Its pathogenesis is not totally defined yet: it is usually more frequent in middle-aged, male obese patients and it is associated with systemic disorders such as obstructive sleep apnea, high blood pressure and diabetes. On the occasions which conservative treatment fails, surgical procedures present good results, including surgical techniques which are constantly evolving.

Methods: Review of the clinicopathological data and surgical techniques used to treat these patients. Results: Cure of the lesions with good esthetic and functional results were obtained in all cases. Conclusions: Surgical excision of the divided nevus of the eyelid in more than one surgical session using flaps from the adjacent skin, and in one case, a free skin graft from the contralateral superior lid, was curative in the studied three cases.

Amyloid cutaneous infiltrative lesions like papules, nodules, or plaques with a serous-hemorrhagic aspect were found in the eyelids, neck and retroauricular region, among others. She had presented intermittent papular lesions on the upper eyelids one year before, which worsened following local trauma. A local skin biopsy showed amorphous and eosinophilic substance in the dermis. Congo red staining confirmed the amyloid deposits. Abnormal exams: proteinuria (570mg/24h), Bence-Jones proteinuria and clonal plasma cells (70%) found in myelogram. Following the diagnosis of multiple myeloma based on amyloid skin lesions, the patient was referred to the Hematology service and died 5 months after the diagnosis.

Cryptophthalmos can be an isolated finding (that has been reported as an autosomal dominant trait) or associated with other congenital anomalies (reported as an autosomal recessive disorder). RMSA, female, 3 m.o., evaluated in the general clinic of the Instituto Brasileiro de Oftalmologia e Prevenção à Cegueira. Child of consanguineous parents. The same finding was observed in an uncle and one of her brothers. Her physical examination showed total unilateral cryptophthalmos (right side), depressed nasal bridge, low set ears, atresia of the external auditory canal, enlarged clitoris, prominent labia majora and syndactyly of the fingers and toes. Ultrasonography of the abdomen showed renal agenesis (left side). Ocular ultrasonography showed a reduced anterior-posterior ocular diameter, anterior segment disorganization, absence of the lens and total retinal detachment in the right eye. The pathogenesis of cryptophthalmia has not as yet been determined, but consanguinity has been reported by many authors as a very important factor. Doctors should be attentive to the clinical findings and the correct diagnosis in order to offer these patients a thorough follow-up and realistic genetic counseling to their parents.
- ItemAcesso aberto (Open Access)Siringomas periorbitários - excisão com tesoura de castroviejo: experiência em 38 pacientes e revisão da literatura(Sociedade Brasileira de Dermatologia, 2006-08-01) Bagatin, Edileia [UNIFESP]; Enokiahara, Mauro Yoshiaki [UNIFESP]; Souza, Patricia Karla De [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)BACKGROUND: Syringomas are benign adnexal tumors from intraepidermal eccrine ducts, treated by diverse surgical modalities with variable results. OBJECTIVES: 1. To report our experience in surgical treatment of multiple periorbital syringomas excised by Castroviejo scissors followed by healing by secondary intention. 2. To present a literature review with emphasis on therapeutic aspects. MATERIAL AND METHOD: During 68 months we treated 38 cases of periorbital syringoma. All of them were submitted to surgical excision using Castroviejo scissors, followed by healing by secondary intention. Evaluation was done 7, 30 and 90 days after surgery. The mean follow-up period was 33 months, ranging from 3 to 62 months. RESULTS: The results were excellent in 24 patients (63.1%), good in 12 patients (31.6%) and regular in two patients (5.3%). The only immediate complication observed was edema; late complications were hypochromia (12 patients), depressed scar (one patient) and hypertrophic scar (one patient). Recurrences were not observed during the follow-up period. CONCLUSIONS: We concluded from our results that excision of periorbital syringomas by Castroviejo scissors followed by healing by secondary intention is a low cost, straightforward outpatient procedure. Results are good, with no recurrences, although transient hypochromia may occur.

There are few cases reported in the literature, and we could only identify one description of trichoadenoma occurring in the eyelid area. We describe the case of a patient with trichoadenoma in the outer corner of the lower eyelid that we treated with surgical excision associated with blepharoplasty.
