METHODS: Forty- two patients with unilateral hip osteoarthritis were evaluated by means of simple radiography and computed tomography. RESULTS: It was observed that there was no significant variation in femoral anteversion between the diseased and healthy hips. There was strongest agreement between observers 1 and 2 in relation to both the diseased hips (cases) and the healthy hips (controls). Moreover, no significant agreement was found between observers 1 and 3 (p = 0.13) and between observers 2 and 3 (p = 0.12), in relation to the neck-shaft angle of the control hips. CONCLUSION: Although there was no relationship between femoral anteversion and the neck-shaft angle in the patients with unilateral hip osteoarthritis, the present study showed that there was also no relationship with these angular deviations.
- ItemAcesso aberto (Open Access)Artroscopia do quadril na epifisiólise grave(Sociedade Brasileira de Ortopedia e Traumatologia, 2010-12-01) Dobashi, Eiffel Tsuyoshi [UNIFESP]; Blumetti, Francesco Camara [UNIFESP]; Pinto, Jose Antonio [UNIFESP]; Milani, Carlo [UNIFESP]; Ishida, Akira [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)We present a case report of a 12-year-old boy diagnosed with slipped capital femoral epiphysis grade III, with an acute-on-chronic presentation, associated with severe functional impairment and significant reduction in hip range of motion. The patient underwent a Dunn-type trapezoidal wedge femoral neck subtraction osteotomy by hip arthroscopy, followed by closed reduction and fixation with a 6.5mm percutaneous screw. There was significant improvement of the clinical picture on the first postoperative day, allowing for early rehabilitation. After a brief review of the literature, the authors propose this novel surgical technique as a viable method to treat severe slipped capital femoral epiphysis, encouraging the development of new studies on the subject.

[UNIFESP]; Benedito-Silva, Ana A.; Carderelli Minozzo, Fabio; Vancini, Rodrigo L.; Barbosa De Liras, Claudio A.BACKGROUND: The aim of this study was to evaluate test-retest reliability of concentric flexor, extensor, abductor and adductor muscular isokinetic hip torques in female soccer players. METHODS: Sixteen highly-trained female soccer players were evaluated. Isokinetic dynamometer assessment was performed at 30 degrees/s and 150 /s concentrically. The muscles tested were hip flexor (Fl), extensor (Ext), adductor (Add) and abductor (Abd). The reproducibility of the measured peak torque (PT) was analyzed by using the intraclass correlation coefficient (ICC). The difference in PT between the first and second tests was tested using Student's t-test. RESULTS: The ICC for the observed PT values revealed moderate to high reproducibility (ranging from 0.55 to 0.76) for the hip Fl and Ext measurements at 150 degrees/s and for Add and Abd measurements at 30 and 150 /s. For the hip Fl and Ext measurements at 30 degrees/s the ICC was poor. CONCLUSIONS: The isokinetic assessment of the concentric PT values generated by the hip Fl and Ext and Add and Abd is moderate to highly reproducible, when assessed at the highest test velocity (150 degrees/s). The test-retest reliability of hip isokinetic strength measures seems to be affected by the type muscle and test velocity.

MATERIALS AND METHODS: The present retrospective study included 92 patients with JIA. The disease subtypes, age at disease onset, length of disease progression, disease activity, and clinical manifestations of the hip pathology were assessed. US examinations were routinely performed, and the images were analysed by two ultrasonographers who were blinded to the patients' clinical conditions. RESULTS: Of the 92 patients included in the study, 69.6% were girls, and the average age was 12.4 ± 5.1 years. Thirty-three (35.9%) participants exhibited the persistent oligoarticular subtype, and 30 (32.6%) exhibited the rheumatoid factor (RF)-negative polyarticular subtype. Forty-four participants exhibited signs and/or symptoms of hip pathology. Twentynine (31.5%) participants exhibited abnormal US findings, and 34.4% exhibited subclinical synovitis. The US alterations exhibited an association with subclinical synovitis in 34.4% of the cases. The US alterations bore a correlation with the presence of hip-related signs and/ or symptoms (P = 0.021), particularly joint limitations (P = 0.006), but were not correlated with the disease activity (P = 0.948) or subtype (P = 0.108). Clinical synovitis was associated with polyarticular involvement (P = 0.002) and disease activity (P = 0.017). Subclinical synovitis was not correlated with the investigated variables. CONCLUSION: Clinical affection of the hip in JIA, particularly joint limitation, is associated with synovitis (revealed by US assessment) independently of the activity and subtype of the disease. Therefore, healthcare professionals should consider the possible occurrence of silent disease and subclinical synovitis, which might contribute to hip deterioration.

METHODS: Cross-sectional study where 9 patients were evaluated with unilateral Girdlestone's and 3 with Girdlestone's in one hip and contralateral total hip prosthesis. The evaluation consisted in filling in a generic questionnaire on quality of life SF-36 and a specific questionnaire for hip function Harris Hip Score (HHS). The comparison between groups was made by using the Student's t-test and the Fisher's test. RESULTS: The patients of the unilateral Girdlestone's group presented a higher number of SF-36 domains classified as high, although 77.8% of these showed poor results on the HHS. All patients had a leg-length discrepancy and positive Trendelenburg's test, which led to limping gait in 11 of 12 patients evaluated. Of these, only 6 underwent physiotherapy after surgery. CONCLUSION: Girdlestone's postoperative quality of life and function in a Brazilian population still requires further studies, because these outcomes are indicative of study variables' behavior and cannot be regarded as definite.

Idiopathic chondrolysis of the hip has an insidious beginning and affects more often female adolescents. Patients report severe hip pain, mobility limitation, and even claudication. This study aimed at reporting one case of that rare disease: an 11-year-old female adolescent with chondrolysis, followed up for three years. Inflammatory activity tests were normal. Imaging tests (radiography, ultrasonography and magnetic resonance) were essential for the diagnosis. The treatment was based on pain control and preservation of the joint mobility, and included low-impact physical activity, non-steroidal anti-inflammatory drugs, and disease-modifying antirheumatic drugs, with good response after 12 months of treatment. Surgery was not necessary.

Little information is available on the bilateral symmetry of hip rotational measures in unilaterally dominant upper extremity athletes.Hypothesis: Side-to-side differences in hip joint range of motion may be present because of asymmetrical loading in the lower extremities of elite tennis players and professional baseball pitchers.Study Design: Cohort (cross-sectional) study (prevalence); Level of evidence, 1.Methods: Descriptive measures of hip internal and external rotation active range of motion were taken in the prone position of 64 male and 83 female elite tennis players and 101 male professional baseball pitchers using digital photos and computerized angle calculation software. Bilateral differences in active range of motion between the dominant and nondominant hip were compared using paired t tests and Bonferroni correction for hip internal, external, and total rotation range of motion. A Pearson correlation test was used to test the relationship between years of competition and hip rotation active range of motion.Results: No significant bilateral difference (P >.005) was measured for mean hip internal or external rotation for the elite tennis players or the professional baseball pitchers. An analysis of the number of subjects in each group with a bilateral difference in hip rotation greater than 10 degrees identified 17% of the professional baseball pitchers with internal rotation differences and 42% with external rotation differences. Differences in the elite male tennis players occurred in only 15% of the players for internal rotation and 9% in external rotation. Female subjects had differences in 8% and 12% of the players for internal and external rotation, respectively. Statistical differences were found between the mean total arc of hip range of internal and external rotation in the elite tennis players with the dominant side being greater by a clinically insignificant mean value of 2.5 degrees. Significantly less (P <.005) dominant hip internal rotation and less dominant and nondominant hip total rotation range of motion were found in the professional baseball pitchers compared with the elite male tennis players.Conclusion: This study established typical range of motion patterns and identified bilaterally symmetric hip active range of motion rotation values in elite tennis players and professional baseball pitchers. Asymmetric hip joint rotational active range of motion encountered during clinical examination and screening may indicate abnormalities and would indicate the application of flexibility training, rehabilitation, and further evaluation.

Methods: A transversal descriptive study was performed in 40 patients (1 to 17 y.o.), 21 (52,5%) female and 19 (47,5%) male from Cruz Verde Association; 19 (47,5%) had history of prematurity; 26 (65,0%) with anoxia; meningitis in 4 (10%), hydrocephalus in 10(25,0%); microcephalus in 15 (37,5%). From the total 38 (95,0%) presented spasticity, 1 (2,5%) athetoid and 1 (2,5%) mixed. According to topographic classification 28 (70,0%) presented tetraparesis and 12 (30,0%) double hemiparesis. All of them are non-ambulators and 38 (95,0%) were classified as GFMCS V and 2 (5,0%) were level IV. The patients were divided in two groups (with or without indications for hip surgical treatment). The hips were analysed by Thomas, abduction, and Nelaton-Galeazzi tests. The scoliosis was clinically analysed considering trunk symmetry and dorsal hump. Results: We observed positive correlation between the patients scoliosis and hip flexion contracture. There was no difference between the patients age and and hip flexion contracture. There was no difference between topographic types and hip contractures. Conclusions: We did not find positive correlation between patients age and hip contractures; patients with scoliosis present worse hip abduction compromise.
- ItemSomente MetadadadosEstudo clínico e radiográfico do fêmur nas artroplastias totais do quadril com a haste não-cimentada de spotorno(Universidade Federal de São Paulo (UNIFESP), 1997) Trigueiro, Gustavo [UNIFESP]; Laredo Filho, José [UNIFESP]O autor avalia em estudo prospectivo clinico e radiografico, a haste de SPOTORNO, em 47 artroplastias totais do quadril nao cimentadas, realizadas em 40 pacientes. Todos os procedimentos foram realizados por via lateral e mesma tecnica cirurgica, padronizada pelo Setor de Patologias do Quadril Adulto da Disciplina de Ortopedia e Traumatologia do Departamento de Ortopedia e Traumatologia da Escola Paulista de Medicina - UNIFESP. Apos um seguimento medio de 6 anos e 1 mes, minimo de 5 anos e 3 meses e maximo de 7 anos, apresenta os resultados, onde a dor nao foi relatada como importante por nenhum paciente; em 43 (91,5%) artroplastias os pacientes estavam satisfeitos com o resultado da cirurgia; nao observou qualquer sinal de osteopenia em 11 (23,4%) exames radiograficos e nao houve formacao de pedestal em 36 (76,6%) femures. De acordo com a metodologia empregada 8 (17,0%) componentes femorais apresentavam sinais sugestivo de soltura e 39 (83%), das 47 artroplastias, foram consideradas satisfatorias

This raises the possibility that the muscle damage observed is due to ischiofemoral impingement. This diagnosis has been noted in middle-aged females either with or without a history of trauma or surgery, is a rarely described feature. We report here on a 31-year-old woman who presented with non-traumatic hip pain and evidence of narrowing of the ischiofemoral space and edema in the quadratus femoris. Nonsurgical treatment was administered, which relieved her hip pain. The diagnosis of ischiofemoral impingement should be considered in female patients complaining of hip pain without any other evident cause.

Methods: Measurements of the acetabulum, proximal femur, and pelvic incidence were made bilaterally on 106 professional soccer players, for a total of 212 hips. 38 individuals had the presence of bilateral cam deformities (alpha angle > 82), and 68 individuals had normal morphology of proximal fêmur (alpha angle < 68). Pelvic incidence and alpha angle were measured using standardized lateral radiographs of lumbar spine and pelvic anteroposterior radiographs, respectively. Independent Mann-Whitney tests were performed to evaluate for differences in measured parameters between groups. Results: The mean PI was 41,9 ± 2,5 for hips with a cam lesion and 49,4 ± 5,4 for normal hips, demonstrating a significant association between decreased PI and the presence of a cam lesion (p < 0,001). Conclusions: The results of our study show statistical association between reduced PI and increased prevalence of cam morphology.

Patients with PFPS usually experience weakness in the gluteal muscles, as well as pain and impaired motor control during activities of daily living. Strengthening the hip muscles is an effective way of treating this disorder. Neuromuscular training has also been identified as a therapeutic tool, although the benefits of this intervention in patients with PFPS patients remain inconclusive.Design: This is a protocol of randomized controlled trial with a blind assessor. Thirty-four women with a clinical diagnosis of PFPS participated. These participants were allocated into two groups (experimental and control). the experimental group performed twelve sessions to strengthen the knee extensors, hip abductor and lateral rotator muscles in association with neuromuscular training of the trunk and lower extremities. the control group performed the same number of sessions to strengthen the muscles of the hip and knee. the primary outcome was functional capacity (Anterior Knee Pain Scale - AKPS) at 4 weeks. Pain intensity, muscle strength and kinematic changes were also measured during the step down test after four weeks of intervention. Follow up assessments were conducted after three and six months to assess functional capacity and pain. the effects of the treatment (i.e. between-group differences) were calculated using mixed linear models.Discussion: the present study was initiated on the 1st of April 2013 and is currently in progress. the results of this study may introduce another effective technique of conservative treatment and could guide physical therapists in the clinical decision-making process for women with PFPS.

The patient was treated with resection of the lesion and presented a very good evolution with total recovery of the hip movements.
- ItemSomente MetadadadosPrevalência de displasia do desenvolvimento do quadril em crianças nascidas no Hospital Municipal e Maternidade Escola Dr. Mário de Moraes Altenfelder Silva(Universidade Federal de São Paulo (UNIFESP), 2020-09-24) Motta, Giovanna Galvao Braga [UNIFESP]; Iared, Wagner [UNIFESP]; Universidade Federal de São PauloBackground: There is no population study in the medical literature that has assessed the prevalence of Developmental Dysplasia of the Hip in Brazil. An estimate published in 1988 is a prevalence close to 1.1%, based on the meta-analysis and logistic regression protocols analyzed. Objective: To evaluate the prevalence of developmental dysplasia of the hip in a sample of population of the city of São Paulo, in children aged zero to three days, born at the Hospital and Maternity School Dr. Mário by Moraes Altenfelder Silva. Methods: We conducted an observational study to assess the prevalence of Developmental Dysplasia of the Hip diagnosed by the Graf method and to verify its correlation with the risk factors related to the literature. Results: A total of 678 children underwent an ultrasound examination of their hips (1356 hips). The prevalence of Developmental Dysplasia of the Hip in the group was 5.46% (37 children with Developmental Dysplasia of the Hip). Twenty-four children had unilateral dysplasia, with 3 on the right (8.1%) and 21 on the left (56.7%). 13 children had bilateral dysplasia. Of the 678 patients who comprised the sample, there were 356 females (52.5%), 364 white (53.7%), the rest composed of blacks, browns and yellows, 449 children born by natural birth (66%), 179 were in breech presentation at birth (26.4%), 302 were firstborn (44.5%), 19 had a positive family history of developmental dysplasia of the hip (2.8%), 4 twins (0.6%), 4 with positive Ortolani test (0.6%), no children with genetic or pathological syndrome. A logistic regression analysis registered odds ratios with statistical significance for the white with Odds Ratio (OR) of 2.561 [95% CI 1.07 to 6.11], multiparity with OR 3.50 [95% CI 1.62 to 7.38], female gender with OR 4.95 [95% CI 1.86 to 13,13] and breech presentation with OR of 2.03 [95% CI 1.01 to 4.11]. Conclusion: The study shows that the prevalence of developmental dysplasia of the hip in the sample is more than five times higher than the estimate in Brazil. The main risk factors associated with the highest risk of Developmental Dysplasia of Hip were female, breech presentation, firstborn and white race.

Recently, a group of people were reported, all female, with pain and swelling in the pelvic region.T2-weighted MRI showed increased signal in the enthesis of the iliotibial band (ITB) along the lower border of the iliac tubercle. We report a case of a 34 year old woman, non-professional runner, with pain at the iliac crest with no history of trauma and whose MRI was compatible with the proximal iliotibial band syndrome.

MATERIALS AND METHODS: Forty patients (17 symptomatic and 23 asymptomatic) were evaluated by 1.5 T magnetic resonance imaging (MRI) after intravenous contrast media injection. MRI findings were divided in inflammatory, degenerative, combined (inflammatory and degenerative) and extra-articular changes. RESULTS: The most frequent findings seen were degenerative and extra-articular changes (22.5% each). Inflammatory changes were seen in only five (12.5%) symptomatic patients, four of them with degenerative findings associated (10%). CONCLUSION: The use of MRI of the hip allowed the evaluation of the main articular abnormalities seen in patients with rheumatoid arthritis. The most frequent findings were degenerative and extra-articular changes. Inflammatory findings were seen in only five patients, four of them with degenerative changes associated.
