kruskal Wallis test was applied, followed by the Dunn test, the nominal variables were analyzed by the Qui Quadrado test. Results: The sample was made by 88 volunteers, 30 non ambulatory group, 29 ambulatory and 29 in the reference group. The difference between the groups in relation to scoliosis, pain, trunk control, function od the UULL, by PUL scale, however there was no difference between the ambulatory and the reference group in all the TJT subtests, and the writing subtest was no different between groups. Conclusion: The trunk control and function of the UULL, by PUL scale, are the best in the reference group, followed by the ambulatory group, and worse in the non ambulatory group. The function of the UULL, by TJT, is different between ambulatory and non ambulatory, except in subtest of writing. The SATCo, PUL and TJT have been able to characterize patients ambulatory and non ambulatory, which can be used by clinical trials to test the efficacy of drugs, genetic therapies and other therapeutic approaches.

- ItemAcesso aberto (Open Access)O efeito do esforço na estabilidade postural em jovens jogadores de futebol(Sociedade Brasileira de Ortopedia e Traumatologia, 2013-06-01) Arliani, Gustavo Gonçalves [UNIFESP]; Almeida, Gabriel Peixoto Leão [UNIFESP]; Santos, Ciro Veronese dos; Venturini, André Manrique [UNIFESP]; Astur, Diego da Costa [UNIFESP]; Cohen, Moises [UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Instituto Cohen de Ortopedia, Reabilitação e Medicina do EsporteOBJECTIVE: Analyze the effects of physical exertion during a soccer match on the functional capacity and stability of the lower limbs of young soccer players. METHODS: We analyzed 10 soccer players who underwent functional capacity assessment of the lower limbs by a Hop Test protocol and evaluation of the level of postural stability in the Biodex Stability System (Biodex, Inc., Shirley, NY) before and immediately after a friendly game lasting 45 minutes. RESULTS: After the match, there was a decrease in overall stability index (F (1,23) = 7.29 P = .024) and anterior posterior index (APSI) (F(1,23) = 5.53 P = .043). Fatigue in the dominant limb was responsible for the significant deficit in OSI (F(1,23) = 3.16, P = .047) and APSI (F(1,23) = 3:49, P = .029), while the non-dominant limb did not cause any change in the pre and post-game. CONCLUSION: A football match can cause decreased stability and functional capacity of the lower limbs in young players. Level of evidence iii, pre-test and post-test study (case-control).

Objectives: This study aimed to analyze the effects of water aerobics on static body posture, QOL, functional capacity, pain level, and risk of falls of elderly people. Methods: Two groups were formed: the intervention group (IG), n=49, mean age of 67.2 ±7.58; and the control group (CG), n=34, mean age of 67.65 ±7.43. At first, the elderly of both groups underwent static body posture assessment using the symmetry and plumb line, as well as photographs in anterior, posterior, and lateral views. The SF-36 questionnaire was applied in QOL evaluation. To analyze functional capacity, three basic questions were used, related to walking 100 meters, getting into and out of bed, and showering. The pain level was analyzed through a pain visual analogue scale, and the risk of falls through the Timed Up and Go test. After the evaluations, the IG elders performed water aerobics classes, twice a week for three months. While the CG elders did not perform water aerobics. After the three months of study, the elderly underwent a new evaluation. In the statistical analysis, the Wilcoxon test and the Wilcoxon-Mann-Whitney test were used. Results: In relation to the static body posture, a significant improvement in the IG was observed when compared to the elderly in the CG, highlighting the segments: head (anterior and lateral view), shoulders (lateral view), and knees (lateral view). As for the QOL, a significant improvement in the IG was verified in all analyzed variables. Regarding functional capacity, no significant results were found (p>0,05). In the pain level, the IG showed a 63.3% increase in mild pain and a 30,6% reduction in moderate pain (p<0,05). About the risk of falls, the IG presented a 19% increase in the low risk for falls and a 18% reduction in the average risk for falls (p<0,05). Conclusion: In relation to the studied sample, it can be concluded that water aerobics contributed effectively and positively to improve the static body posture, QOL, and risk of falls in the elderly, as well as to reduce the pain level. The functional capacity variable demonstrated no significant improvement (Ramos et al. 2013). Given the results, it is recommended the regular practice of water aerobics by the elderly, aiming to prevent and treat the deleterious effects arising from the aging process.

However, there are not many studies on its consequences. The aim of this work was to search for equilibrium alterations in young women used to daily wearing high-heeled shoes. Thirty female undergraduates (mean aged 20 years old) were asked to write down, for two weeks, the number of hours per day they remained with the shoes on. They were then assessed as to posture deviations, as to muscle retractions of the posterior chain by means of the sit and reach test, and as to equilibrium by the Romberg test. Results were statistically analysed. Mean volunteers' height was 1.63 m and mean body mass, 58.5 kg. A negative, strong correlation (r=-0.8692; p<0.0001) was noticed between reach range and weekly time (in hours) of high-heeled shoe wearing. No correlation was found between reach range and time of high-heel use in years. Seventeen out of the 30 volunteers presented positive test for loss of equilibrium when barefoot with closed eyes. A weak, negative correlation (r=-0.4178; p=0.0216) was found between loss of equilibrium and weekly time of high-heel use. A positive moderate correlation was found between reach range and time of equilibrium (r=0.6078; p<0.0004). The use of high-heeled shoes for more than four hours per day can hence be said to cause equilibrium changes and muscle retraction in young women.

Methods: We evaluated 11 children between 2 and 7 years of age of both sexes, with the visually impaired, who had nystagmus and head lock position. The test Lea Grating Acuity Test® was used to collect measurements of visual acuity. This applied on two occasions: with and without postural alignment of the head. For reliability of the postural alignment of the head, the slopes were measured by Fisiologic® software. Results: The children had a poorer performance after physiological postural alignment. This poor performance is possible due to loss of position lock nystagmus to gain postural alignment, said to be ideal. Postural compensations were observed, and sharply increased eyestrain. Conclusion: The pursuit of traditional postural alignment affect the visual response of children with visual impairments.
