To assess the integration of an autologous composite fascia and fat graft implanted into the lamina propria of a porcine vocal fold using two different approaches.Study Design. An experimental prospective study on the porcine larynx was conducted at a tertiary research institution.Methods. An external cervical approach was used to expose the thyroid cartilage of 24 healthy minipigs under general anesthesia. the composite fascia/fat graft was implanted through two distinct approaches, transmuscular and submuscular. Animals were sacrificed at 7, 30, 90, and 180 days for macroscopic and histological study of the larynx. Graft integration and local inflammatory response were studied.Results. the survival rate of the experimental model was 100% and all animals had local inflammatory response to the surgical procedure. Only 41.7% of the grafts placed inside the thyroarytenoid (TA) muscle fibers were identified postmortem, whereas 83.3% of the submuscular grafts remained intact.Conclusions. Graft incorporation was better and there was less inflammation when the architecture of the TA muscle fibers was preserved. Graft extrusion was observed in the most of the cases where it was placed inside the muscle.
- ItemSomente MetadadadosInitial experience with 'inverted U' staged buccal mucosa graft (bracka) for hypospadias repair(Elsevier B.V., 2009-04-01) Barroso, Ubirajara; Macedo, Antonio; Universidade Federal da Bahia (UFBA); Roberto Santos Gen Hosp; Universidade Federal de São Paulo (UNIFESP)Objective: To describe a modification of the Bracka procedure ('inverted U'), used to enlarge the graft and diminish the risk of graft contracture compromising the subsequent urethral reconstruction.Material and methods: Inverted U Bracka procedure was performed in 10 children in whom previous hypospadias correction had failed. the surgical steps are identical to the original Bracka procedure. the only modification is reconfiguration to enlarge the graft.Results: Significant contraction of the graft was not seen in any patient and the second stage was performed uneventfully. After the second surgery there were two complications: one fistula and one wound infection.Conclusion: in this preliminary study, the modification avoided significant contracture of the graft giving a more predictable size of the neourethra for the second-stage procedure. Objective: Introduce an innovation in the instrument used to harvest fat graft to use in patients with breast cancer and sent to Instituto Nacional de Propriedade Industrial as utility model patent. Method: Search of anteriority and trademarks were performed at internationals and nationals databases, and a prototype was built as a fat harvest instrument with 35 cm length, 4 cm of diameter, 10 holes at the distal extremity, with 2mm diameter each. It was performed an experiment with harvest of a viscous substance, comparing the prototype with 2 regular industry cannulas type: 3 holes Mercedes’s type and 5 roles Pitangy’s type. Results: Four patents of high relevance, and 1 patent of media relevance were founded, and were different from the prototype propose in the distal extremity design or harvest function. When the efficiency of the prototype was compared with the other cannulas, the cannula with 5 holes Pitangy’s type was the most efficient, and it was not founded statistic difference between the prototype and the cannula with 3 roles Mercedes’s type. Conclusion: A prototype with 10 holes at the distal extremity with 2 mm each was built, developed for an efficient harvest of fat blocks until 2mm of diameter, to use in breast reconstruction.

This Treatment Is Indicated To, Among Others: Correction Of Cleft Lip, Traumas, Granulomatous Diseases And Functional And Aesthetic Improvements. The Procedure To Obtain Grafts Increases Local Comorbidity And Surgery Duration. Therefore, It Is Necessary To Rationalize And Optimize The Use Of Graft By Creating A Proper Surgical Plan And Dexterity To Use It. The Tools Used To Shape The Cartilaginous Grafts Are Currently Rudimental And Do Not Provide Precise Cuts. Aim: Developing Tweezers To Help The Process Of Precise Cuts Of Cartilaginous Grafts. Methods: Searching For Other Studies In Databanks Such As Uspto, Espacenet, Wipo, And Google Patent Search. To Develop The Tool, We Designed Surgical Forceps That Helps Gripping, Measuring And Cutting The Cartilage.
